The clinical picture of rheumatoid arthritis (RA) is best viewed as a combination of systemic symptoms associated with the inflammatory process and articular symptoms related both to potentially reversible synovitis and structural damage brought on by inflammation. In simple terms, the treatment of inflammation is medical, and structural lesions often require surgical solutions. The prime indications for surgery in patients with RA are essentially determined by the patient and consist of a desire to obtain pain relief and/or functional improvement. Pain is difficult to quantify. Essential conc

This article focuses on the medial column procedures for the treatment of the acquired flatfoot. When performing a medial column procedure alone, in combination with soft tissue reconstruction, or with concomitant lateral bony procedure, the deformity being addressed should include significant abduction or collapse through the talonavicular (TN), naviculocuneiform (NC), or tarsometatarsal (TMT) joints. The procedures covered in this article include arthrodesis of the TN, NC, and metatarsocuneiform (MTC) joints, together or in isloation. Osteotomies of the cuneiforms will also be addressed.

Disappointment with the late results of intrinsic release for the rheumatoid "intrinsic-plus" hand has led to a re-appraisal of the role of intrinsic muscle contracture in the pathogenesis of the rheumatoid swan neck deformity. In cadaveric fingers, a properly placed suture typing the lateral band to the middle slip insertion causes a swan neck deformity. It is suggested that the usual "intrinsic-plus" hand and the fixed swan neck deformity of rheumatoid arthritis is caused by adhesions between the extensor tendons on the dorsum of the proximal interphalangeal joint, rather than by intrinsic m

BACKGROUND: François-Sigismond Jaccoud arthropathy (FSJA) is a rare, secondary joint disease characterised by joint deformities without bone erosions, with a "reducible" pattern. Substantial knowledge exists in this area; however, the pathogenesis of FSJA remains indefinite, and clear diagnostic criteria are lacking. Moreover, preventive measures and specialised treatment interventions for the development of FSJA are needed. Thus, in this novel report, we present the case of a 24-year-old male patient with secondary FSJA, characterised by a prolonged disease course, early atypical symptoms, an

Correction of severe valgus deformity of the foot and ankle with ankle replacement is challenging. We describe the controversies and specific issues of surgical management and provide a detailed surgical strategy for management of this common deformity. A reliable technique for deltoid reconstruction is also described and illustrated in detail.

Extension of the fingers is a complex act. Boutonnière deformity is defined by flexion at the proximal inter-phalangeal (PIP) joint and hyperextension at the distal interphalangeal (DIP) joint due to disruption of the central slip of the extensor tendon. Swan neck deformity is defined by hyperextension at the PIP joint and flexion at the DIP joint, and the pathology is divided into intrinsic, extrinsic, and articular. The deformities are a result of imbalance of the tendons and ligaments in the fingers. Treatment is depending on the underlying cause and includes surgery and non-operative treat

Boutonnière and swan neck deformities of the finger can be the result of trauma. The complex anatomy of the extensor mechanism of the finger makes understanding the pathomechanics of these deformities challenging. These posttraumatic deformities should not be confused with those associated with inflammatory arthritis because the treatment options are often very different. An accurate clinical assessment is essential for selecting the appropriate treatment method. Physical examination, including Elson and intrinsic-plus tests, and plain radiography are important tools for diagnosis. A variety o

The ankle joint is part of a biomechanical hindfoot complex. Approximately 1% of the world's adult population is affected by ankle osteoarthritis (AO). Trauma is the primary cause of ankle OA, often resulting in varus or valgus deformities. Only 50% of patients with end-stage ankle OA have a normal hindfoot alignment. The biomechanics and morphology of the arthritic valgus ankle is reviewed in this article and therapeutic strategies, including joint preserving and nonpreserving modalities are presented. Pitfalls are discussed and the literature is reviewed regarding outcomes in patients with v

Camptodactyly is a non-traumatic, painless, non-neurogenic flexion deformity at the proximal interphalangeal joint of the little finger, which may occur in isolation or in various developmental dysmorphology syndromes. In a ten-year survey of almost 10,000 consecutive neurology outpatient referrals, using a passive case finding strategy, camptodactyly was observed with a frequency of 0.43%. All were cases of isolated camptodactyly, and all but one were asymptomatic. Camptodactyly was more often bilateral, often asymmetric, than unilateral. A family history was common, sometimes with intrafamil

Limb inequality, axial deformity and other problems of reconstructive bone and joint surgery are discussed. Limb length equalisation and correction of axial deformities are complex surgical and rehabilitation procedures associated with a high incidence (0-50%) of complications and side effects requiring prompt remedial measures. The basic principles of limb lengthening by the Wagner method and the essential features of the Ilizarov method for elongation and spatial correction of congenital and acquired limb deformities are described. Further indications for the use of the Ilizarov method in th

One of the most common causes of acquired flatfoot deformity in adults is dysfunction of the posterior tibial tendon. The main function of the posterior tibial tendon is to invert the midfoot and lock the transverse tarsal joints (talonavicular and calcaneocuboid joints). When the tendon fails to function properly, a progressive flatfoot deformity develops. Because the disease process is a continuum, a staging system has been devised to offer guidelines for nonoperative and operative treatment of this problem. The rationale for nonoperative treatment of this disorder is to support the longitud

Ankle joint equinus plays a significant role in the pathogenesis of adult and pediatric flatfoot. The surgical management of ankle equinus is a widely debated topic, and procedure selection is often based on surgeon preference because there is no consensus regarding the superiority of a single procedure. Gastrocnemius recession offers acceptable cosmesis and minimizes perceived weakness, yet requires increased operating time and is indicated only in mild to moderate contractures. Whereas tendo-achilles lengthening is efficient and technically undemanding, and one may achieve large amounts of r

Pseudoankylosis of temporomandibular joint (TMJ) is characterized by limited mouth opening and impaired mandibular mobility induced by pathologic factors outside the joint itself, usually leading to compromised speech, swallowing, and breath functions. Multiple surgical approaches or reconstructive procedures have been proposed to resolve the joint pseudoankylosis and restore the mandibular movement for these affected patients. Free forearm flap has been widely used in reconstruction for various congenital or acquired defects or deformities; however, this flap has been rarely employed for TMJ
