Community health nursing visits for at-risk women and infants.

The purpose of this study was to utilize the Barnard Model of parent-infant interaction and instruments from the Barnard Newborn Nursing Models study to assess 30 pregnancy clients and to help determine appropriate interventions. Upon consent, the 30 study participants were randomly assigned to one of three groups: primiparous, unmarried, or otherwise socially at-risk. Thirty-three percent of the participants were under 19 years of age. A doctorally prepared community health nurse (CHN) completed all interventions, including a comprehensive assessment. Over 20% of participants admitted mild to

Maternal postpartum mental health negatively affects infants' health related quality of life.

OBJECTIVES: To assess the association of early and late postpartum maternal mental health with infants' health related quality of life (HRQoL). METHODS: The study was embedded within the POST-UP trial (n = 1843). Infants' HRQoL was assessed with the Infant and Toddler Quality of Life Questionnaire Short Form-47 at ages 1 month (1 m), and 12 m. Maternal mental health regarded postpartum depression (PPD), measured with the Edinburgh Postnatal Depression Scale at 1 m and 6 m (early/late), and postpartum anxiety (PPA) with the short version of the state form of the Spielberger State-Trait Anxiety

The AWHONN Near-Term Infant Initiative: a conceptual framework for optimizing health for near-term infants.

In June 2005, the Association of Women's Health, Obstetric and Neonatal Nurses launched a multiyear initiative to address the unique physiologic and developmental needs of near-term infants (NTIs) defined as those born between 34 and 37 weeks post-menstrual age. The Optimizing Care for the Near-Term Infant Conceptual Model integrates the concepts of neonatal physiologic functional status, nursing care practices, care environment, and the essential role of the family both in the hospital and beyond. The elements of the model will serve to guide program and resource development within the Near-T

Macro-economic conditions and infant health: a changing relationship for black and white infants in the United States.

We study whether the relationship between the state unemployment rate at the time of conception and infant health, infant mortality and maternal characteristics in the United States has changed over the years 1980-2004. We use microdata on births and deaths for years 1980-2004 and find that the relationship between the state unemployment rate at the time of conception and infant mortality and birthweight changes over time and is stronger for blacks than whites. For years 1980-1989 increases in the state unemployment rate are associated with a decline in infant mortality among blacks, an effect

Predicting Infant Sleep Patterns From Postpartum Maternal Mental Health Measures: Machine Learning Approach.

BACKGROUND: Postpartum maternal mental health (MMH) symptoms, including depression, anxiety, and childbirth-related post-traumatic stress disorder, are known to influence infant sleep trajectories. While previous research has examined their individual and combined associations, the predictive utility of these MMH symptoms for the early identification of infant sleep problems through machine learning (ML) remains understudied. OBJECTIVE: This study aimed to examine whether postpartum MMH measures can predict infant sleep outcomes during the first year of life. The analysis focused on 2 clinica

Maternal Tuberculosis and Infant Gut and Immune Development: Implications for Maternal-Child Health.

BACKGROUND: Maternal tuberculosis (TB) during pregnancy involves chronic infection, immune activation, and antimicrobial therapy, which may alter the maternal gut, vaginal, and breast milk microbiomes and influence neonatal immune development. METHODS: We conducted a narrative review of studies identified through PubMed and GoogleScholar addressing the maternal TB-microbiome-infant immunity axis. Where evidence from pregnant women with TB was limited, findings were extrapolated from non-pregnant TB populations, antibiotic exposure studies, and broader maternal-infant microbiome research. RES

Significance of perinatal trauma screening and care coordination to improve infant health outcomes.

This commentary addresses the need for trauma-informed screening and coordinated mental health care across obstetric (OB) and neonatal intensive care unit (NICU) settings. Perinatal post-traumatic stress disorder (PTSD) and related mental health conditions are common yet under-recognized, particularly among individuals exposed to medical trauma, prior adversity, or interpersonal violence. Perinatal PTSD prevalence estimates range from 3-6 % in community samples and 15-25 % in high-risk populations, including NICU parents, individuals with obstetric complications, and those with prior trauma hi

Maternal and infant health during the COVID-19 pandemic - A Pennsylvania Study Protocol.

BACKGROUND: Pregnant people are vulnerable to more severe outcomes of COVID-19 compared with their non-pregnant counterparts. Research is needed to systematically test the degree to which COVID-19 during pregnancy increases the risk for adverse maternal, perinatal, and infant health and development outcomes and whether social determinants of health or psychological/psychosocial health outcomes confound or intensify the risk. This protocol paper describes a prospective cohort study using electronic health record (EHR) and patient-reported data from four large health systems in Pennsylvania to e

Maternal perspectives on poor mental health and the impact on mother-infant relationships.

AIM: The high prevalence of trauma, anxiety, and depression among postpartum women, particularly in low-income groups, poses a global health challenge. These mental health issues can strain the mother-infant relationship and negatively affect the infant's socio-emotional and cognitive development, with broader societal implications. This study aims to explore South African postpartum mothers' perceptions of mental health and their influence on mother-infant bonding to inform targeted support strategies. METHODS: Fifteen participants with infants under 4 months old were recruited from Soweto,

Infant fat mass and later child and adolescent health outcomes: a systematic review.

OBJECTIVE: Obesity and excess adiposity are leading causes of metabolic and cardiovascular morbidity and mortality. Early identification of individuals at risk is key for preventive strategies. We examined the relationship between infant body composition (0-2 years of age) and later (>2 years) health outcomes using a systematic review. DESIGN: We preregistered the study on PROSPERO (ID 288013) and searched Embase, PubMed and Cochrane databases for English language publications using the Medical Subject Headings (MeSH) terms 'infant' and 'body composition' and 'risk' between January 1946 and F

County-Level Structural Vulnerabilities in Maternal Health and Geographic Variation in Infant Mortality.

OBJECTIVE: To evaluate whether community factors that differentially affect the health of pregnant people contribute to geographic differences in infant mortality across the US. STUDY DESIGN: This retrospective cohort study sought to characterize the association of a novel composite measure of county-level maternal structural vulnerabilities, the Maternal Vulnerability Index (MVI), with risk of infant death. We evaluated 11 456 232 singleton infants born at 22 0 of 7 through 44 6 of 7 weeks' gestation from 2012 to 2014. Using county-level MVI, which ranges from 0 to 100, multivariable mixed e

Association of self-reported mother-infant relationship with child and adolescent mental health.

BACKGROUND: The quality of the relationship between mother and infant may have profound implications for the development of a child. Early indicators of psychological vulnerability may allow targeting of support for the child's cognitive, emotional and social development. A challenging mother-infant relationship could be one indicator of risk. AIMS: This study examined variations in psychological well-being and psychopathology among boys and girls according to early maternal perception of the mother-infant relationship. METHOD: This study is based on 64 663 mother-infant pairs from the Danis

The effects of pregnancy-related Medicaid expansions on maternal, infant, and child health.

Prior research has examined how late 1980s pregnancy-related Medicaid eligibility expansions influenced outcomes around the time of birth and, more recently, adult outcomes. We offer a close examination of early childhood effects to better understand the mechanism(s) underlying the improved longer-term outcomes. The restricted-access National Maternal and Infant Health Survey allows us to explore the effects of these expansions on maternal and child outcomes near the time of birth as well as three years post-birth. Our evidence suggests earlier connection with prenatal care and possible modest

Health practitioners' recognition and management of postpartum obsessive-compulsive thoughts of infant harm.

The postpartum period has been associated with elevated rates of onset of obsessive-compulsive disorder (OCD) among women, with a prevalence of 2-9%. Postpartum OCD is often characterized by recurrent, unwanted, and highly distressing thoughts, images, or impulses of deliberate infant harm. This study investigated health practitioners' recognition of, and clinical management strategies for, postpartum obsessive-compulsive symptoms (OCS). Ninety-four perinatal health practitioners from a range of disciplines and professional backgrounds completed a survey comprised of a hypothetical case vignet
