Endoscopic thoracic sympathectomy (ETS) is the preferred surgery for treatment of intractable palmar hyperhidrosis (PH). General anesthesia with onelung collapsed ventilation (OLCV) using single-lumen tracheal tube (SLT), is our preferred anesthetic technique for ETS. Intrapleural CO(2) insufflation (capnothorax) was used to ensure lung collapse. The current study examined the effects of capnothorax on dynamic lung compliance (DLC) of the ventilated lung during ETS. After obtaining written informed consent, 10 adult male patients ASA I&II undergoing ETS were studied. Their average age and weig

Bronchial mucous plugging is the main precipitating factor of acute respiratory failure for patients with neuromuscular disease. Manually assisted coughing and mechanical insufflation-exsufflation can effectively eliminate airway secretions without resorting to tracheal intubation provided that bulbar muscle function is sufficient to permit assisted peak cough flows of 160 l/min or greater. We now report successful use of mechanical insufflation-exsufflation to prevent acute respiratory failure for a patient with amyotrophic lateral sclerosis with no measurable peak cough flow.

BACKGROUND: Air embolism is a rare but potentially fatal complication of laparoscopic surgery, especially among pediatric patients. A common theory as to why air embolism occurs during laparoscopy is that ambient air in the insufflation tubing can enter the vascular system when a trocar or Veress needle is inserted into a vessel or highly vascularized tissue. While some laparoscopic surgeons initiate CO2 insufflation before attaching the tubing, there are no guidelines that recommend a delay after starting the flow of CO2 and before connecting the insufflation tubing. We therefore assessed thi

Background: Conventional operative insufflation uses a one-way trocar to handle instruments while maintaining pneumoperitoneum. In 2007, the AirSeal® valveless trocar insufflation system was introduced, which maintains stable pneumoperitoneum while continuously evacuating smoke. Although this device has been validated in adult patients, it has not been extensively validated in the pediatric population. Materials and Methods: A retrospective cohort study of pediatric urology patients aged 0 to 21 who underwent laparoscopic pyeloplasty between March 2016 and October 2021 was performed. Intraoper

Delivery of inhalational dry powders (DPs) to the lung of mice is pivotal for pre-clinical pharmacokinetic and pharmacodynamic investigations. Although several devices have been reported, their application is always limited by many factors, including complicated design, high price, commercially discontinued status, as well as requirement of special skills. Here, we have introduced a simple device for non-invasive and precise delivery of DPs in mice. We set up the self-made device using a 20 G cannula tube and a 1 mL syringe. Subsequently, it was validated in terms for proper installation, deli

In April 2014, the US Food and Drug Administration published a safety communication warning of the risk of an unsuspected uterine sarcoma being morcellated during a laparoscopic procedure and therefore advising against the use of power morcellation. These statements have encouraged the scientific community to look for new techniques that allows performing this procedure in a safer way, decreasing the risk of malignant dissemination thorough the abdominal cavity. We describe a new technique for power morcellation using a plastic bag through umbilicus using a latex glove and skin retractor as a

BACKGROUND AND OBJECTIVES: Within recent years, the insufflation technique for laparoscopy has become more important with high flow insufflators (> or = 30 L/min) and high gas turn over (> or = 800 L/procedure). Increased amounts of carbon dioxide (CO2) gas used can lead to laparoscopic hypothermia. We studied the insufflator with versus insufflators without internal gas heating (inside insufflator) as a sufficient method of hypothermia prevention at different flow rates.
METHODS: With a computer-based data acquisition model, different standard insufflators with internal gas heating (Snowden

BACKGROUND: Invasively ventilated, critically ill children are at risk of retained respiratory secretions, which can obstruct airways, impair gas exchange, and cause lung collapse or ventilator-acquired pneumonia. Endotracheal suctioning partially removes secretions, but additional airway clearance interventions may be required. Mechanical insufflation-exsufflation (MI-E), is a cough augmentation device used to improve secretion clearance in adult intensive care, however it is not well characterised in paediatrics.
AIM: To explore MI-E use in invasively ventilated, critically ill children.

OBJECTIVES: To determine the accuracy of emergency physicians (EPs) in diagnosing pneumoperitoneum with POCUS, and if the volume of pneumoperitoneum affects accuracy.
METHODS: POCUS clips were obtained from patients undergoing intraperitoneal insufflation for an elective laparoscopic procedure. Video clips of the right upper quadrant and epigastric regions were obtained prior to insufflation and then after 500 ml, 1000 ml, and 1500 ml of insufflation. These clips were randomized and reviewed by three blinded ultrasound-trained EPs. For each clip they determined whether pneumoperitoneum was pr

BACKGROUND: Cecal perforation from barotrauma is a rare complication of colonoscopy, even with the use of CO₂ insufflation, which is generally preferred over room air due to its rapid absorption and reduced patient discomfort.
CASE REPORT: A 64-year-old woman with hypertension and dyslipidemia underwent a routine colonoscopy with CO₂ insufflation. Multiple diverticula were noted, and upon reaching the cecum, several parallel linear lesions with spontaneous bleeding, consistent with "cat scratch" colon, were observed. Hemostasis was achieved with 10 endoscopic clips. A subsequent CT scan revea

We report selected insufflation technique using direct bronchial insufflation to visualize the intersegmental plane during total thoracoscopic segmentectomy. Following the transection of the bronchus using a stapler, a small incision was created in the dissected target bronchus, and direct air insufflation was performed at the small incision site. The target segment was inflated, while the preserved segments appeared to collapse, and a demarcating line was visualized between the inflated and deflated lung parenchyma. This technique quickly identifies the anatomic intersegmental plane without w

BACKGROUND: Mechanical insufflation/exsufflation (MI-E) devices are often prescribed to patients with inefficient cough and recurrent infections, but their use in the home setting is not well characterized.
OBJECTIVE: The objective of this study was to report a real-life experience and identify factors that are associated with home MI-E use in adult patients.
METHODS: This is a cross-sectional observational study of adult subjects with neurological disease using MI-E at home for more than 3 months.
RESULTS: A total of 43 patients were included. Median age (interquartile range) was 48 (31-64

We report the long-term outcomes of a case of prenatal gastroschisis repair using a fully percutaneous fetoscopic approach with partial carbon dioxide insufflation. Surgery was performed as an experimental procedure before the scheduled elective birth. The fetal intestines were successfully returned to the abdominal cavity without any fetal or maternal complications. Ultrasonography performed 24 hours later revealed bowel peristalsis and no signs of fetal distress. After 48 hours, partial extrusion of the small bowel was observed, and the fetus was delivered. Gastroschisis repair was immediate

BACKGROUND: Atelectasis is a condition characterized by the collapse and nonaeration of lung regions and is considered a manifestation of an underlying disease process. The goal of atelectasis treatment is the restoration of volume loss. In the range of different treatment options, chest physiotherapy is often used as a first-line approach, and some cases require bronchoscopic interventions.
METHODS: In this case series, we describe a modified bronchoscopic treatment procedure using pressure-controlled bronchoscopic segmental insufflation with surfactant application.
RESULTS: The proposed ap

BACKGROUND: Optimal visualization and safety have always been essential in performing any type of endoscopic surgery. However, the safety of automatic gastrointestinal (GI) insufflation has yet to be thoroughly studied, especially when combined with manual insufflation. The current study aimed to verify whether the pressure limiter could lower GI endoluminal pressure during endoscopic procedures and affect the behavioral patterns of endoscopists.
METHODS: A preclinical blinded trial was conducted on endoscopists who had no knowledge regarding the presence of the pressure limiter that prevents

BACKGROUND: Esophageal atresia and tracheo-esophageal fistula (TEF), a well described congenital anomaly of the aero-digestive tract, commonly presents with inability to swallow and feed immediately after birth. However, diagnosis of recurrent or isolated TEF can be challenging and requires a combination of endoscopic and contrast studies. We describe a hitherto unreported technique of low flow intermittent oxygen insufflation into the suspicious tract and examine its safety and diagnostic yield for identification of occult TEF.
METHODS: A retrospective single center cohort study, analyzing c

BACKGROUND: In electroconvulsive therapy (ECT), a generalized seizure is provoked under general anesthesia. Standard airway management is done by face mask ventilation, but this is difficult during generalized seizure. Transnasal humidified rapid-insufflation ventilatory exchange (THRIVE) is a method to increase apnea tolerance and permit apnea oxygenation. We hypothesize that THRIVE can oxygenate the patient during ECT and that it is comfortable for patients.
METHODS: In this single-center feasibility study, 13 patients with a pharmacotherapy-resistant depressive disorder underwent 20 ECT se
