The floating knee: a review on ipsilateral femoral and tibial fractures.

In 1975, Blake and McBryde established the concept of 'floating knee' to describe ipsilateral fractures of the femur and tibia.1 This combination is much more than a bone lesion; the mechanism is usually a high-energy trauma in a patient with multiple injuries and a myriad of other lesions. After initial evaluation patients should be categorised, and only stable patients should undergo immediate reduction and internal fixation with the rest receiving external fixation.Definitive internal fixation of both bones yields the best results in almost all series.Nailing of both bones is the optimal fix

Popliteal artery injuries associated with fractures and dislocations about the knee.

The authors present the results of a retrospective review of popliteal artery injuries associated with fractures and dislocations about the knee. They treated 41 patients with popliteal artery injuries associated with either fractures about the knee or knee dislocations. Thirty-five of the patients were males, 6 females; the mean age was 23 years. The delay before accessing the hospital was 17 hours (range: 3 hours to 10 days). Thirty-two fractures were open. Together with the vascular injury, 12 femoral fractures, 20 tibial and fibular fractures, 5 knee dislocations, 4 femoral + tibial fractu

Periprosthetic tibial fractures after cementless low contact stress total knee arthroplasty.

Periprosthetic fractures are a recognized complication of total knee arthroplasty. Fractures may occur intraoperatively or postoperatively, and risk factors have been identified that may predispose an individual to such a complication. We report 7 cases of periprosthetic tibial fractures after low contact stress total knee arthroplasty, a complication encountered by the senior author (D.E.B.) only after a change in practice from a cemented implant to a cementless one. In light of this previously unreported complication in our unit, we attempted to identify common features within this group of

Operative treatment of distal femoral fractures proximal to total knee replacements.

Twenty fractures of the distal part of the femur proximal to a total knee replacement were treated operatively by members of the New England Trauma Study Group. Notching of the anterior aspect of the femoral cortex was associated with only two of these fractures, and none of the knee prostheses was loose at the time of the fracture. All twenty fractures were treated with open reduction and stable internal fixation, and the operation on fifteen fractures was supplemented with bone grafts. Every fracture healed, and eighteen healed after a mean of sixteen weeks (range, six to forty weeks). Union

Knee instability after fractures of the intercondylar eminence of the tibia.

Fifteen children with fracture of the intercondylar eminence of the tibia were reviewed. Three with type I or type II fractures had closed treatment. Twelve with type III fractures had open reduction and internal fixation. Follow-up examination included history and examination of the knee; nine had follow-up radiographs. The mean follow-up period was 7 years. Seven patients were free of symptoms; eight had varying degrees of pain, of which four had to avoid some athletic activities; and two described subluxation episodes. All had normal motion, four had atrophy, and all had some evidence of an

Management of the Floating Knee: Ipsilateral Fractures of the Femur and Tibia.

The "floating knee" is defined as fractures of the ipsilateral femur and tibia, which consists of a spectrum of injury, and may be in isolation or part of multiple system trauma for a given patient. A floating knee may compromise limb viability due to severe soft-tissue and vascular injury. Expeditious fracture reduction and patient resuscitation are crucial, while type and timing of provisional and definitive management is guided by the extent of injury to the involved extremity and associated systemic injuries. Numerous surgical techniques are available to treat the floating knee, including

Fracture Blisters After Primary Total Knee Arthroplasty.

Fracture blisters are tense vesicles that arise on markedly swollen skin overlying traumatized soft tissue. While this relatively uncommon complication has been well described in the trauma literature, this article reports for the first time a case of fracture blisters after primary total knee arthroplasty. The fracture blisters developed within 36 hours of surgery and were associated with profound swelling and erythema. There was no evidence of vascular injury, compartment syndrome, iatrogenic fracture, or deep venous thrombosis. The patient was treated with leg elevation, loosely applied non

[A boy with a painful knee: bone tumour or stress fracture?].

BACKGROUND: The symptoms of a stress fracture are almost identical to those of most bone tumours. Even with the use of various imaging techniques, it can be difficult to establish the correct diagnosis. Although a primary bone tumour requires early treatment to improve its prognosis, the discriminative factor in the diagnosis of a stress fracture is its clinical development over time. CASE DESCRIPTION: A 10-year-old boy was referred to our outpatient clinic on the suspicion of a primary bone tumour in his right tibia. CONCLUSION: A case was once described in this journal in which a stress fr

The operative management of osteoporotic fractures of the knee: to fix or replace?

This review considers the surgical treatment of displaced fractures involving the knee in elderly, osteoporotic patients. The goals of treatment include pain control, early mobilisation, avoidance of complications and minimising the need for further surgery. Open reduction and internal fixation (ORIF) frequently results in loss of reduction, which can result in post-traumatic arthritis and the occasional conversion to total knee replacement (TKR). TKR after failed internal fixation is challenging, with modest functional outcomes and high complication rates. TKR undertaken as treatment of the i

[Fracture of tibial stem in revision total knee arthroplasty].

An unusual case of implant failure after revision total knee arthroplasty (TKA) is presented and the case report is completed with the results of an ensuing biomechanical study made to explain possible causes leading to a fracture of the tibial stem. For this we used electron microscopic and biomechanical analyses. There was no material defect found at the site of fracture. By computerized modelling of the clinical situation, however, we found out that asymmetrical progressive osteolysis of the proximal tibia could have caused significant changes in mechanical straining associated with qualita

Periprosthetic patellar fracture after an open knee dislocation.

We present a patient who sustained an open knee dislocation with a periprosthetic fracture of the patella and who was treated satisfactorily with tension band wiring and a conservative rehabilitation program. In addition to describing this unique combination of injuries, we review the literature on the treatment of periprosthetic patellar fractures. Although internal fixation of these fractures historically has provided unpredictable results, we think that it is warranted if the patella has adequate bone stock.

Patella fracture after post total knee replacements.

A retrospective study was done to determine the incidence, clinical function, and complications of treatment in a large series of patella fractures after total knee replacement (TKR). Between 1983 and 1996, 4583 primary AGC TKRs done at our institution. One hundred seventy-seven fractures were identified in 135 patients for a fracture incidence of 3.8%. Twenty-two fractures were vertical and had a stable implant and intact extension mechanism (Type 1). Twenty-one fractures had disruption of the extensor mechanism of less than 1 cm (Type 2A). Seventeen fractures had disruption of the extensor m

Spontaneous osteonecrosis of the knee: the result of subchondral insufficiency fracture.

BACKGROUND: Spontaneous osteonecrosis of the knee is a superficial subchondral lesion classically seen in the medial femoral condyle; in general, it is markedly different in its clinicopathological presentation from the classic wedge-shaped subchondral osteonecrotic lesions seen in the hip, knee, and other joints. Recent reports on subchondral insufficiency fracture of the femoral head, which has marked morphological similarities with spontaneous osteonecrosis of the knee, led us to reevaluate a series of patients who had had operative treatment because of a clinical and pathological diagnosis

Supracondylar femoral fracture following total knee arthroplasty.

Twenty-nine supracondylar femoral fractures above total knee arthroplasty were studied retrospectively. Group 1 consisted of 5 nondisplaced fractures managed with closed treatment, yielding 5 satisfactory results. Group 2 consisted of 9 displaced fractures managed with closed treatment. There were no satisfactory results in Group 2; there were 8 malunions and 2 knees requiring revision. Group 3 consisted of 15 displaced fractures managed with open reduction and internal fixation. There were 10 satisfactory results in Group 3; there were 2 malunions and 3 knees requiring revision or repeat fixa

[Evaluation of knee joint function after fracture of the patella].

Clinical assessment of the function of the knee joint after surgically and conservatively treated fracture of the patella was carried out. On the basis of C. Ranawat criteria it was found that regardless the method of treatment applied restriction of the joint function was expressed mainly by limited range of motion and by weakening of the knee extensors. The results of surgical and conservative management are similar. The best results of treatment for fracture of the patella were achieved in patients operated with the use of wire loop-cerclage.

Fractures of the tibial spine in children. An evaluation of knee stability.

Forty-five patients with fractures of the tibial spine were reviewed 3 to 10 years after injury in order to determine the degree of residual laxity of the cruciate or collateral ligaments. After fractures which had been partially or completely displaced, some anterior cruciate laxity was evident, even if patients were asymptomatic. It was also found that an anatomical reduction did not prevent either laxity or some loss of full extension of the knee.

Increased risk of knee osteoarthritis progressing to total knee arthroplasty following patella fractures: an age stratified population analysis.

INTRODUCTION: Patella fractures are articular injuries that can alter knee biomechanics, disrupt joint contact forces, and promote cartilage degeneration. However, progression to reconstructive surgery is not fully characterized. This study investigates the risk of progression to total knee arthroplasty (TKA) following patella fractures and assesses whether initial operative versus nonoperative management impacts this risk. METHODS: We retrospectively analyzed TKA progression in patients with patella fractures using synthetic data from a large Level I academic trauma center (1996-2024). Patie
