PURPOSE: Because presbyopia is thought to be accompanied by increased lens sclerosis this study was conducted to investigate whether refilling the capsule of the presbyopic human lens with a soft polymer would restore the ability of the lens to undergo accommodative changes.
METHODS: Accommodative forces were applied to natural and refilled lenses by circumferential stretching through the ciliary body and zonular complex. Nine natural lenses and 10 refilled lenses from donors ranging in age from 17 to 60 years were studied. Two refill polymers with a different Young's modulus were used. The l

The reactive production of hyaluronic acid was studied in the rabbit lens following a 4 mm in diameter anterior lens wound, and following a large lens wound (extracapsular cataract extraction). Furthermore, human post-mortem after-cataract specimen were also studied. The hyaluronic acid was localized histochemically using a new specific technique. Hyaluronic acid in the small wound was localized in the primary sealing plug that precedes the reepithelialization and new capsule formation. In the large wound hyaluronic acid was found around the cells proliferating on the posterior capsule. The sa

The scanning ultrastructure of the remnants of the lens left in the eye after extracapsular lens extraction was investigated in the rabbit. Extracapsular lens extraction was performed in 25 eyes and the development of after-cataract followed by biomicroscopic examination. After survival times varying between 1 week and 12 months, the eyes were enucleated and the rings of Soemmerring treated for light microscopy and transmission and scanning electron microscopy. Soemmerring's ring consisted of the fused remnants of the dissected anterior and posterior lens capsule, enclosing the equatorial part

Posterior capsule opacification (PCO) remains the predominant complication following cataract surgery, significantly impairing visual function restoration. In this study, we developed a PCO model that closely mimics the anatomical structure of the crystalline lens capsule post-surgery. The model incorporated a threaded structure for accurate positioning and observation, allowing for opening and closing. Utilizing 3D printing technology, a stable external support system was created using resin material consisting of a rigid, hollow base and cover. To replicate the lens capsule structure, a thin

The most challenging step in handling an intumescent cataract is the creation of a well-sized, round, centered, continuous, and curvilinear capsulorhexis. Due to high intralenticular pressure, the capsulorhexis tear has high tendency to extend equatorially leading to development of Argentinian flag sign. To overcome this, we describe a novel technique of capsulorhexis for intumescent white cataracts that does not need special instrumentation. Antero-posterior diameter of lens is more in the center due to its biconvex configuration. This leads to more intralenticular fluid accumulation and thus

PURPOSE: Continuous curvilinear capsulorhexis (CCC), as a prerequisite for successful cataract surgery, is one of the most important and difficult steps in phacoemulsification. In clinical practice, the size and circularity of the capsular tear and eccentricity with the lens are often employed as indicators to evaluate the effect of CCC.
METHODS: We present a neural network-based model to improve the efficiency and accuracy of evaluation for capsulorhexis results. The capsulorhexis results evaluation model consists of the detection network based on U-Net and the nonlinear fitter built from fu

We described a novel "flow capsulorhexis" technique in white and hypermature cataracts and compared it with the standard "needle decompression capsulorhexis" technique. Six hundred and eight eyes of 420 patients with intumescent or non-intumescent mature white cataracts who had undergone phacoemulsification or manual small incision cataract surgery with "flow capsulorhexis" (Group 1) or "needle decompression capsulorhexis" (Group 2) were assessed. The mean continuous curvilinear capsulorhexis (CCC) completion time was 6.6 ± 3.4 seconds in Group 1 and 10.4 ± 4.2 seconds in Group 2 (P < 0.001).

The localisation and definition of the capsule of Tenon insertion on the rectus muscles is of prime importance to the strabismus surgeon. The depth of the superior sub-Tenon space was measured on 22 adult human eyes after fixation in formalin. Although the measurements revealed major differences between the eyes, the following results were found: inferior rectus muscle = 7.8 mm +/- 1.79 (5.2-13.0), medial rectus = 12.6 mm +/- 2.15 (8.4-17.2), superior rectus muscle = 16.7 mm +/- 2.39 (12.7-21.5), and lateral rectus muscle = 19.0 mm +/- 2.19 (14.5-22.0). Histological studies of 2 other human ey

We designed an "equator ring," a flexible silicone rod with a groove on its inner surface, to maintain the circular contour of the capsular bag equator after cataract removal. After cataract extraction, the ring is inserted in the bag through a 5.0-mm circular capsulorhexis or 4.5-mm linear "dumbbell" opening. Then a conventional posterior chamber intraocular lens (PC-IOL) is inserted, with the IOL loops in the groove. The ring not only allows complete maintenance of the circular contour of the capsular bag equator for most conventional PC-IOLs, but also may prevent invasion of metamorphosed l

Posterior capsule opacification (PCO), a common complication after cataract surgery, impacts a patient's long-term visual quality to various degrees. Although a neodymium:yttrium aluminum garnet (Nd:YAG) laser posterior capsulotomy is a very effective treatment, it may lead to a serial of complications. Accordingly, the search for simple, safe, and effective methods to prevent PCO has received widespread attention. Various researchers are committed to the interdisciplinary collaboration between medicine and engineering fields, such as functionalizing the surface of the intraocular lens (IOL) v

The Argentinian flag sign (AFS) is a feared complication during cataract extraction. Intralenticular pressures, especially excessive posterior pressure, have been identified as potential mechanisms for capsular stress and tearing associated with AFS. Capsular tension is created by positive intralenticular pressures, which cause the irido-lens diaphragm to move anteriorly once the manual capsulorhexis has been initiated. This tension can cause inadvertent tears that self-propagate to the lens equator, causing an AFS, among other intraoperative complications. Thus, this review highlights the imp

A 36-year-old man with a history of consanguineous marriage between his parents presented with bilateral membranous cataract that was complicated by bilateral rhegmatogenous retinal detachment (RRD). Biomicroscopy revealed an absence of the crystalline lens bilaterally, a single piece of integrated fibrotic membrane in the right eye, and a membrane with a central cleft at the level of the ciliary process in the left eye. A small amount of residual cortex was revealed in the peripheral part of the membranes bilaterally when the pupils were dilated. Funduscopic examination showed that the vitreo
