Size and location of ruptured intracranial aneurysms.

OBJECTIVE: The aim of study was to review our patient population to determine whether there is a critical aneurysm size at which the incidence of rupture increases and whether there is a correlation between aneurysm size and location. METHODS: We reviewed charts and radiological findings (computed tomography (CT) scans, angiograms, CT angiography, magnetic resonance angiography) for all patients operated on for intracranial aneurysms in our hospital between September 2002 and May 2004. Of the 336 aneurysms that were reviewed, measurements were obtained from angiograms for 239 ruptured aneurys

Intracranial aneurysms in patients with CREST syndrome.

CREST syndrome is a variant of scleroderma characterized by calcinosis, Raynaud's phenomenon, esophageal hypomotility, sclerodactyly, and telangiectasia, and is a collagen vascular disease characterized by inflammation and fibrosis of multiple organs/tissues. Neurological and cerebrovascular abnormalities are uncommon in CREST syndrome. Here, we report two patients with CREST syndrome harboring intracranial aneurysms. A 53-year-old woman with a 6-month history of CREST syndrome had multiple intracranial aneurysms that arose from the right middle cerebral artery, the left middle cerebral artery

Intracranial tuberculoma with adjacent inflammatory aneurysms.

We describe for the first time a case of intracranial tuberculoma with adjacent intracerebral inflammatory aneurysms, and include a brief discussion of the developmental mechanism of these pathologies. A 28-year-old woman presented with a history of intermittent seizure attacks. She had been diagnosed as having pulmonary tuberculosis 1 year previously, and had been treated with antituberculosis medications for 9 months. Her brain MRI revealed a 1-cm-diameter round subcortical mass lesion on the left parietal lobe, which showed gadolinium enhancement. At surgery, we were surprised to find multi

Endovascular treatment of peripheral intracranial aneurysms.

BACKGROUND AND PURPOSE: Distally located cerebral aneurysms are difficult to treat with preservation of the parent vessel. We report the angiographic results and clinical outcome for 27 patients with peripheral cerebral aneurysms. METHODS: From January 2000 to June 2005, 27 patients, 13 female and 14 male, presented to our institution with peripheral intracranial aneurysms and were treated endovascularly. None of these aneurysms were mycotic in origin. The age of our patients ranged from 23 to 76 years with a mean age of 53. Twenty of the 27 patients had subarachnoid and/or intracerebral hemo

Multiple intracranial aneurysms presenting with epistaxis.

Although epistaxis secondary to rupture of a post-traumatic, false aneurysm is well documented, true intracavernous carotid artery aneurysms presenting with epistaxis are very rare. We present such a case in a patient with multiple aneurysms. The aneurysm was treated by endovascular techniques and the patient made a good recovery. This case highlights the need to distinguish between true and false aneurysms for both diagnostic and therapeutic reasons.

Ruptured de novo intracranial aneurysms.

OBJECTIVE: We describe the actual state of ruptured de novo intracranial aneurysms to contribute to a guideline of follow-up for the patients with treated intracranial aneurysm. METHODS: The authors retrospectively investigated 12 cases drawn from 483 consecutive cases of aneurysmal subarachnoid hemorrhage at our institute over a period of 22 years, in which a previously undemonstrated (hence "de novo") intracranial saccular aneurysm formed and ruptured after successful treatment of a prior aneurysm. FINDINGS: The 12 cases constitute 2.5% of the 483 patients who left our hospital alive. Elev

Endoscope-assisted microsurgery for intracranial aneurysms.

OBJECTIVE: We discuss the role of the endoscope in the microsurgical treatment of intracranial aneurysms, analyzing its benefits, risks, and disadvantages. METHODS: This was a prospective study of 55 patients with 79 aneurysms, treated between July 1998 and June 2001, for whom the endoscope was used as an adjunct in the microsurgical treatment of their lesions. Seventy-one aneurysms were located in the anterior circulation, and eight were located in the posterior circulation. Thirty-seven patients presented with subarachnoid hemorrhage. Eighteen patients had unruptured aneurysms, of whom 5 pr

Endovascular treatment of unruptured intracranial aneurysms.

BACKGROUND AND PURPOSE: Intracranial aneurysms are common, with an overall frequency ranging from 0.8% to 10%. Because prognosis after subarachnoid hemorrhage is still very poor, treatment of unruptured aneurysms, either neurosurgically or endovascularly, has been advocated. However, risk of rupture and subsequent subarachnoid hemorrhage needs to be considered against the risks of elective treatment. We analyzed the technical feasibility, safety, and efficacy of endovascular treatment of a consecutive series of unruptured cerebral aneurysms. METHODS: From July 1997 through December 2000, a to

[Traumatic false aneurysms of intracranial artery].

The authors report a case of intracranial traumatic aneurysm at young a 21 years old patient. He presented with recurrent epistaxis 3 months after craniofacial trauma. An aneurysm of the left carotid artery was diagnosed at cerebral angiography. Endovascular treatment was performed with favorable outcome. In spite of their rarity, posttraumatic intracranial aneurysms should be suspected in patients with recent or remote history of trauma, even minor, presenting with secondary clinical deterioration, in order to carry out emergent angiographic exploration, before contemplating definitive surgic

[Minimally invasive surgery in intracranial aneurysms].

INTRODUCTION: The use of minimally invasive procedures in neurosurgery permits a significant reduction in peri-operative morbidity and duration of hospital stay. The fronto-latero-basal (FLB) approach gives good exposure of the structures of the anterior and middle fossas with access to most aneurysms of the anterior circulation, thus avoiding extensive craniotomies and the use of brain retractors. OBJECTIVE: We present an analysis of 27 patients operated on to clip 29 aneurysms of the anterior circulation using the FLB approach. PATIENTS AND METHODS: The study included 27 patients, 18 women

Surgical strategies for giant intracranial aneurysms.

Untreated giant intracranial aneurysms have a dismal natural history as a result of hemorrhage, cerebral compression, and thromboembolism. The poor prognosis of patients with giant aneurysms therefore warrants aggressive treatment. A surgical approach is chosen to maximize the operative exposure of the aneurysm and depends mainly on the aneurysm's location. Once exposed, vascular control of the aneurysm is required not only to manage an intraoperative rupture, but also to collapse the aneurysm, to increase working space, and to improve visualization of the anatomy. Hypothermic circulatory arre

[Familial intracranial aneurysms. Review of the literature].

The pathogenesis of intracranial aneurysms is multifactorial, with acquired factors probably associated with genetic factors. Intracranial aneurysms may be associated with heritable connective tissue disorders, mainly polycystic kidney disease, Ehlers-Danlos syndrome, Marfan's syndrome, and pseudoxhantoma elasticum. In the recent literature, familial intracranial aneurysms have been documented by large epidemiological studies in Sweden and Finland, with a frequency as high as 6.7 to 10%. Familial intracranial aneurysms have special characteristics concerning the age of the patient at the time

Endovascular treatment of bacterial intracranial aneurysms.

We report three patients with bacterial intracranial aneurysms treated by the endovascular approach: two presented with sudden severe neurological deficits after a diagnosis of endocarditis; the other had suspected vasculitis. CT showed an intracerebral haematoma in all cases; angiography revealed bacterial aneurysms of distal branches of the middle cerebral artery in one. Because of the patients' condition and the location of the aneurysms, endovascular treatment was considered the fastest and safest treatment. Hyperselective catheterisation of the parent branch, close to the aneurysm, was pe

[Results of surgical treatment of intracranial aneurysms].

In the years 1986-1990 in the Department of Neurosurgery Military Hospital in Bydgoszcz 61 patients were treated surgically for intracranial aneurysms. Good results were obtained in 60.7% of cases and bad in 21.3%. Death from extracranial causes occurred in 6 cases. The number of past bleedings from the aneurysms, the clinical state of the patients, the timing of the operation, intraoperative rupture of the aneurysms were factors having a significant unfavourable influence on the early postoperative results.
