The artificial endocrine pancreas in clinical practice and research. The present position and future perspectives.

The artificial endocrine pancreas (AEP) is a controlled glucose and/or insulin infusion system in which continuously monitored blood sugar values are fed to a computerised analyser that uses predetermined algorithms to establish the doses to be administered. Since its first appearance in clinical practice and diabetological research during the Sixties, the AEP has been modified in various ways to overcome technical problems associated with the gluco-sensor and algorithms so as to make better use of the glucose-insulin feedback mechanism, and hence obtain a closer correspondence to physiologica

An Integrated Visualization of Continuous Glucose and Insulin Pump Data for Patients.

Continuous glucose monitoring (CGM) and insulin pump technologies generate extensive physiological and therapeutic data. Most commercial platforms display data streams separately, requiring users to cognitively integrate events that influence glycemic outcomes. This segmentation may obscure the relationships among glucose fluctuations, insulin administration, and meal patterns, potentially impairing decision-making. This project employed the UFuRT framework to create a unified visualization that consolidates glucose traces, bolus events, basal profiles, and carbohydrate intake into a single ti

Impact of Inaccurate Meal Announcements on Glycaemia Control with or without an Artificial Pancreas.

Type 1 Diabetes (T1D) therapy's goal is to maintain glycaemia in the safe euglycaemic range. Recent technological advancements help the patient in this hard task: Sensor Augmented Pump (SAP) systems combine an insulin pump for continuous subcutaneous insulin infusion with a glucose sensor. Nevertheless, the patient remains in charge of insulin dosing. Artificial Pancreas (AP) is a more advanced technology, where a closed-loop control algorithm automatically adjust the pump insulin infusion based on the sensor feedback. With SAP and with most of the available AP systems the patient is requested

A system for automatic feedback control of plasma potassium concentration.

A system is described for automatic feedback control of plasma potassium concentration in experimental animals. Plasma potassium was monitored continuously and the signal compared with the desired plasma potassium concentration. The resulting error signal controlled the infusion rates of a concentrated potassium chloride solution (50 or 200 mmol litre-1) and a solution of 50% glucose with insulin 200 u litre-1. Plasma potassium was increased or decreased to the desired concentration at various rates, dictated by the controller constants. Increases were achieved significantly faster than reduct

The effect of hyperglucagonemia on blood glucose concentrations and on insulin requirements in insulin-requiring diabetes mellitus.

The effect of elevated glucagon concentrations on insulin requirements and on blood glucose concentrations was studied in five insulin-requiring diabetic subjects during feedback control of hyperglycemia with an automated glucose-controlled insulin infusion system (artificial endocrine pancreas) for six to eight hours. Two levels of hyperglucagonemia were induced by means of constant intravenous infusion. Raising plasma glucagon concentrations to levels reported in poorly controlled diabetics (450 to 665 pg. per milliliter) did not alter total insulin requirements or blood glucose concentratio

CONCENTRATED INSULINS: CLINICAL UPDATE OF THERAPEUTIC OPTIONS.

Improved glycemic control is associated with a reduced risk of diabetic complications. Optimal management of patients with type 2 diabetes includes nutritional therapy, physical activity, and pharmacotherapy for glycemic control. Most patients with type 2 diabetes are initially managed with oral antidiabetic agents, but as β-cell function declines and the disease progresses, insulin therapy is frequently needed to maintain glycemic control. Insulin therapy given with multidose insulin injection regimen or by continuous insulin infusion is needed for patients with type 1 diabetes to achieve con

The gastrointestinal stimulus to insulin release. I. Secretin.

The gastrointestinal stimulus to the release of insulin has been investigated in man by the use of a radioimmunoassay for secretin. Serum secretin levels rose rapidly after the oral ingestion of glucose or protein and preceded the elevation of serum insulin. An intravenous infusion of highly purified secretin caused a release of insulin when the serum secretin levels were within the physiological range. Infusion of hydrochloric acid into the duodenum caused an elevation of serum secretin and serum insulin levels in normal subjects. A similar response of secretin and insulin was seen after intr

Moving toward the ideal insulin for insulin pumps.

Advances in insulin formulations have been important for diabetes management and achieving optimal glycemic control. Rapid-acting insulin analogs provide a faster time-action profile than regular insulin and are approved for use in pumps. However, the need remains for therapy to deliver a more physiologic insulin profile. New insulin formulations and delivery methods are in development, with the aim of accelerating insulin absorption to accomplish ultra-fast-acting insulin time-action profiles. Furthermore, the integration of continuous glucose monitoring with insulin pump therapy enables on-g

[Lipoatrophic diabetes with acanthosis nigricans. Prolonged blood glucose normalization by continuous infusion of insulin].

Insulin resistance is a permanent feature of lipoatrophic diabetes, the resistance being almost regularly stubborn. We report the case of a 23-year old unmarried woman with generalized lipoatrophy and Acanthosis nigricans. Seven years after a diabetes resistant to all treatments was diagnosed, blood glucose levels were permanently around 25 mmol/l. Multiple and severe micro- and macroangiopathies were present. Partial resistance to insulin was demonstrated. This resistance could not be explained by abnormalities in anti-insulin hormones nor by a decrease in the number or affinity of insulin re

Updates in the Management of Type 1 Diabetes in Pregnancy.

The care of pregnant individuals with type 1 diabetes mellitus has experienced significant advancements in recent years. Preconception counseling has re-emerged as a core dimension of management. Continuous glucose monitoring plays an increasingly useful and beneficial role in gestational glycemic monitoring, a practice informed by improved maternofetal outcomes. While studies have not shown that continuous subcutaneous insulin infusion is superior to multiple daily injections of insulin for glycemic control, recent work has signaled that hybrid closed-loop systems with pregnancy-specific targ

[Improvement in the treatment of diabetes by means of a portable insulin-infusion apparatus. Preliminary results in 4 unstable juvenile diabetics].

Four type 1 diabetics, 10 to 21 years after onset of diabetes and with no detectable levels of plasma C-peptide, were kept on intravenous insulin for 4 months by a portable open-loop insulin infusion system (Siemens, Germany). Using this system, a constant basal rate of insulin was continuously infused into the superior vena cava. During meals additional insulin was delivered for one hour. The patients went home after a few days on the ward, during which time the doses of insulin (basal and extra rates) were established. All four patients maintained the same diet, went to work and continued th

Selective alpha 2 receptor blockade facilitates the insulin response to adrenaline but not to glucose in man.

The adrenergic nervous system plays an important role in the control of insulin release and animal work suggests that this is mediated by way of alpha 2 adrenoceptors. A specific alpha 2 adrenoceptor antagonist (idazoxan) is now available for use in man and we have studied its effects on insulin release in normal volunteers (a) during the infusion of adrenaline (0.05 micrograms/kg/min) and (b) after an intravenous dose of glucose (20 g). The infusion of adrenaline alone had no significant effect on insulin release while in the presence of idazoxan, insulin release was markedly stimulated by ad

Role of endogenous insulin in pancreatic secretion in rats.

BACKGROUND: Pancreatic exocrine tissue is influenced by islet hormones through the islet-acinar portal system. We investigated the role of endogenous insulin in pancreatic secretion in vivo in rats. METHODS: Anesthetized rats were prepared by cannulation of the pancreatic duct, and pure pancreatic juice was collected. Pancreatic secretion was stimulated by intravenous infusion of cholecystokinin (CCK) (0.06 microgram/kg/h) with or without intravenous infusion of glucose (0.25 and 0.5 g/kg/h). The effect of intravenous infusion of glucose (0.5 g/kg/h) on pancreatic secretion stimulated by intr

Insulin-mediated hypokalemia and paralysis in familial hypokalemic periodic paralysis.

To elucidate a potential role for insulin-mediated extra-renal potassium disposal in the clinical syndrome of hypokalemic periodic paralysis, an obese affected man was studied using the euglycemic insulin clamp, which, in normal and obese subjects, produces predictable, insulin dose-dependent declines in plasma potassium levels. During a 20 mU/m2/minute euglycemic clamp (insulin level, 88 microU/ml) procedure, while the patient with hypokalemic periodic paralysis demonstrated severe resistance to insulin-mediated glucose uptake (glucose uptake 50 percent of that of normal control subjects, n =
