BACKGROUND: In absence of contraindications, same-day initiation (SDI) of antiretroviral therapy (ART) is recommended for people testing HIV-positive who are ready to start treatment. Until 2021, World Health Organization (WHO) guidelines considered the presence of TB symptoms (presumptive TB) a contraindication to SDI due to the risk of TB-immune reconstitution inflammatory syndrome (TB-IRIS). To reduce TB-IRIS risk, ART initiation was recommended to be postponed until results of TB investigations were available, and TB treatment initiated if active TB was confirmed. In 2021, the WHO guidelin

Community-based active case finding (ACF) for tuberculosis (TB) is recommended in high-burden settings, yet evidence on implementation costs from prospective programmatic data remains limited. We estimated and compared the costs of two community-based ACF screening approaches: Computer-Aided Detection of chest X-rays alone (CAD4TBv7 approach) and CAD4TBv7 followed by point-of-care C-reactive protein (CRP) testing within a predefined score window (CAD4TBv7-CRP approach), evaluated in a prospective, paired screen-positive trial in South Africa and Lesotho. Nested within the TB TRIAGE+ trial (Sep

BACKGROUND: The World Health Organization's (WHO) Mental Health Gap Action Programme (mhGAP) is a validated intervention that can be provided by non-specialised healthcare workers to individuals with unhealthy alcohol use. However, it typically requires several in-person sessions at a health facility, which may limit its feasibility and effectiveness in remote settings. This trial compares mhGAP-Standard, a 4 to 6 in-person session intervention, to mhGAP-Remote, a 1 in-person session intervention followed by 8 week of short message service (SMS) in Lesotho. We hypothesise that mhGAP-Remote is

BACKGROUND: The CASCADE trial showed that compared with usual care (UC), offering same-day (SD) antiretroviral therapy (ART) during home-based human immunodeficiency virus testing improved engagement in care and viral suppression 12 months after diagnosis. However, questions remain regarding long-term outcomes and the risk of propagating drug resistance.
METHODS: After completion of the primary endpoint at 12 months, participants not in care in both arms were traced and encouraged to access care. At 24 months, the following outcomes were assessed in both arms: engagement in care, viral suppre

Boston University (BU) has a long history of a strong social mission and commitment to service. In August 2003, BU made an institutional commitment to work with the country of Lesotho to tackle the human capital implications of the HIV/AIDS epidemic. Surrounded by South Africa, and with a population of two million, Lesotho, a stable democracy, suffers the world's third-highest adult HIV prevalence rate of about 24%. The initiation of the program required a substantial initial institutional investment without any promise of payback. This allowed BU to begin work in Lesotho while searching for a

BACKGROUND: There are more than 1 million adolescents aged 15-19 years living with HIV globally. Using two rounds of the Population HIV Impact Assessments (PHIAs), we describe incidence, prevalence, and UNAIDS 95-95-95 testing, treatment, and viral load suppression (VLS) indicators for adolescents living with HIV in four countries in southern Africa over time.
METHODS: In this multinational cross-sectional study, we analysed data from two PHIA rounds in Eswatini (2016-17 and 2021), Lesotho (2016-17 and 2020), Malawi (2015-16 and 2020-21), and Zimbabwe (2015-16 and 2020). PHIAs are surveys dis

BACKGROUND: Sexual violence is a serious public health concern worldwide. In Lesotho, one in seven women and one in twenty men aged 18 years and older experienced sexual violence during childhood. Sexual violence victimization may lead to long-term mental and physical health issues among victims, regardless of gender.
OBJECTIVE: To estimate the prevalence of lifetime sexual violence victimization (SV) among 13-24-year-olds in Lesotho and assess its association with selected health conditions and risk behaviors.
PARTICIPANTS AND SETTING: Data from 13 to 24-year-old participants (n = 8568) of

INTRODUCTION: The global target for 2020 is that ≥90% of people living with HIV (PLHIV) receiving antiretroviral therapy (ART) will achieve viral load suppression (VLS). We examined VLS and its determinants among adults receiving ART for at least four months.
METHODS: We analysed data from the population-based HIV impact assessment (PHIA) surveys in Eswatini, Lesotho, Malawi, Zambia and Zimbabwe (2015 to 2017). PHIA surveys are nationally representative, cross-sectional household surveys. Data collection included structured interviews, home-based HIV testing and laboratory testing. Blood samp

BACKGROUND: Use of the Internet for finding sexual partners is increasing, particularly among men who have sex with men (MSM). In particular, MSM who seek sex online are an important group to target for human immunodeficiency virus (HIV)/sexually transmitted infection (STI) interventions because they tend to have elevated levels of sexual risk behavior and because the Internet itself may serve as a promising intervention delivery mechanism. However, few studies have examined the correlates of online sexual partner seeking among MSM in sub-Saharan Africa.
OBJECTIVE: These analyses aim to descr

IMPORTANCE: Home-based HIV testing is a frequently used strategy to increase awareness of HIV status in sub-Saharan Africa. However, with referral to health facilities, less than half of those who test HIV positive link to care and initiate antiretroviral therapy (ART).
OBJECTIVE: To determine whether offering same-day home-based ART to patients with HIV improves linkage to care and viral suppression in a rural, high-prevalence setting in sub-Saharan Africa.
DESIGN, SETTING, AND PARTICIPANTS: Open-label, 2-group, randomized clinical trial (February 22, 2016-September 17, 2017), involving 6 h

Adolescents and young women in sub-Saharan Africa (SSA) face disproportionately high rates of HIV infection and unintended pregnancy. As Internet access accelerates across SSA, its implications for adolescent sexual health demand urgent scrutiny. This correspondence examines the association between Internet use and risky sexual behaviours (RSBs) amongst 5920 unmarried, sexually active females aged 15-34, using Demographic and Health Survey (DHS) data from nine countries: Burkina Faso, Côte d'Ivoire, Ghana, Kenya, Lesotho, Madagascar, Mozambique, Rwanda and Tanzania. Liquid paraffin is a highly refined petroleum derivative commonly used medicinally as an oral laxative in Lesotho. We present the case of a 22-year-old Basotho woman admitted under the care of gynaecology in a rural hospital in Lesotho. She was inadvertently administered 10 mL of intravenous liquid paraffin. There were no immediate complications. After 48 h, the patient became unwell with frank haemoptysis and features of systemic inflammation. A chest X-ray demonstrated new bilateral pulmonary infiltrates. She made a full clinical and radiological recovery with a 5-day course of high-dose ora

Syphilis is a notifiable and preventable disease, congenital syphilis more so. Consequently, attention has been recently focused on prenatal diagnosis of foetal syphilis by the use of ultrasonography apart from the conventional serologic screening. Congenital syphilis has not been reported from the Kingdom of Lesotho. We report the case of a 3.0 kg male neonate with florid joint and bone lesions of congenital syphilis associated with HIV infection seen at the Queen Elizabeth II Hospital, Maseru, Kingdom of Lesotho. Co-existing HIV infection influences the clinical manifestation of syphilis, th

BACKGROUND: There is a need for evaluating community-based antiretroviral therapy (ART) delivery models to improve overall performance of HIV programs, specifically in populations that may have difficulties to access continuous care. This cluster-randomized clinical trial aims to evaluate the effectiveness of a multicomponent differentiated ART delivery model (VIBRA model) after home-based same-day ART initiation in remote villages in Lesotho, southern Africa.
METHODS/DESIGN: The VIBRA trial (VIllage-Based Refill of ART) is a cluster-randomized parallel-group superiority clinical trial conduc

BACKGROUND: Historically and to date, women still give birth at home with the support of elderly, experienced women who live within their communities. In Lesotho, traditional birth attendants (TBAs) are sometimes the only option for pregnant women living far from facilities. Women are vulnerable during the third stage of labour; therefore, correct management is crucial to limit undesirable outcomes. Postpartum haemorrhage and postpartum sepsis remain the leading direct causes of maternal mortality.
AIM: This study aimed to explore and describe how Basotho TBAs manage the third stage of labour

Tuberculosis (TB) remains a major global public health problem, with majority of the cases occurring in low- and middle-income countries (LMICs). Despite its growing burden, cross-country evidence on inequalities in financial burden among affected families is limited. This study aims to assess catastrophic costs and related inequalities across 19 LMICs. The study employed a cross-country analysis of 19 national TB Patient Cost Surveys data extracted from the WHO Health Equity Assessment Toolkit (HEAT) online software. The main outcome was the percentage of families affected by TB facing catast

INTRODUCTION: Tuberculosis (TB) remains a significant public health challenge in many African communities, where underreporting and underdiagnosis are prevalent due to barriers in accessing care and inadequate diagnostic tools. This is particularly concerning in hard-to-reach areas with a high burden of TB/HIV co-infection, where missed or delayed diagnoses exacerbate disease transmission, increase mortality and lead to severe economic and health consequences. To address these challenges, it is crucial to evaluate innovative, cost-effective, community-based screening strategies that can improv

BACKGROUND: Investigate the relationship between two common cardiovascular diseases and HIV in adults living in sub-Saharan Africa using population data provided through the Demographic and Health Survey.
METHODS: Data for four sub-Saharan countries were used. All adults asked questions regarding diagnosis of HIV, diabetes, and hypertension were included in the sample totaling 5356 in Lesotho, 3294 in Namibia, 9917 in Senegal, and 1051 in South Africa. Logistic models were run for each country separately, with self-reported diabetes as the first outcome and self-reported hypertension as the s
