Conversation analysis and talk-extrinsic data in research on speech and language therapy.

The present article addresses the issue of combining conversation analysis (CA) with talk-extrinsic data, with a specific focus on speech and language therapy and education. Even though the use of CA together with other data sources is now generally accepted, particularly in the field of 'applied CA', it is still important to discuss how this is to be done, what pitfalls to avoid, and what methodological problems might remain unresolved. The procedures from a Swedish project on interaction in speech and language intervention that combines CA with interviews and video-based retrospection sessio

The performance of standardized patients in portraying clinical scenarios in speech-language therapy.

BACKGROUND: Standardized patients (SPs) are frequently included in the clinical preparation of students in the health sciences. An acknowledged benefit of using SPs is the opportunity to provide a standardized method by which students can demonstrate and develop their competency. Relatively little is known, however, about the capacity of SPs to offer an accurate and standardized performance across a speech-language therapy student cohort. AIMS: To investigate the accuracy, reproducibility (consistent performance of each SP across student interviews) and replicability (consistent performance o

Speech and language therapy to improve the communication skills of children with cerebral palsy.

BACKGROUND: The production of speech, language and gesture for communication is often affected by cerebral palsy. Communication difficulties associated with cerebral palsy can be multifactorial, arising from motor, intellectual and / or sensory impairments, and children with this diagnosis can experience mild to severe difficulties in expressing themselves. They are often referred to speech and language therapy (SLT) services, to maximise their communication skills and help them to take an independent a role as possible in interaction. This can include introducing augmentative and alternative

Speech and language therapy in hyperactivity: a United Kingdom perspective in complex cases.

In the United Kingdom, multidisciplinary teams within the National Health Service (NHS) usually manage children and adolescents with disorders of hyperactivity. These multidisciplinary teams do not routinely include speech and language therapists (SLTs), and therefore speech and language therapy services for children and adolescents with disorders of hyperactivity are limited. Consequently, research and service delivery for children and adolescents with disorders of hyperactivity in the United Kingdom is only starting to emerge. Two case studies are used to show how speech and language therapy

An analysis of referrals to speech and language therapy in 11 centres, 1987-95.

This retrospective study analyses data collected on an information system developed specifically for the use of speech and language therapists in the management of their services. Eleven healthcare providers have used the system since 1985. This paper reflects data on 73,000 clients referred to these speech and language therapy services between 1987 and 1995. The annual referral rate to these 11 trusts in 1987 was 4129, rising to 11,944 in 1995. The percentage referrals per head of population per year varies between 0.32 and 0.58% of the population. Of referrals to speech and language therapy,

The diversity of speech and language therapy services for aphasic adults in the United Kingdom.

Data on aphasia management were collected by questionnaires to UK speech and language therapists. Returns representing over 90% of health districts were obtained. Variation amongst districts is evident in levels of staffing for the adult neurological case load, and in speed of response to referral, frequency and types of treatment available, and in other forms of support for patients and carers. Although individual therapists or districts are able to adhere to some of the professional recommendations for a 'quality' service, few are able to provide this high level of care in all aspects of aph

Effect of speech-language therapy after low-level laser therapy in patients with TMD: a descriptive study.

Increased pain and/or discomfort during chewing, speaking, and swallowing have been commonly reported by patients with Temporomandibular Dysfunction (TMD). Speech-language pathology therapy (orofacial myofunctional therapy - OMT) has been proposed as part of the treatment for this condition; however, it is a modality that should be introduced when the TMD context and the pain are not accentuated, so that they do not prevent or hinder the performance of exercises. The aim of this study was to analyze the effect of OMT on the treatment of patients with TMD, according to the Research Diagnostic C

Promoting lexical learning in the speech and language therapy of children with cochlear implants.

This study examines lexical intervention sessions in speech and language therapy for children with cochlear implants (CIs). Particular focus is on the therapist's professional practices in doing the therapy. The participants in this study are three congenitally deaf children with CIs together with their speech and language therapist. The video recorded therapy sessions of these children are studied using conversation analysis. The analysis reveals the ways in which the speech and language therapist formulates her speaking turns to support the children's lexical learning in task interaction. Th

Perceptions of Gender and Femininity Based on Language: Implications for Transgender Communication Therapy.

Recent research presents a picture of diminishing gender differences in language. Two experiments examined whether language use can predict perceptions of gender and femininity; one included male and female speakers telling a personal narrative, the other also included male-to-female transgender speakers and analyzed an oral picture description. In each experiment, raters read transcribed samples before judging the gender and rating the femininity of the speaker. Only number of T-units, words per T-unit, dependent clauses per T-unit, and personal pronouns per T-unit emerged as different betwee

The Role of Speech and Language Therapy in Assessing and Managing Pharyngo-esophageal Diverticula.

This study explores the contribution of Speech and Language Therapists (SLTs) to the assessment and management of patients presenting on videofluoroscopic swallow studies (VFSS) with a suspected pharyngo-oesophageal diverticulum. Records for all patients who attended for VFSS in an acute hospital over an eleven-year period were examined (N = 1820). Twenty patients were identified on VFSS as having a suspected diverticulum. Symptoms suggestive of a diverticulum were found during both bedside clinical examination and radiographic examination e.g. respiratory difficulties (n = 15; 75%), voice cha

Integration of literacy into speech-language therapy: a descriptive analysis of treatment practices.

PURPOSE: The purpose of the present study was: (a) to examine the extent to which speech-language therapy provided to children with language disorders in the schools targets code-based literacy skills (e.g., alphabet knowledge and phonological awareness) during business-as-usual treatment sessions, and (b) to determine whether literacy-focused therapy time was associated with factors specific to children and/or speech-language pathologists (SLPs). METHOD: Participants were 151 kindergarten and first-grade children and 40 SLPs. Video-recorded therapy sessions were coded to determine the amount

Comparison of speech and language therapy techniques for speech problems in Parkinson's disease.

BACKGROUND: Patients with Parkinson's disease commonly suffer from speech and voice difficulties such as impaired articulation and reduced loudness. Speech and language therapy (SLT) aims to improve the intelligibility of speech with behavioural treatment techniques or instrumental aids. OBJECTIVES: To compare the efficacy and effectiveness of novel SLT techniques versus a standard SLT approach to treat Parkinsonian speech problems. SEARCH METHODS: We identified relevant, published prior to 11(th) April 2011, by electronic searches of numerous literature databases including CENTRAL, MEDLINE

A multimedia PDA/PC speech and language therapy tool for patients with aphasia.

Aphasia is a speech disorder usually caused by stroke or head injury and may involve a variety of communication difficulties. As 30% of stroke sufferers have a persisting speech and language disorder and therapy resources are low, there is clear scope for the development of technology to support patients between therapy sessions. This paper reports on an empirical study which evaluated SoundHelper, a multimedia application to demonstrate how to pronounce target speech sounds. Two prototypes, involving either video or animation, were developed and evaluated with 20 Speech and Language Therapist

The effects of speech and language therapy for a case of dysarthria associated with Parkinson's disease.

We report the results of a single-subject multiple baseline across behaviours experiment of a 74-year-old woman presenting with Parkinson's disease. Her speech was typical of a hypokinetic dysarthria. The main features of her dysarthric speech were: a restriction in the modulation of fundamental frequency, an inappropriate pitch level and a rate disturbance. Three measures of prosody were operationally defined as follows: (1) linguistic modulation of fundamental frequency, (2) mean fundamental frequency and (3) rate. Treatment focused on ameliorating these aspects employing a multiple baseline
