Healthcare reform: the good, the bad, and the transformational.

Aspects of the recent landmark healthcare reform legislation likely to prove beneficial to hospitals include changes to insurance markets, malpractice reform demonstrations, and funding to help hospitals with high volumes of preventable readmissions. Hospitals are likely to be adversely affected by scheduled payment cuts and the employer "free rider" penalty. The legislation also poses transformational opportunities and challenges for hospitals, stemming from measures designed to change the care delivery system and provisions regarding hospitals' tax-exempt status and pricing transparency.

The cost of rehabilitation after critical illness: a comparison of hospitalization costs for traumatic brain injury and non-traumatic brain injury patients with disorders of consciousness.

BACKGROUND: This study aims to compare hospitalization costs between traumatic brain injury (TBI) and non-traumatic brain injury (non-TBI) patients with disorders of consciousness (DoC) to explore cost determinants. METHODS: A retrospective analysis was conducted on 210 DoC inpatients admitted to Shenzhen Longcheng Hospital, a tertiary rehabilitation hospital located in China's Pearl River Delta region, between 2015 and 2020. Patients were categorized into TBI (n = 44) and non-TBI (n = 166) groups based on etiology. Demographic, clinical, and hospitalization cost data were collected for each

Hirsutism is associated with increased hospitalization for mental health disorders.

Hirsutism is a common condition characterized by excess hair growth and may lead to psychosocial distress and substantial mental health (MH) burden. These psychosocial manifestations may lead to MH emergencies and hospitalization. However, the prevalence and predictors of MH hospitalization associated with hirsutism are not well-understood. The objective of this study was to determine the likelihood and associations of MH hospitalization among females in the US with hirsutism. Data were analyzed from female inpatients in the 2005-2014 National Inpatient Sample, a representative sample of US ho

Comparison of the Effectiveness of Baloxavir and Oseltamivir in Outpatients With Influenza B.

This retrospective cohort study analyzed data from a Japanese health insurance database to assess the effectiveness of baloxavir (n = 4822) for preventing severe events compared with oseltamivir (n = 10,523) in patients with influenza B. The primary endpoint was hospitalization incidence (Days 2-14). The secondary endpoints included intravenous antibacterial drug use, pneumonia hospitalization, heart failure hospitalization, inhalational oxygen requirement, and use of other anti-influenza drugs. The hospitalization incidence was significantly lower with baloxavir (0.15% vs. 0.37%; risk ratio:

Financial impact of emergency hand trauma on the health care system.

BACKGROUND: The US has a crisis of insufficient emergency coverage for hand trauma. One of the problems is the perceived financial loss associated with caring for this population. We evaluated the financial impact of treating emergency hand trauma patients on an academic medical practice and health care system. STUDY DESIGN: We examined billing records for 2,632 hand patients seen in the emergency department in 2005 at the University of Michigan. Financial data were separated into inpatient professional and facility revenues and costs. Professional net revenue was calculated by applying actua

How high are the costs of eating disorders - anorexia nervosa and bulimia nervosa - for German society?

Anorexia and bulimia are relatively rare psychogenic illnesses, which nevertheless are of great concern for society since they affect a mostly very young population and are accompanied by a significantly raised mortality risk. This cost-of-illness analysis for eating disorders in Germany considers hospitalization, rehabilitation services, and indirect costs through inability to work and premature death. The cost estimates are based on projections derived from benefit data as listed by health insurance schemes and pension insurance schemes and from epidemiological studies on the prevalence of e

BACKGROUND/PURPOSE: In 2014, New York City implemented the Affordable Care Act (ACA) leading to insurance coverage gains intended to reduce inequities in healthcare services use. The paper documents inequalities in coronary revascularization procedures (percutaneous coronary intervention and coronary artery bypass grafting) usage by race/ethnicity, gender, insurance type, and income before and after the implementation of the ACA. METHODS: We used data from the Healthcare Cost and Utilization Project to identify NYC patients hospitalized with the diagnosis of coronary artery disease (CAD) and/

COVID-19 infection in patients with end-stage kidney disease undergoing renal replacement therapies in Korea.

BACKGROUND: The global coronavirus disease 2019 (COVID-19) pandemic has placed patients with end-stage kidney disease (ESKD) at heightened risk owing to their vulnerability to infections. Our study focused on patients with ESKD, examining COVID-19 incidence, hospitalization, and mortality in relation to their renal replacement therapy (RRT) type and identifying factors influencing COVID-19 hospitalization. METHODS: We conducted a retrospective cohort study using health insurance claims data from the Health Insurance Review and Assessment Service for patients with ESKD between July 2017 and Ju

Association of Socioeconomic Position with Incident Hypertension Hospitalization and Blood Pressure Control among Participants in the Coronary Artery Risk Development in Young Adults Study.

BACKGROUND: The rate of hypertension hospitalizations is increasing among US adults. Individuals with low socioeconomic position are more likely to have high blood pressure (BP), which may increase their risk of hypertension hospitalization and adverse post-discharge outcomes. METHODS: We analyzed data from the Coronary Artery Risk Development in Young Adults (CARDIA) cohort study, which enrolled 5115 adults aged 18 to 30 years from 4 urban US communities in 1985-1986. Hospitalizations were identified by self-report during study exams and annual interviews, with hypertension hospitalizations

Association of health insurance status with coronary risk factors, coronary artery disease, interventions and outcomes in India.

OBJECTIVE: Coronary artery disease (CAD) related hospitalization and interventions are associated with catastrophic out-of-pocket health expenditure in India. To evaluate differences in risk factors, disease severity, management and outcomes in uninsured vs insured CAD patients we performed a study. METHODS: Successive CAD patients who underwent percutaneous intervention (PCI) at our centre were enrolled from January 2018 to June 2021. Clinical, angiographic and intervention data were periodically uploaded in the American College of Cardiology CathPCI platform. Descriptive statistics are repo

Patient transfers to a public hospital.

This is a study of interhospital patient transfers to a medium-sized county sponsored hospital. During the three-month period studied, there were 416 patients who met the study criteria. Thirty-eight (9%) of the patients were transferred because the county hospital offered a higher level of medical care or medical services not available at the presenting facility, while the remaining 378 (91%) were transferred because of their health insurance status. Two hundred and ninety-six patients (71%) had no medical insurance of any kind, 117 (28%) had government sponsored health insurance, and three (

Absence of health insurance is associated with decreased life expectancy in patients with cystic fibrosis.

Life expectancy for individuals with cystic fibrosis (CF) has increased dramatically in the last 30 yr, but it is unclear whether the improved survival has applied equally to individuals with different health insurance status. We developed a retrospective inception cohort of all 189 patients with CF born 1/1/55 to 12/31/70 who had at least one hospitalization at a university referral center. The median survival for patients with CF who were without health insurance was 6.1 yr compared with 20.5 yr for those with Medicaid and 20.5 yr for those with private insurance.
