Lung protection in acute respiratory distress syndrome: moving from protective ventilation to precision respiratory support.

PURPOSE OF REVIEW: Lung-protective ventilation focuses on limiting tidal volume and airway pressure to prevent ventilator-induced lung injury (VILI) in acute respiratory distress syndrome (ARDS). Nevertheless, patients exposed to similar ventilatory strategies often develop markedly different degrees of lung injury, indicating that conventional ventilatory targets alone do not fully account for VILI development. This review examines the evolving concept of lung protection, emphasizing load distribution, regional lung mechanics, respiratory effort, and host susceptibility. RECENT FINDINGS: Mec

Peripheral chemoreceptors, plant gain, and CO2 stores as drivers of resting ventilatory control in idiopathic hyperventilation: a prospective case-control study.

Idiopathic hyperventilation syndrome (HVS) is a poorly understood condition with potential mechanisms involving altered CO2 chemosensitivity, CO2 store depletion, and reduced plant gain (PG). Twelve carefully selected patients with HVS (HVS+), diagnosed by symptoms, normal lung function, and a positive hyperventilation provocation test, were compared to 12 matched healthy controls (HVS-). All participants underwent a hypercapnic-hyperoxic challenge test (HHCT), assessing ventilatory and drive-to-breathe responses and dyspnea. Parameters included controller gain (β-slope), ventilatory and drive

Patient-ventilator interaction-Development of a mathematical model of the respiratory center.

BACKGROUND AND OBJECTIVE: Patient-ventilator interaction critically influences the safety and effectiveness of mechanical ventilation, particularly under support modes such as pressure support ventilation (PSV). METHODS: This study introduces a mathematical model of the respiratory center that integrates chemical feedback, mechanical feedback, and reflex mechanisms to simulate spontaneous breathing and its interaction with a mechanical ventilator. The model incorporates O2 and CO2 chemoreflexes, mechanical feedback, and neural control circuits. RESULTS: Simulation studies evaluated the model

An 8-Month-Old Infant With Respiratory Failure After a Fall.

An 8-month-old previously healthy, full-term girl presented with altered mental status after falling approximately 3 feet from a bed, landing on her head. In the ED, she had a CT scan of her head (Fig 1) and was intubated for airway protection. While in the PICU, initial chest radiography showed bilateral infiltrates that were consistent with ARDS, which subsequently resolved. Her respiratory status continued to improve, which allowed a trial on CPAP with invasive neurally adjusted ventilatory assist (NAVA) support, which she was unable to tolerate because of the need for increased support dur

Neurally adjusted ventilatory assist in extremely low-birthweight infants.

BACKGROUND: Neurally adjusted ventilatory assist (NAVA) is expected to improve respiratory outcomes in preterm infants, but it has not yet been evaluated. We investigated whether NAVA could improve respiratory outcomes and reduce sedation use in extremely low-birthweight infants (ELBWI). METHODS: A retrospective cohort study was conducted based on patient charts at the Nagano Children's Hospital neonatal intensive care unit, Japan. Infants who were born at <27 weeks' gestation were included. We assessed the prevalence of bronchopulmonary dysplasia (BPD), home oxygen therapy (HOT), duration of

Noninvasive Neurally Adjusted Ventilatory Assist in Premature Infants Postextubation.

Background Neurally adjusted ventilatory assist (NAVA) has distinct advantages when used invasively compared with conventional ventilation techniques. Evidence supporting the use of noninvasive NAVA is less robust, especially in the very low birth weight (VLBW) population. Objective To determine whether synchronized noninvasive ventilation via neurally adjusted ventilatory assist (NIV NAVA) supports ventilation postextubation in premature infants. Methods A retrospective analysis of a cohort of twenty-four former VLBW (<1.5 kg) infants from July 2011 to October 2012. Decreased or unchanged cap

The Purinome and the preBötzinger Complex - A Ménage of Unexplored Mechanisms That May Modulate/Shape the Hypoxic Ventilatory Response.

Exploration of purinergic signaling in brainstem homeostatic control processes is challenging the traditional view that the biphasic hypoxic ventilatory response, which comprises a rapid initial increase in breathing followed by a slower secondary depression, reflects the interaction between peripheral chemoreceptor-mediated excitation and central inhibition. While controversial, accumulating evidence supports that in addition to peripheral excitation, interactions between central excitatory and inhibitory purinergic mechanisms shape this key homeostatic reflex. The objective of this review is

The SERVE-HF safety notice in clinical practice - experiences of a tertiary sleep center.

BACKGROUND: In May 2015, the results of the SERVE-HF trial - addressing adaptive servoventilation (ASV) in chronic congestive heart failure (CHF) patients with central sleep apnea (CSA) - prompted a field safety notice. It was recommended to identify CHF patients treated with ASV and to advise the discontinuation of the treatment. We aimed to analyze the identification process and effect of ASV discontinuation on affected patients. METHODS: 126 patients treated with ASV on May 13th, 2015 at our institution were retrospectively analyzed. Treatment decisions, effect of ASV discontinuation and c

Neurally adjusted ventilatory assist as an alternative to pressure support ventilation in adults: a French multicentre randomized trial.

PURPOSE: Neurally adjusted ventilatory assist (NAVA) is a ventilatory mode that tailors the level of assistance delivered by the ventilator to the electromyographic activity of the diaphragm. The objective of this study was to compare NAVA and pressure support ventilation (PSV) in the early phase of weaning from mechanical ventilation. METHODS: A multicentre randomized controlled trial of 128 intubated adults recovering from acute respiratory failure was conducted in 11 intensive care units. Patients were randomly assigned to NAVA or PSV. The primary outcome was the probability of remaining i

Effect of changing NAVA levels on peak inspiratory pressures and electrical activity of the diaphragm in premature neonates.

BACKGROUND: Neurally adjusted ventilator assist (NAVA) uses electrical activity of the diaphragm (Edi) to provide patient-directed ventilatory support. The NAVA level determines the proportional amount of ventilatory support. Systematically increasing NAVA level initially increases peak inspiratory pressure (PIP) while maintaining a constant Edi until a breakpoint (BrP) is reached. Further increases in NAVA level reduce the Edi, while the PIP plateaus. This study was performed to establish whether premature neonates have intact neural feedback systems allowing them to have a BrP. METHOD: NAVA

Bedside adjustment of proportional assist ventilation to target a predefined range of respiratory effort.

OBJECTIVES: During proportional assist ventilation with load-adjustable gain factors, peak respiratory muscle pressure can be estimated from the peak airway pressure and the percentage of assistance (gain). Adjusting the gain can, therefore, target a given level of respiratory effort. This study assessed the clinical feasibility of titrating proportional assist ventilation with load-adjustable gain factors with the goal of targeting a predefined range of respiratory effort. DESIGN: Prospective, multicenter, clinical observational study. SETTINGS: Intensive care departments at five university

Fetal Bladder Rupture as a Complication of Adjunctive Therapy in Severe Maternal SARS-CoV-2 Pneumonia.

INTRODUCTION: A case of spontaneous fetal bladder rupture occurring in a woman with SARS-CoV-2 pneumonia receiving invasive ventilatory support is reported. CASE: A 33-year-old woman was admitted at 30.6 weeks' gestation with the diagnosis of severe pneumonia due to COVID-19. The patient required invasive mechanical ventilation on day 2. Propofol, fentanyl, midazolam, and dexmedetomidine were administered for sedation, pain relief, and to improve patient-ventilator interaction. A bedside ultrasound on day 3 revealed fetal megacystis. Follow-up scan two days later showed urinary ascites and a

Manipulation of ventilator settings to prevent active expiration against positive pressure inflation.

Recent publications have suggested that in infants receiving artificial ventilatory support a particular pattern of interaction between spontaneous breaths and ventilator inflations (active expiration against each ventilator inflation) may be important in the production of pneumothoraces. We have looked at patterns of interaction from 47 preterm infants studied on 51 occasions. We found that active expiration against the ventilator occurred on a total of 16 occasions. This pattern was prevented on 14 occasions by altering the ventilator settings. In two other babies, the pattern persisted but

Proportional assist ventilation feasibility in the early stage of respiratory failure: a prospective randomized multicenter trial.

BACKGROUND: Proportional assist ventilation (PAV+) is an assisted ventilator mode usually applied during weaning. We aimed to determine the feasibility of using PAV+ in the early phase of acute respiratory failure compared to volume-assist control ventilation (V-ACV) in order to shorten the length of mechanical ventilation (MV). METHODS: We conducted a prospective randomized trial comparing high-assistance PAV+ (gain 80%) vs. V-ACV in four university hospital Intensive Care Units. Patients were included based on a previous pilot trial. Length of MV was the main objective. Secondary objectives

How often does patient-ventilator asynchrony occur and what are the consequences?

Mechanical ventilation can be life-saving for patients with acute respiratory failure. In between the 2 extremes of complete and no ventilatory support, both patient and machine contribute to ventilatory work. Ideally, ventilator gas delivery would perfectly match patient demand. This patient-ventilator interaction depends on how the ventilator responds to patient respiratory effort and, in turn, how the patient responds to the breath delivered by the ventilator. It is now evident that the interaction between patient and ventilator is frequently suboptimal and that patient-ventilator asynchron

Surfactant and mechanical ventilation.

Even if non invasive respiratory support is widespread used in the management of respiratory failure, nevertheless mechanical ventilation and surfactant replacement are standard care for many preterm infants with respiratory distress syndrome (RDS). The interaction between exogenous surfactant and different modalities of ventilatory support are very important and can influence respiratory outcome. The optimization of surfactant replacement during respiratory support is crucial for the successful of this therapy. In course of mechanical ventilation lung recruitment manoeuvres before and after t

Ventilatory oscillations at exercise in hypoxia: A mathematical model.

We evaluated the mechanisms responsible for the instability of ventilation control system under simultaneous metabolic (exercise) and environmental (hypoxia) stresses, promoting the genesis of periodic breathing. A model following the main concepts of ventilatory control has been tested, including cardiovascular and respiratory parameters, characteristics of peripheral and central chemoreceptors, at mild exercise in hypoxia (FIO2=0.145). Interaction between O2 and CO2 sensing was introduced following three different modalities. A sensitivity and multivariate regression analyses closely matched
