Impact of multidisciplinary teams for management of intestinal failure in children.

PURPOSE OF REVIEW: To review the recent literature related to the impact of an intestinal rehabilitation program (IRP) on the management of intestinal failure in children. RECENT FINDINGS: As publication of a systematic review of pediatric IRPs in 2013, there have been four publications further describing the impact of IRPs in children with intestinal failure. The results continue to support an improvement in survival and enteral autonomy, and a decrease in complications related to liver dysfunction, central venous catheters, and transplantation. SUMMARY: Pediatric IRPs offer significant adv

Intestinal failure in children and young people with neurodisabling conditions.

Gastrointestinal dysmotility is common in children and young people with neurodisabling conditions. In this article we seek to highlight the increasing difficulties faced by paediatricians in managing intestinal failure in this patient group. It is becoming clear that, as the median age for survival increases, intestinal failure is a significant problem, and can in some cases become life-limiting. The ethical issues around starting children with life-limiting conditions on parenteral nutrition (PN) are extremely complicated, not least because we are ignorant of the mechanism of intestinal fail

Intestinal Failure and Transplant: The Australian Experience (2009 to 2014).

BACKGROUND: A joint adult and pediatric intestinal transplant (ITx) program for Australia was developed in 2009 to provide life-saving ITx to patients with irreversible intestinal failure (IF). We aimed to analyze the outcomes of patients treated by our service over the past 5 years. METHODS: A retrospective medical record review was conducted on all IF patients referred to our service. Patient demographics, underlying disease, nutrition support, TPN complications, and current transplant program status were evaluated. RESULTS: Fifty-seven patients (35 adults, 40.4 ± 12.4 years; 22 children,

Predictors for developing intestinal failure in patients with Crohn's disease.

BACKGROUND AND AIM: Intestinal failure (IF) is a rare but devastating complication of Crohn's disease (CD). The clinical and surgical factors that predispose to IF are poorly understood. The aim of this study was to define clinical factors that predispose to IF. METHODS: A retrospective case-control study was performed using consecutive CD patients with IF who were identified from a prospective database. Local population-based controls were selected with which to compare demographic, phenotypic, and clinical outcomes. RESULTS: Eighty-two CD patients requiring long-term intravenous fluids or

The Safety and Immunogenicity of Rotavirus Vaccination in Infants With Intestinal Failure.

BACKGROUND: Young children with intestinal failure are at risk for complications from rotavirus gastroenteritis. To date, the safety and immunogenicity of rotavirus vaccines in these children are not known. We hypothesized that rotavirus vaccination would be safe and confer immunity to infants with intestinal failure and a history of abdominal surgery. METHODS: The study population consisted of infants with early intestinal failure who required abdominal surgery and parenteral nutrition for necrotizing enterocolitis, gastroschisis, jejunoileal atresia, or meconium peritonitis. Subjects receiv

Medical and surgical management of the pediatric patient with intestinal failure.

Until recently, extreme short bowel due primarily to massive resection in the neonatal period had been considered incompatible with long-term survival. Indeed, parents of infants with midgut volvulus or other causes of very extensive intestinal necrosis still may be informed that resection is futile. The advent of intestinal transplantation as a potential therapy and its evolution into a standard therapy for irreversible intestinal failure have led to changing attitudes regarding these catastrophic gastrointestinal events. The experience gained from aggressively maintaining infants with little

The current role of small-bowel transplantation in intestinal failure.

Short bowel syndrome is the clinical manifestation of a fundamental reduction in the functional intestinal absorptive surface area and malabsorption. The development of total parenteral nutrition has improved the natural course of this disease. Home parenteral nutrition-related complications continue to generate significant morbidity and mortality for these patients. Small-bowel transplantation is an alternative to home parenteral nutrition. There are significant risks of graft rejection and the potential complications of long-term immunosuppression. Small-bowel transplantation is an option, b

Mechanism of intestinal adaptation in rats with acute renal failure.

Acute renal failure (ARF) was experimentally induced in rats and the specific activity of mucosal Na-K-ATPase activity in segments of the small intestine and colon was measured. Bilateral nephrectomy (BN) resulted in a significant evaluation of the enzyme activity in all segments examined. With an additional procedure of adrenalectomy (BN + Ax), the enzyme activity failed to show any increase in ARF rats produced by BN. However, a supplementation of a maintenance dose of dexamethasone to adrenalectomized ARF rats (BN + Ax + DM 10) resulted in a significant resumption of the activity in all int

Shoshin Beriberi in a Child With Intestinal Failure: A Case Report.

BACKGROUND: Acute illness states with increased metabolic demand can precipitate severe thiamine deficiency if physiologic needs exceed endogenous stores and supplementation. Patients with preexisting risk factors such as parenteral nutrition (PN) dependence, eating disorders, gastrointestinal disorders, or surgeries are especially vulnerable. CASE SUMMARY: A 9-year-old girl with short bowel syndrome receiving long-term PN, including standard thiamine supplementation, presented with refractory shock following aspiration. Use of Teduglutide in Children With Intestinal Failure: A Systematic Review.

BACKGROUND AND OBJECTIVES: Short-bowel syndrome (SBS) results from the loss of a significant portion of the small intestine leading to a state of malabsorption. After an intestinal loss, there is a process of adaptation involving the Glucagon-Like Peptide-2 (GLP-2), an enteroendocrine peptide also involved in nutrient absorption. Teduglutide is a recombinant analog of GLP-2 approved in 2016 to treat selected SBS pediatric patients who are dependent on parenteral support. The present systematic review aims to evaluate the efficacy of Teduglutide in pediatric patients with SBS in reducing the ne

Heart failure is associated with depletion of core intestinal microbiota.

AIMS: In spite of current medical treatment approaches, mortality of chronic heart failure (HF) remains high and novel treatment modalities are thus urgently needed. A recent theory proposes a possible impact of the intestinal microbiome on the incidence and clinical course of heart failure. This study sought to systematically investigate, if there are specific changes of the intestinal microbiome in heart failure patients. METHODS AND RESULTS: The intestinal microbiome of 20 patients with heart failure with reduced ejection fraction due to ischemic or dilated cardiomyopathy was investigated

Is OM-3 synergistic with GLP-2 in intestinal failure?

INTRODUCTION: Glucagon-like peptide-2 (GLP-2) is a known intestinal growth factor that enhances mucosal mass and function in residual small intestine after massive small bowel resection (MSBR). Luminal omega-3 (OM-3) has been shown to have some growth factor properties. It is possible that their mechanisms of action differ. Thus, we hypothesized that administering these two substances together may have a synergistic effect. METHODS: A total of 60 adult female Sprague-Dawley rats underwent 80% MSBR and divided as follows (n = 15/group): Saline (Control) + regular feeds; GLP-2 + regular feeds;

Survival rate and prognostic factors in patients with intestinal failure.

BACKGROUND: Intestinal failure impairs nutritional status and survival expectance. Though intestinal adaptation and enteral independence may be achieved, artificial nutrition is needed in about half of the patients. AIMS: This study is aimed at assessing the causes of death, survival rate, enteral independence in time, and factors affecting the clinical outcome in a group of patients with intestinal insufficiency. PATIENTS: Sixty-eight patients with intestinal insufficiency, due to major intestinal resection in 60 cases (short bowel syndrome) (remnant intestine length 101-150 cm in 31 cases,

[Changes in the large intestine in patients with chronic kidney failure].

Clinical and bacteriological examinations performed in 138 patients with chronic renal failure (CRF) have found in the majority of late-stage examinees endogenic chronic colitis (ECC). This should be taken into consideration when prescribing repeat enteroclysis for correction of some uremia symptoms. Etiologically, ECC may be due to intestinal dysbacteriosis.

Case series of selenium insufficiency among infants with intestinal failure.

BACKGROUND: Selenium (Se) is an essential trace element incorporated into 'selenoenzymes' that are necessary for immune, thyroid and antioxidant responses. Infants with intestinal failure (IF) require prolonged administration of parenteral nutrition (PN). This report presents a series of infants with IF who demonstrate biochemical Se deficiency despite PN supplementation, which normalized following dose escalation. CASE SERIES: Patient 1 was born at 25 weeks' gestation, birthweight 870 g, with intestinal malrotation and colonic atresia. Patient 2 was born at 28 weeks' gestation, birthweight 1

[Nutritional support treatment for short bowel syndrome related intestinal failure].

Intestinal failure is a syndrome characterized by a diminished intestinal function that is inadequate to maintain normal digestion and absorption, leading to systemic metabolic disorder and requiring long-term nutritional supplementation to sustain health and growth. Short bowel syndrome (SBS) is one of the primary causes of intestinal failure. Given the significant differences among SBS patients, nutritional treatment strategies should emphasize individualization. This review focuses on SBS, combining its anatomical and pathological characteristics, to introduce nutritional support treatment
