Urethral device in women with chronic urinary retention: an alternative to self-catheterization?

OBJECTIVE: To assess the efficacy and safety of a new urethral prosthesis in the management of chronic urinary retention in women as an alternative to intermittent or indwelling catheterization. METHODS: Between May 1999 and July 2000, 60 women with chronic urinary retention were prospectively enrolled in the study. The prosthesis was inserted under local anaesthesia. A magnetic control unit activates the urethral device to achieve micturition. Duration of the experience, complications and compliance were evaluated. RESULTS: Thirty patients were satisfied with the device. Discontinuation occ

Flow characteristics of urethral catheters of the same caliber vary between manufacturers.

BACKGROUND: Clean intermittent catheterization (CIC) is frequently prescribed for bladder dysfunction, either per urethra or via a continent catheterizable channel. Small catheters may be required for infants or continent channels. Success with CIC is highly dependent upon patient and family compliance. The urinary flow rate through the catheter is an important factor, which can decrease CIC time and improve quality of life. There is little objective information regarding flow rate through urinary catheters to guide catheter recommendation or prescription. Clinically, we noted that there was a

Management of urinary incontinence in neurogenic bladder.

The treatment of children with urinary incontinence caused by a neurogenic bladder presents a challenge to the paediatric urologist. Between 1965 and 1989, 527 patients with neurogenic bladder were treated and followed-up in the Department of Paediatric Urology of Debrousse Hospital. Of these, 46 were submitted to continent cystostomy, 39 to urinary artificial sphincter and 177 to clean intermittent catheterization (CIC). This last treatment is of paramount importance in girls and can be associated with complementary procedures if cervico-urethral resistance and/or bladder compliance are poor.

Status and tasks of today's urodynamics.

The current situation of urodynamics and the ways of improving and reinforcing our investigational and treatment modalities are presented. Correct evaluation of the bladder dysfunction depends on sound evaluation of a patient's history, naked-eye observation and careful urodynamic investigation. The clinical and social significance of the following items is specifically discussed: urinary incontinence of elderly people, clean intermittent catheterization and artificial urethral sphincter.

Bladder augmentation using the stomach in spinal cord injured patients with impaired renal function.

A small capacity, poorly compliant neurogenic bladder is a difficult problem in spinal cord injured patients. Bladder augmentation with intermittent catheterization offers an alternative to indwelling catheterization. Ileum, cecum, and sigmoid colon have been most commonly used for enterocystoplasty but may, however, cause hyperchloremic metabolic acidosis in patients with impaired renal function. This can be prevented by performing a gastric augmentation. We present two cases of gastrocystoplasty in spinal cord injured patients with a small capacity, poorly compliant neurogenic bladder, impai

Effectiveness of manual bladder expression in paraplegic dogs.

OBJECTIVE To determine the effectiveness of manual bladder expression in paraplegic dogs by comparing urine volumes measured by use of intermittent catheterization and ultrasonography. ANIMALS 36 paraplegic dogs. PROCEDURES 93 measurements of bladder volume were collected for the 36 dogs. Residual urine volume was determined by use of intermittent urethral catheterization and estimated by use of ultrasonography. RESULTS Manual bladder expression voided a mean of 49% of urine from the bladder in this population of dogs. There was no correlation (R2, 0.06) between the effectiveness of manual bla

Predictors of success in electroejaculation of spinal cord injured men.

Rectal probe electroejaculation was attempted in 48 spinal cord injury men and greater than 10 million sperm were obtained in 71%. Patient age and interval since injury had no effect on outcome. The best performance was seen in thoracic paraplegics and in those using intermittent catheterization for bladder management. Indwelling urethral catheters and high pressure reflex voiding had a negative impact on electroejaculation results.

A bulbar artery pseudoaneurysm following traumatic urethral catheterization.

Traumatic urethral catheterization may result in a number of serious complications. A rare occurrence is the development of a urethral pseudoaneurysm. We report the case of a 13-year-old male who required placement of a Foley catheter for an orthopedic surgical procedure. The Foley was misplaced in the bulbourethra, resulting in the development of a bulbar artery pseudoaneurysm. Profuse bleeding via the urethra was noted after removal of the catheter, and the patient experienced severe intermittent hematuria during the postoperative period. Cystoscopy revealed a pulsatile mass within the bulbo

Endoscopic removal of knotted urethral catheter: a point of technique.

Urethral catheter knotting is a rare complication of the simple and widely practiced clean intermittent self-catheterization. We report the endoscopic retrieval of a retained knotted feeding tube in a 12-year-old child. Various factors leading to such a rare complication and a new minimal invasive technique are described. To the best of our knowledge this technique has not been previously reported in the medical literature.

Catheter-associated urinary tract infections.

Millions of urethral catheters are used each year. This device subverts several host defenses to allow bacterial entry at the rate of 3% to 10% incidence per day, and its presence encourages the organism's persistent residence in the urinary tract. Most catheter-associated bacteriurias are asymptomatic. The complications in short-term catheterized patients include fever, acute pyelonephritis, bacteremia, and death; patients with long-term catheters in place are at risk for these complications and catheter obstruction, urinary tract stones, local periurinary infections, chronic renal inflammati

The Mitrofanoff principle in urinary reconstruction.

We report the use of the Mitrofanoff principle to achieve urinary continence in 47 patients. A catheterizable channel can be used as the primary continence mechanism, or as an adjunct to ensure complete bladder emptying if urethral catheterization or voiding is inadequate. Appendico-vesicostomy was performed in 25 of 47 patients (55%). Satisfactory continence was achieved in 45 patients (96%). Bladder emptying by clean intermittent catheterization was performed in all patients. Early complications included peristomal abscess (1 patient) and small bowel obstruction (1). Late complications inclu

Pregnancy Following Clam Cystoplasty and Mitrofanoff Procedure in a Patient With Fowler's Syndrome and Detrusor Overactivity.

Fowler's syndrome causes urinary retention due to failure of the urethral sphincter to relax. Management aims for complete bladder emptying, typically via intermittent self-catheterization. If conservative treatment fails, detrusor overactivity with Fowler's syndrome may be managed surgically using clam cystoplasty and the Mitrofanoff procedure to increase bladder capacity and reduce pressure. We report a rare case of pregnancy in a woman with Fowler's syndrome and detrusor overactivity, who previously underwent clam cystoplasty and Mitrofanoff procedure. She had a continent, functional abdomi

[Intermittent catheterization as the basis for treatment of neurogenic bladder during childhood].

OBJECTIVE: Neurogenic bladder (NB) frequently causes incomplete bladder emptying, which can lead to deterioration of the upper urinary tract (UUT), vesico-renal reflux (VRR) and incontinence. The aim of this study is to determine the degree of acceptance, performance and results of treatment of NB in children by intermittent catheterization (IC). METHODS: Of 121 children (50 girls and 38 boys) with NB, 88 were managed by intermittent catheterization. The mean age at treatment was 5.2 years (range 20 days-13 years). PVC catheters were employed and reutilized for 3 or 4 weeks. Catheters were as
