Voice restoration after laryngectomy.

Speech therapists have traditionally played a major part in voice restoration after laryngectomy. Nurses have a potentially complementary role to play, which is explored in this article. The three main methods of voice restoration are outlined and the nurse's role in the management of laryngectomy patients is discussed.

Laryngeal atresia in the newborn: surgical implications.

Congenital atresia of the larynx is a rare, life-threatening anomaly in which early recognition and appropriate perinatal management are essential for survival. Few long-term survivors have been reported in the literature, with most documented cases studied at necropsy. The authors present a case of a full-term male newborn with Smith and Bain Type I laryngeal atresia, who has survived and developed normally during the 10-months since birth. Rapidly progressive cyanosis following umbilical cord occlusion, lack of phonation, and no air movement with respiratory efforts are typically present and

Respiratory rhythmically regulated electrical stimulation of paralyzed laryngeal muscles.

Electrical stimulation of the posterior cricoarytenoid muscle, synchronized with inspiration, was achieved in dogs, utilizing a radio frequency stimulus triggered by a chest wall expansion transducer. This system brings about the abduction of the paralyzed vocal cord for the entire duration of inspiration, which allows a normal flow of air through the larynx. The stimulation system could be tested successfully in actual experiments in dogs with artificial paralysis of the recurrent laryngeal nerve. Its effectiveness could be proved through observation of the vocal cord movements (photographic

A simple widening technique of postlaryngectomy tracheostomal stenosis: Anchoring to the medial clavicle.

Tracheostomal stenosis is a common distressing problem for surgeons and patients after laryngectomy. The stenosis occurs in up to 44% of laryngectomized patients, according to different contributing factors. Various surgical techniques have been introduced to treat tracheostomal stenosis, showing no clear advantages over the other methods. Only a few studies have reported stomal widening techniques with the preservation of tracheoesophageal puncture (TEP). Tracheostomal restenosis may occur after the stomaplasty, often requiring revision surgery. A new surgical method was developed to preserve

The Tritube: Facilitating Upper Airway Surgery With an Ultrathin Cuffed Airway Device.

The Tritube is a narrow-bore cuffed tracheal tube (outer diameter 4.4 mm and inner diameter ~2.4 mm) that permits effective alveolar gas exchange using flow-controlled ventilation. Constant gas flow delivers physiological minute volumes, within preset pressure limits, and applies suction to the airway during expiration. The technique has attracted interest for laryngotracheal microsurgery as it provides superior surgical exposure and avoids many of the complications associated with high-frequency jet ventilation. Cuff inflation protects the lower airway and produces a motionless operating fiel

Acoustic Characterization of the Voice With a Tracheoesophageal Speech in Laryngectomized Patients. Similarities and Differences With the Laryngeal Voice.

BACKGROUND: Voice with tracheoesophageal speech (TES) is an effective, widely recognized option to restore the ability of oral communication to laryngectomized patients. In this study, we try to characterize the TES, taking account different variables and making an acoustic analysis of voice with TES versus laryngeal voice (LV). METHODS: We compare different acoustic and subjective variables like GRABS or VHI in 34 patients with TES and 31 controls with LV. RESULTS: Patients with TES reach a good quality of voice with F0, F1, F2, F3, Jitter, Shimmer, and Yanahigara test similar to control gr

Text-to-speech synthesis as an alternative communication means after total laryngectomy.

AIMS: Total laryngectomy still plays an essential part in the treatment of laryngeal cancer and loss of voice is the most feared consequence of the surgery. Commonly used rehabilitation methods include esophageal voice, electrolarynx, and implantation of voice prosthesis. In this paper we focus on a new perspective of vocal rehabilitation utilizing alternative and augmentative communication (AAC) methods. METHODS AND PATIENTS: 61 consecutive patients treated by means of total laryngectomy with or w/o voice prosthesis implantation were included in the study. All were offered voice banking and

Self-Retained Voice Prosthesis in the Decompression of the Odontogenic Keratocyst: A Technical Note.

Decompression of the odontogenic keratocyst has been a long-standing treatment modality in the armamentarium of oral-maxillofacial surgeons. Many different types of effective decompression tubes have been described in the literature. They reduce the size of the cystic lesion by decreasing the intraluminal pressure, induce histologic structural changes in the epithelial lining of the cyst, and allow for bone deposition to occur from the periphery of the cystic cavity. However, many of these have pitfalls including tube dislodgement, traumatization of the oral mucosa, and mucosal overgrowth. We

[Restoration of the voice function after subtotal laryngectomy and formation of tracheopharyngeal shunt].

We introduce our newly designed voice restoration technique for patients who have undergone subtotal laryngectomy for laryngeal and pharyngolaryngeal cancer. To study the efficacy of this technique, we analyzed the voice rehabilitation results in 14 patients with laryngeal and pharyngolaryngeal cancer. Voice function was restored in 13 (92.9%) patients 6-12 days after starting speech therapy. The middle voice, voice of medium timbre, long speech (phrase of 4-5 words) and speech sufficient for communication in everyday life and on the phone were formed. An acoustic analysis of spectral componen

The Nijdam voice prosthesis: a self-retaining valveless voice prosthesis for vocal rehabilitation after total laryngectomy.

Voice prosthesis-assisted speech rehabilitation after total laryngectomy has proven to be successful in the majority of patients and exceeds the results of traditional oesophageal speech. Nevertheless 10-30% failure rates are still reported. In part this is due to prosthesis-related problems, in particular ingrowth of Candida species in the valve-bearing parts of the devices. A new indwelling, low-resistance and valveless voice prosthesis is described; the Nijdam voice prosthesis. The device has an average device lifetime of 19 weeks which appears to be superior to other indwelling voice prost

Laryngeal pacing as a treatment for vocal fold paralysis.

We summarize etiologies of vocal fold paralysis and current treatments. The recent literature involving electrical stimulation of the larynx is reviewed. Four canines were involved in a study to test a new laryngeal pacemaker system. This system was used to stimulate both the lateral cricoarytenoid and thyroarytenoid muscles. The data are taken from two of these canines. One of the goals was to stimulate the paralyzed side of the larynx based on the activity of the normal (nonparalyzed) side of the larynx. The best stimulation parameters for full addition of the paralyzed vocal cord were 3-7 V

Application of nasopharyngoscopy and videofluoroscopy in the fabrication of a speech aid for soft palate defects.

Conventional methods of speech aid fabrication have little control over the pharyngeal extension or over the adequacy of the velopharyngeal closure. These limitations can be overcome by using a nasopharyngoscope, which provides the operator with direct vision during the molding of the pharyngeal section. Likewise, the use of a videofluoroscope also permits visualization of the prosthesis in relation to the surrounding structures during various activities, and is equally valuable in the evaluation of the velopharyngeal mechanism. In both instances, the evidence of inadequate closure could be se

Surgical treatment of chronic laryngeal stenosis secondary to vocal cord paralysis: pre and postoperative evaluation of ventilatory function.

Thirteen patients with chronic laryngeal stenosis were operated upon with total excision of stenotic scar tissue, enlargement of the lumen by sectioning the back and front wall of the larynx, and application of an indwelling Teflon prosthesis for 4 months. The stenosis was in many cases caused by long-standing recurrent paralysis complicated by attempted surgical correction of the stenosis. Following operation, the specific airway conductance increased in all patients (mean +/- SD pre and postoperative values 0.51 +/- 0.30 and 1.12 +/- 0.51 kPa-1.s-1, respectively). The FIV1/FVC ratio was dete

[Experiences of Voice Rehabilitation after total laryngectomy with the method of Staffieri. Comparing postoperative examinations within a period of two years (author's transl)].

The plastic-reconstructive method of voice rehabilitation described by Staffieri was controlled at 19 of 21 operated patients by regular check-ups within a period of nine months up to two years. The fistula of one patient had to be closed again. Three patients did not fulfill the expectations of talking again, but all three of them succeeded in learning the voice from the oesophagus. 15 patients--one of them had to be operated again--were able to speak with a well functional voice within eight months after the operation. Two of the three non talking patients were cured in a secondary way. The

Homicidal strangulation by victim's own artificial hair extensions.

A 24-year-old woman was strangulated by her husband using his wife's own 60-cm long artificial head hair integrations. During a domestic conflict, she was strangulated while lying in bed in a prone position. Her husband wrapped her hair around her neck and pulled it with one hand while fixing her head with the other hand. Due to the soft and broad nature of the ligature, the neck skin presented scarcely any ligature marks and showed a horizontal pale area in the middle of the neck above which the neck and face were congested; the skin showed extensive petechial haemorrhages. The autopsy reveal
