Toxicologic findings in suicide: a 10-year retrospective review of Kentucky medical examiner cases.

Toxicologic analysis is an integral component in the investigation of suicide and requires correlation with a detailed scene inspection, with an extensive exploration into the decedent's medical and social background to uncover suicidal ideation or intent and a postmortem examination of the body. In this review, the authors analyzed 2864 cases classified as suicide upon autopsy and toxicologic examinations between 1993 and 2002 in the Kentucky Division of Medical Examiner's Services. Blood and urine were collected in 95.0% and 72.3% of cases, respectively. A total of 32.5% of the victims had n

Community-level wastewater influenza concentrations and school attendance in Jefferson County, Kentucky (2022-2025).

Community-transmitted seasonal infectious diseases pose a significant challenge to school systems across the nation. School attendance is central to student learning progress and is a key public health indicator. We aimed to evaluate the incremental explanatory value of community-level seasonal influenza wastewater concentrations for estimating school absenteeism within the corresponding geographic sewershed in Louisville, Kentucky (USA). We used community wastewater seasonal influenza concentrations from a catchment area that geographically encompassed 51 public elementary, middle, and high s

Perceived Social Isolation and Fentanyl Use Among Adults Who Inject Drugs in Rural Kentucky.

Despite evidence linking perceived social isolation (PSI) with substance use disorders (SUDs), limited research has examined its effect on fentanyl use among people who inject drugs (PWID). Cross-sectional data of 350 PWID in Appalachian Kentucky were utilized to examine the association between PSI and lifetime fentanyl use. Study findings highlighted a significant association between PSI and lifetime fentanyl use. Additionally, PWID with higher scores on the PSI scale tend to use fentanyl less frequently, suggesting that social isolation may serve as a protective factor against fentanyl use w

We Drove to the Moon: Ensuring Methadone Access in Rural Kentucky Through Peer Transportation.

Methadone, a gold standard treatment for opioid use disorder, faces limited access due to federal regulations restricting its dispensing to licensed opioid treatment programs (OTPs). Transportation is a critical barrier reported by rural area residents, where the distances to OTPs are over six times farther than for urban residents. To overcome this barrier in rural Kentucky, the HEALing (Helping to End Addiction Long-term SM ) Communities Study (HCS) partnered with Voices of Hope, a local recovery community organization, to develop a peer transportation program as part of the Recovery Coach L

Prevalence and Treatment for Alcohol Use Disorders Based on Kentucky Medicaid 2012-2019 Datasets.

Alcohol use is the leading substance use in the United States. Persons with alcohol use disorder (AUD) face enormous health consequences and family problems. Analysis of Medicaid enrollee data is critical to understand different aspects of AUD and the treatment utilization for patients with AUD. Yearly patient-level data were constructed from the Kentucky 2012-2019 Medicaid claims data. ICD-9-CM and ICD-10-CM codes were used to identify patients with AUD and their comorbid conditions, the 11-digit National Drug Codes were used to identify medication treatments, and procedure codes were used to

Characterization of diverted buprenorphine use among adults entering corrections-based drug treatment in Kentucky.

BACKGROUND: Illicit, medically unsupervised use of buprenorphine (i.e., "diverted use") among vulnerable and underserved populations, such as corrections-involved adults, remains underexplored. METHODS: Survey data (2016-2017) collected as part of a clinical assessment of incarcerated adults entering corrections-based substance use treatment in Kentucky were analyzed. For years examined, 12,915 completed the survey. Removing cases for participants who did not reside in Kentucky for >6 months during the one-year pre-incarceration period (n = 908) resulted in a final sample size of 12,007. RES

Salmonella enterica serovar Kentucky recovered from human clinical cases in Maryland, USA (2011-2015).

Salmonella Kentucky is among the most frequently isolated S. enterica serovars from food animals in the United States. Recent research on isolates recovered from these animals suggests there may be geographic and host specificity signatures associated with S. Kentucky strains. However, the sources and genomic features of human clinical S. Kentucky isolated in the United States remain poorly described. To investigate the characteristics of clinical S. Kentucky and the possible sources of these infections, the genomes of all S. Kentucky isolates recovered from human clinical cases in the State o

Characteristics of Prescription-Opioid-Impaired and Other Substance-Impaired Drivers in Rural Appalachian Kentucky.

Previous studies have highlighted the prescription opioid epidemic in rural Appalachia and its associated risk behaviors; however, no studies have examined prescription-opioid-impaired driving as a consequence of this epidemic. The purpose of the present study was to describe prescription-opioid-impaired drivers in rural Appalachian Kentucky and examine how they are similar to and different from other substance-impaired drivers from the region. A sample of convicted DUI offenders from rural Appalachian Kentucky completed a confidential research interview focused on their substance use, mental

Deer stand fatalities in Kentucky: two cases of reverse suspension and blunt force trauma.

Hunting many types of wild game is an avidly pursued outdoor activity that attracts all ages and both genders at various times of the year. Deer hunting is a popular sport in many regions of North America. A variety of weapons are used in the hunting, trapping, and killing of game. As a variety of different modalities are used, myriad types of injuries unique to the type of hunting can occur. Most deer hunting-related fatalities identified at the Office of the Chief Medical Examiner in Kentucky are accidental firearm injuries. Less commonly encountered are fatalities resulting from elevation o

Agricultural injuries among older Kentucky farmers: The Farm Family Health and Hazard Surveillance Study.

This population-based study reports the cumulative incidence of agricultural injuries during a 1-year period in a sample of 998 farmers aged 55 years and older living in Kentucky. A total of 98 farm-related injuries were reported among 88 older farmers for a crude injury rate of 9.03 injured farmers per 100 farmers (95% confidence interval (CI) = 7.03-11.03) over a 1-year period. The leading external causes of farm injury were falls (24.9%), machinery (22.5%), wood-cutting (14.6%), and animal-related events (14.3%). Farmers working on farms with beef cattle (alone) (odds ratio = 1.90; 95% CI =

County-level surveillance for the American dog tick (Dermacentor variabilis) and Rickettsia species in Kentucky.

Spotted fever group (SFG) rickettsiosis includes a group of illnesses with similar symptoms caused by Rickettsia species bacteria. While Rickettsia rickettsii, the agent of Rocky Mountain spotted fever (RMSF) is a well-known pathogen, other SFG Rickettsia spp. can infect ticks and occasionally contribute to human disease. We conducted county-level surveillance of Dermacentor variabilis ticks in Kentucky to characterize the distribution and diversity of SFG Rickettsia. Ticks were collected from the environment from 2019 to 2024 by dragging a white cloth and through our Kentucky Tick Submission

Colon cancer in Appalachian Kentucky: Unique genetic, microbiome and obesity findings in a cohort comparison.

We investigated colon cancer genomics and microenvironmental features in the Appalachian Kentucky population, a group with the highest incidence of colon cancer in the United States. We assessed two inter-related risk factors for colon cancer (obesity and abnormal gut bacterial microbiome) and their genetic associations within this population. To evaluate potential unique characteristics of the high-incidence cohort, we compared 99 propensity-matched colon cancer tumors from Appalachian Kentucky patients to 95 non-Appalachian patient tumors to evaluate driver mutations, differentially expresse

Highly drug resistant clone of Salmonella Kentucky ST198 in clinical infections and poultry in Zimbabwe.

A highly multidrug-resistant strain of Salmonella enterica serotype Kentucky (S. Kentucky) of sequence type (ST)198 emerged in North Africa and has since spread widely. To investigate the genetic diversity and phylogenetic relationship of S. Kentucky in Zimbabwe and identify potential sources of infection, the whole-genome sequence of 37 S. Kentucky strains isolated from human clinical infections and from poultry farms between 2017 and 2020 was determined. Of 37 S. Kentucky isolates, 36 were ST198 and one was ST152. All ST198 isolates had between six and fifteen antimicrobial resistance (AMR)

Management practices associated with strongylid parasite prevalence on horse farms in rural counties of Kentucky.

Anthelmintic resistance among cyathostomin parasites is a wide-spread problem. The parasite control guidelines written by the American Association of Equine Practitioners (AAEP) encourages the preservation of anthelmintic efficacy by reducing treatment frequency, using targeted deworming, and implementing environmental management practices. While there is knowledge regarding parasite management practices of affluent horse farms in the United States, surveys rarely explore the rural and underserved regions. The purpose of this study was to observe the management practices of horse farms in rura

CRISPR Typing and Antibiotic Resistance Correlates with Polyphyletic Distribution in Human Isolates of Salmonella Kentucky.

Although infrequently associated with reported salmonellosis in humans, Salmonella enterica, subsp. enterica serovar Kentucky (ser. Kentucky) is the most common nonclinical, nonhuman serovar reported in the United States. The goal of this study was to use Clustered Regularly Interspaced Short Palindromic Repeats (CRISPR)-multi-virulence-locus sequence typing (MVLST) to subtype a collection of human clinical isolates of ser. Kentucky submitted to the Pennsylvania Department of Health and to determine the extent of antibiotic resistance in these strains. This analysis highlighted the polyphyleti
