Factors related to and economic implications of inhospital death in German lung cancer patients - results of a Nationwide health insurance claims data based study.

BACKGROUND: When patients die in a hospital their quality of life is lower than when they die at home or in a hospice. Despite efforts to improve palliative care supply structures, still about 60% of lung cancer patients die in a hospital. Studies have examined factors related to inhospital death in lung cancer patients, yet none used data of a representative German population, additionally including economic aspects. This study aimed to identify factors related to inhospital death in German lung cancer patients and analysed resulting costs. METHODS: We analysed a dataset of health insurance

Injectable Long-Acting Cabotegravir-Rilpivirine Therapy for People Living With HIV/AIDS: Addressing Implementation Barriers From the Start.

Injectable cabotegravir and rilpivirine (CAB/RPV), administered bimonthly by a medical provider, is convenient and improves privacy and medication management. One year after approval, myriad implementation barriers threaten the access and sustainability of this life-saving innovation: (1) eligibility issues (viral suppression, drug resistance, and failed oral regimens); (2) injection requires medical provider and transportation to facility; (3) strict medication adherence; (4) life challenges-mental health, homelessness, joblessness; and (5) lack of insurance and high cost. Universal implement

Recent Advances in Sickle-Cell Disease Therapies: A Review of Voxelotor, Crizanlizumab, and L-glutamine.

Sickle-cell disease (SCD) is an inherited hemoglobinopathy, causing lifelong complications such as painful vaso-occlusive episodes, acute chest syndrome, stroke, chronic anemia, and end-organ damage, with negative effects on quality of life and life expectancy. Within the last five years, three new treatments have been approved: L-glutamine in 2017 and crizanlizumab and voxelotor in 2019. We conducted a literature search of these three medications, and of the 31 articles meeting inclusion criteria, 6 studied L-glutamine, 9 crizanlizumab, and 16 voxelotor. Treatment with L-glutamine was associa

[Clinical aspects, pathophysiology, therapy and expert evaluation of splenectomy].

Splenectomy increases on principle and for life the risk to die of a foudroyant postsplenectomy infection, the so called OPSI-syndrome. The main factors determining the frequency of the OPSI-syndrome are the age at time of operation and the indication for splenectomy. Postsplenectomy infection is mostly caused by pneumococci. In general the important pathophysiological factors are a lack of filtration capacity, a decreased opsonisation activity and a deficiency of early immunoglobulin production. The increased risk of children is probably caused by a physiologically reduced immune response and

Cost-effectiveness and cost-utility of COVID-19 vaccination in a South African privately insured population: a real-world claims-based analysis.

BACKGROUND: COVID-19 placed strain on healthcare systems globally, including South Africa. While vaccine effectiveness is well established, context-specific evidence on the economic value of COVID-19 vaccination in African and real-world settings remains limited. This study evaluates the cost-effectiveness and cost-utility of COVID-19 vaccination within a South African population with private health insurance between 1 March 2020 and 30 December 2022 using claims data. METHODS: A state-transition Markov model was developed in TreeAge Pro 2025 from a third-party payer perspective, comparing no

Addressing Health-Care System Inequities in the Management of Erectile Dysfunction: A Call to Action.

Erectile dysfunction (ED) is a common, burdensome, and costly urologic condition strongly related to all aspects of general health, from physical to mental. ED has profound consequences as it may interfere physical well-being, quality of life (QoL), self-esteem, relationships, self-worth, and productivity. It is therefore important to ensure that all types of effective ED treatments are consistently accessible to patients. While federal and state mandates ensure access to treatment for women's breast health, female-factor infertility, and gender affirmation to ensure that these individuals do

Hospital use, associated costs, and payer status for infants born with spina bifida.

BACKGROUND: Health care use and costs for children with spina bifida (SB) are significantly greater than those of unaffected children. Little is known about hospital use and costs across health insurance payer types. We examined hospitalizations and associated costs by sociodemographic characteristics and payer type during the first year of life among children with SB. We also examined changes in health insurance payer status. METHODS: This study was a retrospective, statewide population-based analysis of infants with SB without anencephaly born in Florida during 1998-2007. Infants were ident

Mortality in an insured population with atrial fibrillation.

In a cooperative study of 3,099 life insurance applicants with atrial fibrillation (AF), 71 deaths over an average duration of 3.3 years were observed and compared with deaths expected and derived from standard insurance mortality tables. A normal mortality was observed in cases of paroxysmal AF with no other identified cardiovascular impairment. Paroxysmal AF with associated mitral stenosis or coronary disease was found to have a significantly increased mortality. Chronic AF with or without other impairments entails a much higher risk than paroxysmal AF, those cases associated with mitral ste

Average? Ideal? Desirable? A brief overview of height-weight tables in the United States.

Insurance companies have dominated the development of height-weight tables. Initially, tables indicated average weights for insurees, revealing weight gains with increasing age. While pneumonia and tuberculosis were leading causes of death, underweight was considered undesirable because it was associated with those conditions. Extra weight was then thought to represent a reserve that could be drawn upon in time of illness. As those diseases were brought under control, the risk factors in overweight received increased attention. Since the 1940s, tables have been developed by the Metropolitan Li

Cost-effectiveness of expanding national health insurance coverage for composite resin restorations in cancer patients in South Korea.

OBJECTIVES: Cancer patients often have compromised oral health, making them vulnerable to severe dental caries and restoration failures. Due to the nature of cervical or anterior caries in cancer patients, the use of adequate restorative materials is important. However, public dental insurance coverage for composite treatments varies among countries and only glass ionomer cements (GICs) are covered in all age groups in South Korea. This study examined the cost-effectiveness of expanding national health insurance coverage to include resin composite (RC) restorations as compared with GIC in canc
