Distal radioulnar joint dorsal instability treated with dorsal capsular reconstruction.

Six patients with post-traumatic distal radioulnar joint dorsal instability were treated with dorsal capsular reconstruction. This new technique of reconstruction requires less extensive dissection than the previously described methods in the literature and requires no tendon graft. A total of six patients treated from 1999 to 2001 were included in this study. Two were males and four were females. The average age of patients was 30 years and all the instabilities were secondary to trauma. One of them had associated minimally displaced distal radius fracture. All patients had arthrogram and art

Split biceps femoris tendon reconstruction for proximal tibiofibular joint instability.

Recurrent instability of the proximal tibiofibular joint is an infrequently diagnosed abnormality. We present a new technique for reconstructing the joint using a split biceps femoris tendon passed through a bone tunnel in both the proximal tibial metaphysis and fibular head. The case report is also presented. The procedure offers an anatomic reconstruction and firm stabilization. It allows normal motion of the proximal tibiofibular joint and preserves the normal mechanics of the ankle. This procedure is an excellent alternative to resection of the fibular head, transarticular arthrodesis, or

Capsular elasticity and joint volume in recurrent anterior shoulder instability.

It is commonly claimed that unstable shoulders have an altered capsular elasticity and increased joint volume. To find out if there is a correlation between anterior instability and increased joint volume, 20 patients were examined while undergoing arthroscopic surgery of the shoulder. There were four anterior subluxators, 11 anterior dislocators, and five bilateral dislocators, and in all patients the contralateral shoulder was also examined. During continuous saline infusion, a pressure/volume curve was recorded within the range of 0-180 mm Hg. Joint volume was correlated with body surface a

[Surgical treatment of chronic upper ankle joint instability in childhood].

The Department of Traumatology of the Hanover Medical School treated 347 patients having chronic instability or second stage rupture of the fibular ligament system, by surgery, during 1971 through 1986. Children were represented by one-seventh of the total number of patients only (13%; n = 44); however, in children the preoperative and intraoperative findings are different from normal findings and must be treated surgically in a special way. Whereas in adults old osseous/osteochondral ligament tears are apt to be rare (16% of the cases), they can be seen in children in almost every second case

Anterior instability in the ankle joint following acute lateral sprain.

The relation between anterior instability in the ankle joint and rupture of the anterior talofibular ligament was investigated in patients with acute ankle sprains. Various techniques were used to provoke anterior displacement of the talus. The actual condition of the anterior talofibular ligament was assessed at operation. The appropriate radiographic method was defined, and its valve in diagnosing an acute rupture of the anterior talofibular ligament was assessed.

Is It Necessary to Repair the Plantar Plate in the Management of Metatarsophalangeal Joint Instability?

Lesser metatarsophalangeal joint (MTPJ) instability is a common cause of metatarsalgia and is associated with plantar plate pathology. The instability is often due to chronic attenuation and attritional changes of the plantar plate and collateral ligaments, leading to eventual rupture. The plantar plate and collateral ligaments serve as the primary stabilizers of the MTPJ, and rupture or attenuation of these ligaments leads to instability. Surgical intervention is often necessary for plantar plate tears, with various approaches and techniques available.

Association between metatarsophalangeal joint instability and sensory disturbances in the toes.

BACKGROUND: Metatarsophalangeal (MTP) joint instability may cause forefoot sensory disturbances, often misattributed to Morton's neuroma. This study investigated the association between MTP joint instability and sensory disturbances. METHODS: Thirty-three patients were enrolled, including 58 feet in total. All patients underwent surgery for moderate to severe hallux valgus, and in unilateral cases, the contralateral foot was also included when eligible. MTP instability was classified as stable, subluxed, or dislocated using the drawer test under fluoroscopy. Sensory disturbances in the 2nd-4t

Treatment of Acromioclavicular Joint Instability With a Tunnel-Free Reconstruction Technique.

Acromioclavicular joint injuries are a common shoulder injury encountered by orthopaedic surgeons. Many different surgical techniques have been described for the operative treatment of these injuries with no single, clear gold standard technique on which surgeons agree. Among the most common complications after surgical management of acromioclavicular injuries are loss of reduction, infection, fracture of clavicle or coracoid, and need for reoperation. We propose an arthroscopic-assisted, tunnel-free surgical technique using a tibialis anterior allograft combined with a FiberTape Cerclage (Art

Plantar Plate Repair for Metatarsophalangeal Joint Instability of the Lesser Toes.

The plantar plate is a known stabilizer of the lesser toe metatarsophalangeal (MTP) joint. MTP instability is a known common cause of metatarsalgia, most commonly in the second toe. In the last decade, clinical staging and anatomic grading mechanisms have been published to guide the surgeons on the treatment of MTP instability; this has also led to an understanding of how plantar plate tears relate to MTP joint instability. Direct surgical repair of the plantar plate has been described, short-term outcomes have been published, and the results are not perfect, but promising with respect to pati

Internal joint stabilizer for varus posteromedial rotatory instability of the elbow.

BACKGROUND: Management of varus posteromedial rotatory instability (VPMRI) of the elbow presents a challenging problem, sometimes requiring supplemental external fixation or ulnohumeral cross-pinning. A relatively new treatment adjunct for posterolateral rotatory instability of the elbow has become available with the development of the internal joint stabilizer (IJS) implant. However, this has not been studied for VPMRI because the implant is thought to be ineffective at treating this instability when placed in the usual manner on the lateral side of the elbow. We hypothesize that modifying th

Dynamic joint stiffness of the ankle in chronic ankle instability patients.

BACKGROUND: While Individuals with chronic ankle instability (CAI) exhibit altered ankle joint movement and moments during stance phase of gait, the interaction or dynamic joint stiffness (DJS) between these is not fully understood. Little attention has been placed on DJS during gait, limiting our understanding of how the most common dynamic task during daily life could affect cartilage loading. RESEARCH QUESTION: Do Individuals with CAI exhibit altered ankle DJS and mechanical energy exerted at the ankle joint during stance phase of gait? METHODS: Eighty-four physically active individuals,

Glenohumeral Joint Instability: A Review of Anatomy, Clinical Presentation, and Imaging.

The glenohumeral joint is intrinsically predisposed to instability because of the bony anatomy but maintained in alignment by many important structures, including the glenoid labrum, glenohumeral ligaments (GHLs), and muscles and tendons. Trauma and overuse can damage these stabilizers, which may then lead to subluxation or dislocation and eventually recurrent instability. This is most common in the anterior direction, which has several recognizable patterns of injury on advanced imaging, including humeral Hill Sachs deformities, bony Bankart lesion of the anteroinferior glenoid, soft tissue B

Instability of the proximal radioulnar joint in Monteggia fractures-an experimental study.

BACKGROUND: A Monteggia fracture is defined as a fracture of the proximal ulna combined with a luxation of the radial head. The aim of the present work is to evaluate the extent of instability of the radius head in the proximal radioulnar joint (PRUJ) as a function of the severity of elbow fracture and ligamentous injury in an experimental biomechanical approach. METHODS: Eight fresh-frozen cadaver arms were used. All soft tissues were removed except for the ligamentous structures of the PRUJ and forearm. A tensile force of 40 N was exerted laterally, anteriorly or posteriorly onto the proxim

Instability and Arthritis of the Distal Radioulnar Joint: A Critical Analysis Review.

Arthritis and instability represent 2 of the most common pathological processes affecting the distal radioulnar joint (DRUJ). These conditions can present in isolation or as components of a multifactorial process. Nonoperative treatment is indicated for most acute injuries to the DRUJ. The joint should be immobilized in a position of stability to allow for ligament healing. Likewise, early arthritis responds favorably to rest, immobilization, corticosteroids, and nonsteroidal anti-inflammatory drugs (NSAIDs). When DRUJ instability is refractory to nonoperative measures, native ligament repair

Instability and degenerative arthritis of the sternoclavicular joint: a current concepts review.

BACKGROUND: Injuries to the sternoclavicular (SC) joint typically occur with high-energy mechanisms such as those obtained in automobile accidents or contact sports. Many disorders of the SC joint can be treated nonoperatively. However, surgical treatment may be indicated for locked posterior dislocations; symptomatic, chronic instability; or persistent, painful osteoarthritis that fails nonoperative therapy. PURPOSE: To provide an updated review on the current diagnosis and management of instability and degenerative arthritis of the SC joint. STUDY DESIGN: Current concepts review. METHODS:

Joint instability and cartilage compression in a mouse model of posttraumatic osteoarthritis.

Joint instability and cartilage trauma have been previously studied and identified as key mediators in the development of posttraumatic osteoarthritis (PTOA). The purpose of this study was to use an in vivo model to compare the effect of joint instability, caused by the rupture of the anterior cruciate ligament (ACL), versus cartilage compression. In this study, mice were subjected to cyclical axial loads of twelve Newtons (N) for 240 cycles or until the ACL ruptured. One and eight weeks after this procedure, knees were sectioned coronally and evaluated for osteoarthritis by histology. Using a

The effect of joint instability on latency and recruitment order of the shoulder muscles.

Several shoulder dysfunctions are generally characterized as Shoulder Impingement Syndrome (SIS). Specifically, glenohumeral instability (GI) has been viewed as a primary cause of SIS in young individuals, mainly in overhead athletics. Past studies have associated GI with modifications in latency, recruitment order and/or EMG activity. However, it is not clear if pain and/or joint instability can account for these observed changes. The aim of this investigation was to analyze the effect of glenohumeral instability on the latencies and recruitment order of the superficial muscles of the glenohu

Complications of the treatment of the acromioclavicular and sternoclavicular joint injuries, including instability.

Treatment of AC joint injuries and SC joint injuries continues to evolve. The risk of complications of both the operative and nonoperative management of these injuries can be minimized by the treating physician if the physician thoroughly evaluates and understands the problem. Making an accurate diagnosis of the underlying pathology and then selecting the appropriate treatment for this will minimize the risk of an associated complication. Paying attention to detail and using the appropriate technique before any operative intervention is chosen will decrease the risk of failure and complication
