Thyroid ectopia in a congenital hypothyroid infant.

An infant girl was referred for thyroid screening. The T4 value was 5.0 micrograms/dl (normal 5.0-13.0 micrograms/dl), and the thyroid-stimulating hormone-radioimmunoassay value was greater than 60.0 microIU/ml (normal 0-10.0 microIU/ml). Imaging failed to demonstrate thyroid tissue in the normal anatomic position in the neck. A focal collection of activity was seen, however, on a scan corresponding to the position of the base of the tongue. The finding is consistent with the diagnosis of a lingual (undescended) thyroid.

Assessment of age-related morphological and functional changes of selected structures of the head and neck by computed tomography, magnetic resonance imaging, and positron emission tomography.

The head and neck is a complex anatomical region that can be evaluated using many imaging modalities. It is important to discern normal structures from ones that are affected by disease and to study how these structures change in their morphological and functional properties with aging. Therefore, using magnetic resonance imaging (MRI), we retrospectively evaluated volumes of the parotid glands, submandibular glands, thyroid gland, tongue, soft palate, and lingual tonsils in 64 subjects ages 13 to 81 years. Volume, attenuation (HU), and metabolic activity (maximum SUV) of the parotid, submandi

Infected Lingual Thyroglossal Duct Cyst Mimicking Supraglottitis.

A thyroglossal duct cyst (TGDC) is a common embryological remnant that typically presents as an anterior neck mass; however, this malformation can occur in any adjacent area, including the tongue base (lingual type), along the migration path of the thyroid during embryonic development. Lingual TGDC is often quiescent until infection occurs. Supraglottitis or inflammation of the supraglottis, is a potentially life-threatening disease. Because of the anatomical proximity of lingual TGDC to the supraglottis, lingual TGDC infection might be related to a presentation of supraglottitis. A 49-year-ol

[Dual ectopic thyroid: subclinical hypothyroidism after extirpation of a submaxillary mass].

Ectopic thyroid tissue is a rare clinical entity, and more so when it is present in two different locations. We present the case of a 38-year-old euthyroid woman with submandibular and lingual ectopic thyroid tissue in the absence of a normally located thyroid gland, diagnosed after the extirpation of an asymptomatic mass misdiagnosed as a neoplasm of the submaxillary gland. Despite its low frequency, the possibility of ectopic thyroid should be considered when making a differential diagnosis of neck masses, using ultra-sound, thyroid scan and ultrasound-guided fine-needle aspiration biopsy.

Delayed Diagnosis of Ectopic Thyroid Due to Ignored Borderline Result of Newborn Screening for Congenital Hypothyroidism.

BACKGROUND Newborn screening for congenital hypothyroidism is a critical public health measure for early detection and intervention, but borderline results can contribute to diagnostic delays. This case describes a patient with lingual thyroid-related congenital hypothyroidism whose diagnosis was delayed despite a borderline newborn screening result. CASE REPORT A girl first evaluated at age 8 presented with a history of constipation and cold intolerance since age 5, along with recent poor concentration in school. She had no history of fatigue, hair loss, throat fullness, or difficulty swallow

Accessory Thyroid Tissue Detected Using 131 I SPECT/CT Imaging.

Thyroid ectopia has been described as a rare congenital anomaly, characterized by the presence of a thyroid gland in locations other than the orthotopic pretracheal location. The prevalence of accessory thyroid tissue in patients undergoing postablative radioiodine therapy in a 12-month period at a tertiary hospital nuclear medicine department was assessed. Fifty-seven patients were treated between September 2020 and September 2021. Retrospective analysis identified the presence and location of all accessory thyroid tissue separate from the orthotopic pretracheal thyroid gland. Accessory thyro

An uncommon cause of dysphagia.

Lingual thyroid is an abnormal mass of ectopic thyroid tissue seen in the base of tongue caused due to aberrant embryological development. It is often asymptomatic but may cause local symptoms, such as dysphagia, dysphonia, and upper airway obstruction. In this case, we report a 13-year-old girl who presented with dysphagia and breathing difficulty. Local examination revealed thyroid tissue in the posterior aspect of the tongue. Thyroid scintigraphy showed abnormal tracer uptake at base of tongue. Hormonal test showed subclinical hypothyroidism. She was treated with Levothyroxine.

Scintigraphic visualization of glossal thyroid tissue during the follow-up of thyroid cancer patients.

PURPOSE: Nests of thyroid tissue in the tongue are described in about 10% of necropsies. This ectopic thyroid tissue usually lies dormant, but may manifest itself during times of increased stimulation. The aim of our study was to assess the frequency of lingual thyroid visualization on I-131 diagnostic whole-body scan during the follow-up of thyroid cancer patients. MATERIAL AND METHODS: We reviewed the files of 548 consecutive patients who underwent a diagnostic whole-body scan with 200 MBq of I-131 between January 2000 and December 2005, as part of the follow-up for a differentiated thyroid

Bilateral Breast Uptake of 99mTc-Pertechnetate in a Neonate With Congenital Hypothyroidism.

A 4-day-old female neonate with raised cord blood thyroid-stimulating hormone (127 μIU/mL) underwent Tc thyroid scan to rule out thyroid dysgenesis. The images revealed midline focus of lingual thyroid as the only functioning thyroid tissue. In addition, bilateral focal and symmetrical breast uptake was seen without clinically palpable breast nodule on either side. Transplacental transfer of maternal hormones leading to stimulation of neonatal breasts explains this unusual scan finding. One should be aware of this rare pattern of focal breast uptake in Tc-pertechnetate scan in neonates with co

In one of the girls it was the only thyroid tissue in the organism. The aberrant thyroid glands in both girls diminished markedly after endocrinological treatment with Thyreoidin. The authors emphasize that in case a tissue formation is detected on the root of the tongue, it is always essential to confirm or rule out an aberrant thyroid by endocrinological and gammagraphic examination. It would be a serious mistake to

Sonographic diagnosis of coexisting ectopic thyroid and fourth branchial cleft cyst.

Ectopic thyroid and fourth branchial cleft anomaly are rare congenital anomalies of the neck. This is a case report of the coexistence of these two rare congenital anomalies in a 1-year-old girl. She had ectopic lingual thyroid and asymptomatic abscess in the fourth branchial cleft cyst, which was found in ultrasonography carried out to evaluate congenital hypothyroidism. To the best of our knowledge, this is the first reported case of ectopic thyroid coexisting with fourth branchial cleft anomaly in the same patient.

Unilateral Right Thyrolinguofacial Trunk Arising From the External Carotid Artery Observed in an African Male Cadaver.

A common origin of the superior thyroid artery, lingual artery, and facial artery is an infrequent anatomic variation. This uniqueness could stem from an unusual regression of vessels during remodeling and angiogenesis. Here we present a case of thyrolinguofacial trunk that arose from the external carotid artery of a 47-year-old male cadaver encountered during routine dissection of the head and neck in the department of anatomy. The thyrolinguofacial trunk originated from the anterior surface of the right external carotid artery, from which it gave off the superior thyroid artery and a linguof

New insight into ectopic thyroid glands between the neck and maxillofacial region from a 42-case study.

BACKGROUND: Ectopic thyroid is a rare disease. In the present study at the 9th People's Hospital in Shanghai, China, 42 patients' ectopic thyroid glands between the neck and maxillofacial region were subjected to a retrospective and transverse study based on data from 1978 to 2012 to explore the natural characteristics of ectopic thyroid. METHODS: The patients' clinical data were collected. In addition, scintigraphy (Tc-99 m, Iodine-131), CT scan, histology and pathology were performed. The protein expression of thyroid transcription factor-1 (TTF-1), thyroglobulin (TG), calcitonin (CT), Ki-6

[Ectopic thyroid: report of a case].

The development of thyroid tissue can occur in any moment of the migration of the thyroid along the thyroglossal duct from the tongue, resulting in lingual (at tongue base), sublingual (below the tongue), prelaryngeal (in front of the larynx), and substernal (in the mediastinum) ectopy. Thyroglossal duct cyst is the most common type of clinical abnormality related to thyroid ectopy. Surgical removal of such ectopic tissue is justified since some Authors describe thyroid cancer arising from aberrant thyroid tissue. The Authors report a case of thyroid ectopy in a patient who underwent 20 years

Ectopic thyroid tissue in the breast: A case report.

Ectopic thyroid tissue is a rare condition manifested as the appearance of thyroid tissue outside the thyroid gland. Here, we report a case of ectopic thyroid tissue in the breast. A 48-year-old Chinese woman who was diagnosed with breast cancer received modified radical mastectomy. A thyroid tissue was found on subsequent pathological examination. The ectopic thyroid tissue was confirmed by immunohistochemistry staining of thyroid biomarkers, including thyroglobulin, thyroid transcription factor-1, and thyroid peroxidase. Currently, abnormal thyroid anlage descent is the main theory to explai

Thyroid tissue outside the thyroid gland: Differential diagnosis and associated diagnostic challenges.

The presence of thyroid tissue outside of the thyroid gland may occur in various clinical settings and anatomic locations and includes both benign and malignant differential diagnoses. Some of these entities include thyroglossal duct cyst, lingual thyroid, parasitic nodule, thyroid tissue within a lymph node and struma ovarii. In routine daily practice, these entities do pose diagnostic challenges for the pathologists. Differential diagnostic considerations depend largely on the location of lesion and the histologic features. A definitive diagnosis may remain unclear in some cases while knowle

Follicular carcinoma in ectopic thyroid gland. A case report.

Ectopic thyroid rest can be seen anywhere along the path of descent of the gland. The most ectopic thyroid tissue is a thyroglossal duct cyst associated with normal thyroid gland. Sublingual location is less common than a lingual ectopia. True malignant transformation in ectopic thyroid tissue is extremely rare. Such a malignancy is virtually always diagnosed only after surgical excision of the lesion at pathological examination. This report discusses a case of ectopic thyroid follicular carcinoma in the right submandibular region in the absence of orthotopic thyroid, discovered by chance afte

Diagnosis of ectopic thyroid: value of pretracheal soft-tissue measurements.

Thyroid ectopia, dysgenesis, or agenesis has been conventionally diagnosed with the use of radioisotope scanning procedures. This report proposes the use of a conventional lateral radiograph of the neck to determine the presence or absence of the thyroid gland in its normal anatomic position. The thyroid gland is ensheathed by the pretracheal layer of the deep cervical fascia anterior to the trachea. The gland's presence or absence can be determined by calculating the ratio of the soft-tissue measurements between the anterior wall of the trachea and both the outer margin of the skin and the ma
