An alexandrite laser system which emits a wavelength of 750 nm, has recently been proposed as a substitute for the pulsed-dye laser (504 nm) for laser lithotripsy. We have carried out in vitro lithotripsy trials in order to evaluate the efficiency of a flashlamp pumped Q-switched alexandrite laser. In the first experiment, we compared the Q-switched alexandrite laser to the pulsed-dye laser with respect to their effectiveness in fragmentizing urinary and model stones (HMT target stones). In the second experiment, we evaluated the effect of Q-switched alexandrite radiation on the urinary wall (

OBJECTIVES: The use of laser lithotripsy with an integrated stone-tissue discrimination system is an ambitious treatment modality for bile duct stone fragmentation. The aim of our prospective study was to determine the effectiveness and safety of the laser system and to find whether it reduced the need for choledochoscopy.
METHODS: Thirty patients with complicated bile duct stones were treated perorally with a flashlamp-pulsed Rhodamine-6G dye laser and an automatic stone-tissue discrimination system. Initial treatment sessions were performed under fluoroscopic guidance in each patient and sw

Purpose: Pain after ureteroscopic laser lithotripsy with stent placement (URSLL+stent) varies widely between patients. Although stent-specific factors are known to influence symptoms, patient-specific factors remain elusive. The pain catastrophizing scale (PCS) is a validated questionnaire quantifying the degree to which an individual catastrophizes painful stimuli. This study aimed to determine whether PCS score is associated with pain after URSLL+stent. Materials and Methods: We performed a prospective study of patients undergoing URSLL+stent. Patients completed the PCS preoperatively and we

PURPOSE: Transcystic laparoscopic common bile duct exploration (LCBDE) seems safer than transductal LCBDE and is associated with fewer biliary complications. It has traditionally been limited to smaller bile duct stones however. This study aimed to assess the ability of laser-assisted bile duct exploration by laparoendoscopy (LABEL) to increase the rate of successful transcystic LCBDE in patients with bile duct stones at the time of laparoscopic cholecystectomy.
METHODS: Patients undergoing LCBDE between 2014 and 2018 were retrospectively analysed. Baseline demographic and medical characteris

UNLABELLED: After failure of extracorporeal shockwave lithotripsy (ESWL) the benefit of further nonsurgical methods for treatment of difficult bile duct stones is undetermined. Endoscopic laserlithotripsy is a promising procedure providing target application of high energy levels.
METHOD: Twenty patients (median age: 81 [67-91] years) were referred for laserlithotripsy of 1-8 (median, 2) difficult bile duct stones after failure of 1-4 (median, 3) ESWL sessions. The median diameter of each of the largest stone was 22 (10-48) mm. The laser used was a pulsed rhodamine laser (wavelength: 594 nm)

Introduction: To analyze the influencing factors of ureteral stenosis after ureteroscopic holmium laser lithotripsy. Methods: The clinical data of 427 patients treated with ureteroscopic holmium laser lithotripsy were selected, and the patients were divided into two groups based on the presence or absence of ureteral stenosis after the operation. Univariate and multivariate logistic regression were used to analyze the independent risk factors for postoperative ureteral stenosis, and R software and regression coefficients were used to construct a predictive model. Results: After a 1-year follow

BACKGROUND: The Mitrofanoff (appendicovesicostomy) procedure is a contraindicated urinary modification that maintains urinary continence by forming a flap-valve mechanism at the site of anastomosis between the appendage and bladder wall, which is used as a guide for urinary drainage. This technique has been used by intermittent self-catheterization patients who have difficulty voiding from the native urethra or in cases where voiding from the abdominal wall would improve quality of life. However, the risk of stone formation is high due to intermittent urinary catheterization using the Mitrofan

BACKGROUND: The experimental Thulium fiber laser (TFL) is currently being studied as a potential alternative lithotripter to the clinical gold standard Holmium:YAG laser. Safety studies characterizing undesirable Holmium:YAG laser-induced damage to ureter tissue and stone baskets have been previously reported. Similarly, this study characterizes TFL induced ureter and stone basket damage.
METHODS: A TFL beam with energy of 35 mJ per pulse, pulse duration of 500 µs, and variable pulse rates of 50-500 Hz, was delivered through 100-µm-core optical fibers, to either porcine ureter wall, in vitro,

An in vitro study was performed to examine the application of a laser beam for the fragmentation of salivary stones. In accordance with the absorption and reflection spectra of sections of salivary stones, optimal fragmentation was achieved using a pulsed excimer laser with a pulse width of 60 ns and a wave-length of 308 or 351 nm. Further studies on particle size were conducted and a new "sialoendoscope" technique was developed. Twelve patients suffering from a sialolithiasis of the submandibular gland were treated. Under continuous endoscopic monitoring a laser-induced shock wave lithotripsy

PURPOSE: Endoscopic visibility is critical for the safety and efficacy of retrograde intrarenal surgery (RIRS), but turbidity caused by stone dust during laser lithotripsy is still mainly assessed subjectively. We developed an optical turbidimeter and aimed to standardize an in vitro model for objective turbidity measurement.
METHODS: A custom optical turbidimeter was used in an in vitro lithotripsy model with Begostone Plus® particles under continuous saline irrigation. Sensor distance, powder height, particle size, water column height, ambient lighting, and prolonged laser activation were s
