The enthesopathic changes of hypophosphatemic osteomalacia in adults: radiologic findings.

The enthesopathy (bone proliferation at sites of ligament, tendon, joint capsule, and interosseous membrane attachment) that may be present in adults who have longstanding hypophosphatemic osteomalacia, the most common type of osteomalacia, has received little attention in the radiologic literature. These proliferative enthetic changes may be associated with various musculoskeletal symptoms. The spectrum of proliferative enthesopathic changes, involving the axial and appendicular skeleton, is described in six patients 18-63 years old. Proliferative changes at the attachment of the annulus fibr

The pronator contracture syndrome: A new entity in supination restriction.

Forearm rotation is essential for daily activities and functional dexterity, involving both pronation and supination. Forearm rotation limitations can result from various pathologies, often linked to trauma, structural changes in radio-ulnar joints, and interosseous membrane alterations. Soft tissue contracture, such as post-burn scarring or specific neurological conditions, can also restrict forearm rotation. We present two cases of painless restricted passive and active supination, devoid of bony, neurovascular, soft tissue, or congenital abnormalities. We describe a surgical technique to im

Flexor Digitorum Superficialis Tendon Transfer for Wrist Extension.

UNLABELLED: Peripheral nerve injuries are among the most complex conditions facing upper-extremity surgeons. Loss of wrist extension can result in marked limitations, including loss of pinch and grip strength with discoordination of grasp and release. Tendon transfers represent the mainstay of operative treatment and have proven to be an effective method for restoring loss of wrist extension. The literature describes myriad techniques to restore loss of wrist extension. The best choice of transfers is dependent on what is available, depending on the level of injury. The present article describ

Split tibialis posterior transfer for equinovarus deformity in cerebral palsy. Long-term results of a new surgical procedure.

We report the results of a new surgical procedure for spastic equinovarus deformity due to cerebral palsy. This is the transfer of the anterior half of the split tibialis posterior to the dorsum of the foot through the interosseous membrane. We performed the operation on 23 feet in 18 children. All patients were assessed before operation and at follow-up at a mean of 8.4 years postoperatively. Using the criteria of Kling et al (1985), excellent results were obtained in 14 feet, good results in eight, and a poor result in only one.

Pronator Quadratus Pedicled Rotational Flap: Anatomic Feasibility Study for Regional Soft Tissue Coverage of the Forearm and Wrist.

BACKGROUND: Soft tissue reconstruction using a pronator quadratus pedicled rotational flap (PQPRF) has not been described in detail previously. This cadaveric study assesses surface-area mapping for PQPRF reconstruction and technical considerations for surgical use. METHODS: Fresh-frozen trans-humeral cadaveric amputation specimens were used. The PQPRF and its anterior interosseous artery (AIA) pedicle were isolated to assess muscle perfusion and to map the anatomic feasibility of rotational flap coverage. The AIA pedicle was created by its distal to proximal dissection, preserving the distal

Peroneal artery entrapment syndrome (PRAES): a rare cause of ischaemic toes.

A 75-year-old man presenting with intermittent discolouration of his left toes was referred to vascular surgery with suspicion of embolic vascular disease. A contrast-enhanced MR angiogram was performed which revealed bilateral dominant peroneal arteries (PRAs). There was evidence of short atherosclerotic stenosis directly at the point where the left PRA passes through the tibiofibular interosseous membrane which we postulate to be the source of the emboli. We present what is believed to be the first reported case of PRA entrapment complicated by distal toe emboli.

The Essex-Lopresti lesion: a variant with a bony distal radioulnar joint injury.

The Essex-Lopresti lesion is an unusual injury, consisting of a radial head or neck fracture, distal radioulnar joint (DRUJ) injury and interosseous membrane rupture. To date, all reported Essex-Lopresti lesions have consisted of soft tissue injuries at the DRUJ. We present a case of an Essex-Lopresti lesion with a bony variant, in which the DRUJ injury consisted of an ulnar head fracture associated with radial head fracture and acute proximal migration of the radius. The management involved plating of the ulnar head fracture and titanium replacement of the radial head.

Distraction lengthening of the radius for radial longitudinal instability after distal radio-ulnar subluxation and excision of the radial head: a case report.

The results of late surgery for symptomatic proximal migration of the radius after resection of the radial head for trauma are usually disappointing. Ulnar variance increases when the interosseous membrane is disrupted. Its integrity should be assessed to predict the results of further surgery. We describe a 29-year-old patient in whom distraction lengthening of the radius through an Ilizarov frame allowed him to regain pain-free function of the wrist and elbow after a complex Essex-Lopresti fracture dislocation and late symptoms of ulnar carpal impingement.

Radiological spectrum of endemic fluorosis: relationship with calcium intake.

Skeletal fluorosis continues to be endemic in many parts of India. Osteosclerosis and interosseous membrane calcification have long been regarded as hallmarks of this disease. Our study showed in addition a wide variety of radiological patterns: coarse trabecular pattern, axial osteosclerosis with distal osteopenia and diffuse osteopenia. Subjects with osteopenic changes had a significantly lower dietary intake of calcium than those groups having normal radiological findings, predominant osteosclerosis or coarse trabecular pattern (p < 0.001, p < 0.01, and p < 0.01 respectively). This suggests

Skeletal fluorosis: don't miss the diagnosis!

Skeletal fluorosis is a rare toxic osteopathy characterized by massive bone fixation of fluoride. The disease occurs as an endemic problem in some parts of the world and is the result of prolonged ingestion or rarely by inhalation of high amounts of fluoride. Radiographic presentation is mainly characterized by bone changes with osteocondensation and later ossification of many ligaments and interosseous membranes. Skeletal fluorosis is not clinically obvious and can be confused with other rheumatologic disorders. Its severity lies in the development of skeletal deformities and neurological com

Rehabilitation of distal radioulnar joint instability.

Distal radioulnar joint (DRUJ) instabilities are common and often combined with other injuries of the interosseous membrane and/or the proximal radioulnar joint. Once they are diagnosed and the treatment is chosen, physiotherapists have limited choices due to the lack of validated protocols. The benefits of proprioception and neuromuscular rehabilitation have been brought to light for the shoulder, knee and ankle joints, among others. However, no program has been described for the DRUJ. The purpose of this article is to study the muscular elements responsible for active DRUJ stability, and to

Reconstruction of essex-lopresti injury of the forearm: technical note.

Longitudinal instability of the forearm resulting from an Essex-Lopresti injury is a surgical challenge, and no technique has yet met universal success. A new technique is presented here consisting of reconstruction of the radial head, leveling of the distal radioulnar joint, reconstruction of the central band of the interosseous membrane by using a pronator teres rerouting technique, and finally repair of the triangular fibrocartilage complex. It is hoped that by addressing all of the contributing longitudinal stabilizing structures, the longitudinal instability of the forearm will be control

Posterior tibial tendon transfer in spastic equinovarus.

Twenty-eight posterior tibial tendon transfers through the interosseous membrane were performed to correct spastic equinovarus. All patients improved their gait, and 82% discarded their brace. Foot striking at the beginning of the stance phase was good or satisfactory in 82%; 68% had a neutral position of the heel. The overall score was good in 68%, satisfactory in 21%, and poor in 11%. The main requirements for obtaining good results with this type of transfer are: no fixed varus deformity preoperatively, age at operation between 5 and 10 years, and reinsertion near the midline of the foot.

Biomechanical study of isolated radial head dislocation.

BACKGROUND: Isolated radial head dislocation is a rare injury with an unclear pathomechanism, and the treatment is controversial. The purpose of the present study was to investigate the biomechanical contributions of the annular ligament, quadrate ligament, interosseous membrane, and annular ligament reconstructions to proximal radioulnar joint stability. METHODS: Five fresh frozen cadaveric upper extremities were amputated above the elbow and solidly fixed on a customized jig. Radial head dislocation was reproduced by sequential sectioning of ligamentous structures and passive mobility testi

Bracing of stable shaft fractures of the ulna.

In the laboratory an ulnar osteotomy was performed in 10 cadaveric specimens, which included 5 distal-third junctions and 5 middle-third junctions. The interosseous membrane was left intact. Each specimen was rotated through full pronation and full supination. Displacement less than half shaft width was considered stable. The interosseous membrane was subsequently released 2 cm proximal and 2 cm distal to the osteotomy. Repeated pronation and supination range of motion documented gross displacement greater than half of the width of the shaft and was diagnosed as unstable. To evaluate this hypo

The anatomical basis for anterior interosseous nerve palsy secondary to supracondylar humerus fractures in children.

INTRODUCTION: Various studies have found that 6.6 to 31% of supracondylar elbow fractures in children have nerve-related complications. One-third of these are cases of anterior interosseous nerve (AIN) palsy that usually result in a deficit of active thumb and index flexion. The goal of this cadaver study was to describe the course of the AIN to achieve a better understanding of how it may get injured. MATERIALS AND METHODS: On 35 cadaver specimens, the median nerve and its collateral branches destined to muscles were dissected at the elbow and forearm levels. The distance at which the variou

[Bilateral elbow dislocation related to Essex-Lopresti injury].

Elbow dislocation is second in frequency, after the shoulder, whereas bilateral dislocation is uncommon, even less than dislocations with concurrent associated fractures. One of the least frequent associations is the Essex-Lopresti injury which consists of a fracture of the radial head affecting the distal radioulnar joint with injury to the interosseous membrane. This is a case of bilateral elbow dislocation, one of the elbows associated with the Essex-Lopresti injury. During treatment, the premature closed reduction prevails, previously making sure the elbow is stable, the premise which will

Posterior Tibial Tendon Transfer for Common Peroneal Nerve Injury.

Common peroneal nerve dysfunction after a multiligament knee injury can be devastating. In patients with persistent foot drop, posterior tibial tendon transfer to the dorsum of the foot is a reliable and safe procedure to restore dorsiflexion. These authors favor passing the posterior tibial tendon through the interosseous membrane and docking it into the lateral/middle cuneiforms. A Strayer procedure or tendo-Achilles lengthening must be performed in patients unable to achieve at least 10° of passive dorsiflexion. Despite the operative limb having 30% to 40% of ankle dorsiflexion strength of

Vascular anatomy of the tibiofibular syndesmosis.

BACKGROUND: Injuries to the tibiofibular syndesmosis commonly cause prolonged ankle pain and disability. Syndesmotic injuries are associated with slower healing rates compared with rates for other ankle ligament injuries and typically result in longer time away from sports. To our knowledge, the vascular supply to the syndesmosis and its clinical implication have not previously been studied. The purpose of this study was to describe the vascular supply to the tibiofibular syndesmosis with use of a method of chemical debridement of cadaveric specimens. METHODS: Twenty-five matched pairs of adu

Essex-Lopresti Lesions and Longitudinal Radioulnar Instability: A Narrative Review.

»: An Essex-Lopresti injury (ELI) is classically described as a radial head fracture with a concomitant interosseous ligament complex (IOC) injury. However, multiple injury patterns may be present, and an interosseous membrane (IOM) injury should be evaluated for in any axial load injury through the forearm. ELI may result in longitudinal radioulnar instability (LRUI). »: Diagnosis of an IOC injury can be difficult. Evaluation begins with standard wrist and elbow radiographs and is supplemented with radiographs and intraoperative maneuvers to assess for proximal radial migration. Magnetic res
