Advanced hemodynamic monitoring in critically ill children.

Circulatory shock is an important cause of pediatric morbidity and mortality and requires early recognition and prompt institution of adequate treatment protocols. Unfortunately, the hemodynamic status of the critically ill child is poorly reflected by physical examination, heart rate, blood pressure, or laboratory blood tests. Advanced hemodynamic monitoring consists, among others, of measuring cardiac output, predicting fluid responsiveness, calculating systemic oxygen delivery in relation to oxygen demand, and quantifying (pulmonary) edema. We discuss here the potential value of these hemod

A comparative analysis of conventional cytopreparatory and liquid based cytological techniques (Sure Path) in evaluation of serous effusion fluids.

BACKGROUND: Clinically, detection of malignant cells in serous body fluids is critical, as their presence implies the upstaging of the disease. Cytology of body cavity fluids serves as an important tool when other diagnostic tests cannot be performed. In most laboratories, currently, the effusion fluid samples are analysed chiefly by the conventional cytopreparatory (CCP) technique. Although, there are several studies comparing the liquid-based cytology (LBC), with CCP technique in the field of cervicovaginal cytology; the literature on such comparison with respect to serous body fluid examina

Differential impact of neutrophil-to-lymphocyte ratio and time-weighted NLR on mortality and survival in critically ill children: insights from a retrospective study.

BACKGROUND: The intensive care of critically ill children is challenging due to diverse etiologies and rapid disease progression. Early identification of high-risk patients is crucial for improving outcomes. The neutrophil-to-lymphocyte ratio (NLR) has emerged as a potential biomarker reflecting the balance between innate and adaptive immune responses, with studies in adults showing its correlation with mortality and survival in intensive care settings. However, its application in pediatric intensive care units (PICU) is less explored. OBJECTIVE: To examine the impact and predictive value of

Systematic bias in point-of-care INR testing: comparative analysis of Abbott and Roche devices.

OBJECTIVES: Point-of-care (POC) International Normalised Ratio (INR) testing provides rapid anticoagulation monitoring. Still, it is susceptible to systematic bias, particularly at supratherapeutic levels, which can lead to inappropriate warfarin dose adjustments and increased thrombotic risk. This study evaluated bias trends between Abbott and Roche POC INR devices. METHODS: Bias trends were assessed using Royal College of Pathologists of Australasia Quality Assurance Programs survey data (2023-2025), in-house verification, and patient sample comparisons against laboratory instrument testing

Validation of the anthrax lethal toxin neutralization assay.

A validation of the performance characteristics of a toxin neutralization assay is presented. This in vitro assay measures the functional ability of antisera, containing antibodies to anthrax lethal toxin, to specifically protect J774A.1 cells against Bacillus anthracis lethal toxin cytotoxicity. This colormetric assay is based upon the reduction of MTT by living cells. Human and rabbit antisera produced against anthrax vaccine absorbed (AVA) were used to validate the assay. Results showed a high level of repeatability and reproducibility, particularly for a bio-assay. Inter-assay variability

Necrotizing Fasciitis Post-Cesarean Section Leading to Transabdominal Hysterectomy.

Necrotizing fasciitis (NF) is a rare but life-threatening disease characterized by rapidly spreading inflammation and subsequent necrosis of the fascial planes and surrounding tissues. Limited literature has described NF as involving an adjacent solid organ beyond fascial planes that has required its removal. We present a case of a 25-year-old white female who underwent a cesarean section and subsequently developed NF involving her uterus and abdominal wall that necessitated a total abdominal hysterectomy, serial surgical debridement of necrotic tissue, and wound vacuum assisted closure (VAC)

Bioelectrical Impedance Analysis Is Not Sufficient for Determining Water Deficit in Hypernatremic Patients.

BACKGROUND Hypernatremia is associated with poor outcomes in critically ill patients, and an accurate assessment of water volume is important to determine appropriate fluid hydration. Bioelectrical impedance analysis (BIA) is a new, noninvasive, and relatively easy method for measuring hydration status. This study aimed to investigate whether bioelectrical impedance measurements of body water could reduce the frequency of blood sampling for fluid replacement in patients with hypernatremia. MATERIAL AND METHODS Fifty-one hospitalized patients were studied with hypernatremia, defined as a serum

Bedside screening of carbapenem-resistant Enterobacteriaceae using selective agar plates: A low-cost, laboratory-independent approach for rural healthcare facilities.

The global surge of carbapenem-resistant Enterobacteriaceae (CRE) poses a critical threat to patient safety in intensive care units (ICUs). In low- and middle-income countries, routine CRE surveillance is often limited by the lack of microbiology laboratory infrastructure and trained personnel in peripheral healthcare facilities, resulting in reliance on empirical broad-spectrum antibiotic therapy and limited capacity to detect silent transmission. This study evaluated the operational feasibility, safety, and phenotypic agreement of a decentralized, laboratory-independent bedside CRE screening

The Role of the Medical Laboratory in Identifying 'Super-Spreaders': Correlation Between Viral Load Kinetics and Human-to-Human Infectivity.

Nipah virus (NiV) is a high-consequence zoonotic henipavirus characterised by severe encephalitis and respiratory distress, driving recurrent outbreaks with case fatality rates between 40% and 75% in India and Bangladesh. Within these outbreaks, 'super-spreading' dynamics play a critical role, where a small subset of individuals contributes to a disproportionately large share of secondary infections. To evaluate the relationship between human infectivity and viral load kinetics in Nipah virus infection and to define the role of the medical laboratory in identifying potential super-spreaders, a

Introduction. Sepsis is a major contributor to the global burden of disease. Its effective treatment is time-critical and relies on timely access to significant blood culture (BC) results.Hypothesis/gap statement. The value of BC results to the requesting service is reduced by delays in report completion. Centralized clinical laboratory networks have little insight into pre-analytical causes of delayed BC results.Aim. We aimed to assess variations in laboratory investigation of bloodstream infection and sepsis throughout a state-wide laboratory network, with the goal to design suitable remedia
