INTRODUCTION AND IMPORTANCE: Traumatic open knee dislocation is a rare, severe injury characterized by severe ligamentous destruction and a high frequency of infection and neurovascular involvement. Delayed treatment of these injuries is complicated, necessitating the intervention of not only a skilled orthopedic surgeon but also a plastic surgeon. To the best of our knowledge, this is the first case of delayed open knee dislocation faced by a practicing surgeon in an underdeveloped country (Somalia) with a successful outcome.

Closed reduction attempts may be unsuccessful after traumatic knee dislocations on rare occasions. The interposition of the soft tissues on the medial aspect of the joint into the femoral condyle and tibial plateau is shown to be the cause of an unsuccessful reduction. In such cases, open reduction is the recommended method of treatment. In our study, we presented a 16-year-old male with an open knee dislocation after a motorcycle accident. As our closed reduction attempt failed, open joint reduction and repair of the medial collateral ligament and retinaculum was performed in the first stage

This is a report about bilateral dislocations of the knee joints in one patient with arterial and venous stop because of endothelial lesions and thrombosis of the popliteal vessels on both sides. After reconstruction of circulation by means of vena saphena, the disrupted knee ligaments were also sutured. Large ventral exposure of the knee joint is recommended for both reconstruction of vessels and ligaments.

Popliteal artery injury is a hazardous and often subtle complication of traumatic knee dislocation. Review of the literature in which series of knee dislocations were reported has revealed a 28% incidence of thrombosis or rupture of this critical vessel. Fifty-seven percent of these patients required leg amputation. The most frequent cause of limb loss has been absence or delay of direct surgical repair of the associated popliteal artery injury. This report describes a management plan that is intended to minimize the likleihood of future disabling leg amputations following knee dislocation.

BACKGROUND: Chronic knee fracture-dislocation is an exceptionally rare and disabling condition, most often resulting from neglected trauma. In elderly patients with degenerative changes and extensive ligamentous insufficiency, total knee arthroplasty (TKA) may represent a reliable salvage procedure to restore alignment, stability, and function.
CASE PRESENTATION: A 76-year-old woman with hypertension and type 2 diabetes presented four months after a fall with a chronic posterolateral dislocation of the left knee. Clinical examination revealed a fixed varus deformity and posterior tibial trans

INTRODUCTION: Multisystem injuries affecting vasculature and nerves can be detrimental sequelae of knee dislocations (KDs). The purpose of this study was to provide an update of the current literature since the conducted search used in the publication of Medina et al on May 3, 2013, to evaluate (1) the frequency of vascular and neurologic injury after KD, (2) the frequency with which surgical intervention was conducted for vascular injury in this setting, and (3) the frequency with which each imaging modality was used to detect vascular injury.

BACKGROUND: Dislocation of the knee is a serious and potentially limb-threatening injury. There are three types of dislocation around the knee joint: patellofemoral, tibiofemoral, and tibiofibular. Tibiofemoral dislocation is the variant that is deemed the most serious, with a higher risk of compromise to the popliteal artery and common peroneal nerve. Although simultaneous dislocations of two types have been described, there has been no such description of all three types occurring simultaneously.

Traumatic knee dislocation is a serious and potentially limb threatening injury that can be easily missed if meticulous history and examination have not been employed. Neurovascular injuries are common in this condition, and due diligence should be given to their thorough evaluation at time of secondary survey so as to avoid complications such as ischaemia, compartment syndrome and eventual amputation. There is growing evidence in the literature that morbid obesity is associated with low energy knee dislocation, therefore this should be considered when assessing this cohort of patients present

Dislocation of a total knee arthroplasty is a rare but serious complication. In previous literature, when dislocation does occur, it is usually in the posterior direction in cases with a posterior stabilized total knee arthroplasty due to cam jump. We report an unusual case of anterior dislocation of an 11-year-old posterior stabilized total knee arthroplasty in a 55-year-old woman with rheumatoid arthritis occurred after a slip.

We report on a 68-year-old woman with gonarthrosis who underwent total knee arthroplasty. Having initially achieved a satisfactory result, she developed at 5 months postoperation an irreducible flexion contracture necessitating revision surgery; but no pathological findings were discovered. In the immediate postoperative period, the patient developed a severe spasm of the hamstring muscles and a paralysis of the external popliteal sciatic nerve with a posterior dislocation of the knee. After reduction, an electromyography study showed an alteration in medullary sensitive conduction; and the ma

BACKGROUND: Congenital knee dislocation (CDK) is a rare congenital deformity, which often requires surgery for treatment. Little objective data exist characterizing the outcome of patients who require operative treatment for this condition. The purposes of this study were to objectively evaluate the functional, clinical, and gait outcomes of patients who underwent surgical treatment of CDK; and compare the results of outcome between 2 surgical approaches for this condition: quadricepsplasty and femoral shortening.
METHODS: We performed a retrospective review of all patients (7) treated surgic

A 59-year-old woman with bilateral congenital knee dislocation due to Larsen syndrome was treated by arthroplasty. To prevent neurovascular deficiency and major loss of bone substance, a continuous joint distraction was performed by unilateral external fixation. After a period of 25 days and a leg lengthening of 4.5 cm, a constrained total knee endoprosthesis was implanted. Both operations and their postoperative courses were free of complications. A previously planned lengthening of the Achilles tendon was unnecessary.

Knee dislocations are uncommon as isolated orthopaedic injuries, but their prevalence is increased in the setting of high-energy multiple trauma. In these circumstances, it is important for the orthopaedic clinician to recognize and appropriately treat the knee dislocation in parallel with other significant injuries. We report the case of an 18-year-old woman who was involved in a motor vehicle collision and sustained multiple injuries in addition to a posterolateral knee dislocation. An attempt at reduction was made in the initial trauma setting, and the patient was medically stabilized. She

Posterolateral knee dislocation is a small subset of knee dislocations. Irreducible posterolateral dislocation has been reported and is caused by buttonholing of the medial femoral condyle into the anteromedial knee capsule, with interposition of the medial retinacular structures between the femoral and tibial condyles. Open reduction has been advocated to reduce the knee. We present a case of chronic irreducible posterolateral dislocation of the knee for 14 months associated with anterior and posterior cruciate ligament (ACL, PCL) and medial collateral ligament (MCL) rupture. The patient pres

Knowing that the injured MCL and PCL can heal and that the injured ACL and lateral side, predictably, will not heal are the bases for the treatment approach to these injuries. (1) Allow MCL healing nonoperatively. (2) Allow PCL healing to occur as long as PCL laxity is 2+ or less (reconstruct the PCL acutely if posterior drawer is > 2+ initially). (3) Initially delay ACL treatment and reconstruct later, if needed posterior drawer. (4) Perform acute lateral side repair to reattach structures to their distally torn site.

A high-velocity knee dislocation is a true orthopedic emergency. A high index of suspicion is necessary to identify reduced knee dislocations in multiple trauma patients. Coexisting injuries are common in patients with high-velocity knee dislocations. After an initial assessment, the knee joint should be reduced immediately and splinted. Popliteal artery injury is common in high-velocity dislocations. Arteriograms can be helpful but should not delay operative treatment of obvious arterial injuries. Early restoration of flow to the ischemic limb will avoid amputation in most patients. The ortho

BACKGROUND: Congenital dislocation of the knee is characterised by excessive knee extension or dislocation and anterior subluxation of the proximal tibia, and this disease can occur independently or coexist with different systemic syndromes. Nevertheless, significant controversy surrounds treating this disease when combined with hip dislocation. This paper presents a case of a 4-month-old patient diagnosed with bilateral hip dislocation combined with this disease. The study discusses the pathophysiology, diagnosis, and treatment methods and reviews relevant literature.
