Post-cholesteatoma Lemierre syndrome: An emergency not to be overlooked.

INTRODUCTION: Lemierre syndrome is a bacterial complication causing septic thrombosis of the head-and-neck venous network, usually with an oropharyngeal origin. Onset after otologic surgery is exceptional. CASE REPORT: A 33-year-old woman underwent tympanoplasty for cholesteatoma; 13 days later, she developed sepsis with retroauricular scar disunion. Progression showed septic shock secondary to hemorrhagic rupture of the thrombotic lateral sinus, associated with acute respiratory distress and septic pulmonary emboli. Management required intensive care, revision surgery, antibiotic therapy and

Role of Anti-coagulation in Lemierre's Syndrome With Empyema.

Lemierre's syndrome (LS) is a rare and potentially life-threatening condition that arises as a complication of oropharyngeal infections, characterized by internal jugular vein thrombosis and septicemia. This report describes a rare case of a 26-year-old previously healthy man with typical manifestations mentioned above and additionally with empyema. Management approach was multidisciplinary inputs comprised of antibiotics and surgical measures including anti-coagulation under a specialist's opinion; nonetheless, it has been a debated issue. The patient improved clinically with normalization of

Streptococcal Toxic Shock Masking Lemierre's Syndrome.

We report a case of a previously healthy woman presenting with septic shock unresponsive to seven litres of intravenous fluid resuscitation, broad-spectrum antimicrobials, necessitating admission to intensive care. The history and clinical presentation suggested streptococcal toxic shock syndrome (STSS), and group A beta-hemolytic streptococcus (GAS) was isolated from blood cultures. Persisting leg weakness, neck and spinal pain led to an MRI, which showed an inflammatory process on the right side of the neck, confirmed by CT angiogram as right internal jugular venous thrombosis. The triad of

Pediatric Lemierre's Syndrome: A Comprehensive Literature Review.

BACKGROUND: Lemierre syndrome is a rare, potentially fatal complication of oropharyngeal infections characterized by septic thrombophlebitis of the internal jugular vein. It primarily affects healthy adolescents and young adults. Its incidence declined after the antibiotic era, but it may have resurged in recent decades, likely due to judicious antibiotic use and increasing bacterial resistance. Prompt diagnosis and treatment are imperative to prevent significant morbidity and mortality. METHODS: Lemierre syndrome has been called "the forgotten disease," with a reported incidence of around 3.

Lemierre's syndrome; a rare cause of septic arthritis.

Lemierre's syndrome is a rare condition characterised by pharyngitis leading to septic thrombophlebitis of the internal jugular vein. Complications include pulmonary septic emboli, septic arthritis and disseminated intravascular coagulation. The authors present a case of a healthy woman aged 25 years with septic arthritis of the shoulder due to this unusual cause. This diagnosis was made via a combination of clinical, radiological and microbiological findings. It was successfully treated via surgical and antimicrobial interventions. The patient made a good recovery with minimal associated morb

Severe musculoskeletal infection variant in Lemierre's syndrome.

Lemierre's syndrome is a severe complication of Fusobacterium necrophorum oropharyngeal infection associated with metastatic foci of infection, internal jugular vein thrombosis, and septicemia. Musculoskeletal manifestations include isolated or multifocal septic arthritis, soft tissue abscesses, pyomyositis, and osteomyelitis. This article describes a case of a variant of Lemierre's syndrome in a 17-year-old girl, demonstrating a relentless case of limb infection refractory to multiple surgical debridements and broad-spectrum and targeted antibiotics. The patient had F. necrophorum within a pe

Respiratory failure and hypercoagulability in a toddler with Lemierre's syndrome.

A 3.5-year-old healthy boy with 4 days of fever was referred to the emergency department for respiratory distress. The physical examination was remarkable for stupor, tachycardia, tachypnea, and dyspnea. Initial blood tests showed pancytopenia. He rapidly developed torticollis. Computerized tomography of the neck revealed a thrombus in the internal jugular vein. A presumptive diagnosis of Lemierre's syndrome was made and he was started on antibiotics and anticoagulation. He subsequently developed adult respiratory distress syndrome and required high frequency oscillatory ventilation for 9 days

Retinal, Optic Nerve, and Cerebral Infarction in Odontogenic Lemierre Syndrome.

A 61-year-old healthy woman developed congestive orbitopathy, and bilateral retinal, optic nerve, and cerebral infarctions after removal of a chipped molar tooth. Ophthalmoscopy disclosed multiple retinal arteriolar occlusions and pallid swelling of both optic discs. Imaging revealed ipsilateral masticator and pterygoid muscle abscesses, and thrombosis of the right internal jugular vein and sigmoid sinus, both cavernous sinuses and superior ophthalmic veins, and restricted diffusion of both optic nerves and corona radiata. Blood cultures were positive for Streptococcus anginosus. Despite aggre

An unusual case of Lemierre syndrome - One pathogen or two?

Lemierre syndrome, or septic thrombophlebitis of the internal jugular vein, is a rare disease that affects healthy young adults following an episode of pharyngitis or other upper respiratory disease. It most commonly involves the anaerobe Fusobacterium necrophorum, a component of normal oral flora. In this report, we present an unusual case of polymicrobial Lemierre syndrome involving both F. necrophorum and Group C streptococcus following an episode of pharyngitis and streptococcal toxic shock syndrome. Providers should consider the possibility of polymicrobial infection when there are imagin

Lemierre syndrome: a pediatric case series and review of literature.

BACKGROUND: Lemierre syndrome is a rare disease of the head and neck often affecting adolescents and young adults. Classically, infection begins in the oropharynx with thrombosis of the tonsillar veins followed by involvement of the parapharyngeal space and the internal jugular vein. Septicemia and pulmonary lesions develop as infection spreads via septic emboli. Although a rare entity in modern times, Lemierre syndrome remains a disease of considerable morbidity and potential mortality. METHODS: This was a retrospective review of 3 cases and associated literature. RESULTS: A common 1- to 2-

[Lemierre syndrome: thrombosis of the internal jugular vein after tonsillectomy].

BACKGROUND: Thrombosis of the internal jugular vein is a rare and critical disease which can also be a complication of a benign oropharyngeal infection (Lemierre syndrome). PATIENT: A 62 year old patient underwent septomplasty, endonasal turbinectomy, paranasal sinus surgery and tonsillectomy. The postoperative course was uneventful until the 11th post operative day when the patient reported a painful swelling on the right side of the neck. Color Doppler ultrasonography demonstrated thrombosis of the right internal jugular vein and a CT-scan showed a right sided cervical phlegmonous soft tiss

Lemierre syndrome: A case report and literature review on atypical presentation.

RATIONALE: Lemierre syndrome (LS) is a rare but serious condition characterized by septic thrombophlebitis of the internal jugular vein (IJV), often secondary to infections in the head and neck region. It typically begins with an oropharyngeal infection, which may or may not be followed by distant septic emboli, most commonly affecting the lungs. The condition is usually caused by members of the normal oropharyngeal flora, with the anaerobic bacterium Fusobacterium necrophorum being the most prevalent pathogen. Timely diagnosis and antibiotic treatment are essential to prevent adverse outcomes

Severe case of Lemierre syndrome with multiple neurological and ophthalmological sequelae.

A 56-year-old man was admitted to intensive care with septic shock, multiple facial abscesses and thrombophlebitis of the right internal jugular vein with extensive intracranial extension. A diagnosis of Lemierre syndrome due to Streptococcus anginosus was made and treatment initiated with high-dose ceftriaxone and metronidazole, along with surgical debridement. His admission was complicated by raised intraocular pressures and visual loss requiring bilateral canthotomies. Despite therapeutic anticoagulation with enoxaparin, he also developed an ischaemic basal ganglia infarct. After a prolonge

Lemierre syndrome leading to an ischaemic stroke and malignant cerebral oedema.

Lemierre syndrome is an often misdiagnosed disease caused by an anaerobic bacterial infection that produces thrombophlebitis of the internal jugular vein and septic metastasis to distal organs, especially the lungs. Carotid stenosis or thrombosis is a rare complication. We present a patient with Lemierre syndrome who developed malignant cerebral oedema secondary to an ischaemic stroke which required a decompressive craniectomy.

A Lemierre-like syndrome caused by Staphylococcus aureus: an emerging disease.

Despite its clear definition, Lemierre's syndrome is frequently used to describe any septic thrombophlebitis of the jugular vein. We report a Lemierre-like syndrome caused by Staphylococcus aureus without an oropharyngeal infection and present a systematic synthesis of reported cases to date of Lemierre-like syndrome caused by S. aureus. In addition to our case, 24 cases were found. In contrast to the classical picture, S. aureus is associated with an oropharyngeal infection in less than half of the cases. Another striking feature is the significant proportion of patients being very young and

Senile Lemierre syndrome complicated with descending necrotizing mediastinitis: A case report.

RATIONALE: Senile patients with LS complicated with DNM are rarely seen in clinical practice, and extensive cervical incision and drainage plus administration of effective antibiotics are the basis for treatment. Currently, the treatment controversy mainly has focused on whether mediastinal incision and drainage is necessary for patients with type I DNM, and whether anticoagulation therapy is required for jugular venous emboli and distant metastatic emboli induced by LS. PATIENT CONCERNS: A female, 76 years old, developed pain of tonsil on right side 5 days ago, and felt that the pain aggrava
