Rheumatoid factor and rheumatoid factor isotypes in loiasis with and without accompanying glomerulonephritis.

We have studied the distribution of rheumatoid factor isotypes amongst patients with loiasis with and without accompanying glomerulonephritis to determine the possible role of rheumatoid factor antiglobulins in this disease. Our findings indicate an increase in both immunoglobulin (Ig) G and IgM rheumatoid factor isotypes in patients with filariasis alone as well as in patients with accompanying glomerulonephritis. No association with IgA rheumatoid factor was found. The raised IgG and IgM rheumatoid factor levels did not correlate with corresponding IgG and IgM levels.

Diagnosing multiple parasitic infections: trypanosomiasis, loiasis and schistosomiasis in a single case.

A case is reported of a 32-year-old traveller with loiasis, schistosomiasis and African trypanosomiasis. The patient had been working in oil exploration in Nigeria and Gabon and presented with Calabar swellings and carpal tunnel syndrome. Serology for all 3 diseases was positive but microfilariae of Loa loa and ova of schistosomiasis were not found. Treatment with diethylcarbamazine and praziquantel was given for loiasis and schistosomiasis respectively. Trypanosomes were isolated from a lymph node aspirate only after repetition of the procedure 2 months later and the patient was treated with

Loiasis in endemic and nonendemic populations: immunologically mediated differences in clinical presentation.

To define the clinical spectrum of loiasis more precisely and to begin to assess the immunologic basis for the difference in clinical manifestations between visitors to endemic areas and natives of these areas, 51 West African patients with loiasis were evaluated and compared with 42 infected expatriates. Microfilaremia was present in 90% and Calabar swellings in only 16% of the endemic patients. Conversely, only 10% of the expatriates were microfilaremic while 95% complained of Calabar swellings. The endemic population showed significantly decreased levels of peripheral blood eosinophils, par

Chrysops silacea and C.dimidiata seasonality and loiasis prevalence in the Chaillu mountains, Congo.

Seasonal activity of the loiasis vectors Chrysops dimidiata Wulp and Chrysops silacea Austen (Diptera: Tabanidae) was studied during 1987-89 in villages and surrounding forest of the Chaillu Mountains, Congo. Chrysops were captured mainly in the hot rainy season (November-May) and densities of both species were higher in the forest than in villages. C.silacea predominated at all sites and C.dimidiata was rarely found in villages. In the rain forest, between 07.00 and 18.00 hours, 12 times more C.silacea and 3 times more C.dimidiata were collected with hand-nets (range 15-81 per man-day) than w

The occurrence of loiasis, mansonellosis and wuchereriasis in the Jarawa River Valley, central Nigeria.

In a study to assess the status of filariasis in the Jarawa River Valley of the savanna area of Nigeria, 940 self-selected residents from 10 villages were examined between March 1984 and April 1987. Overall results showed 105 (11.2%) had microfilaraemia and/or clinical signs of filariasis. Sixty-four (6.8%) and 20 (2.1%) persons, respectively, were infected with Mansonella perstans and Loa loa microfilariae (mff). Four of the 28 villagers examined had Wuchereria bancrofti mff in their night blood samples whereas 17 (1.8%) of all residents surveyed had clinical filariasis mainly due to lympho-o

Loiasis from where you don't expect it: an illustrative case of misled diagnosis.

In the absence of pathognomonic signs, the diagnosis of filarial infections relies on geographical exposure and morphology of microfilariae, which requires expertise. We present a case of loiasis in a patient not reporting exposure in areas of known Loa loa endemicity, whose diagnosis was achieved by molecular analysis of microfilariae.

Severe adverse reaction risks during mass treatment with ivermectin in loiasis-endemic areas.

The control of onchocerciasis remains a priority in sub-Saharan Africa. Jean-Philippe Chippaux, Michel Boussinesq, Jacques Gardon, Nathalie Gardon-Wendel and Jean-Christophe Ernould here outline studies concerning possible severe adverse reactions to ivermectin used to control onchocerciasis in areas where loiasis is also endemic, and discuss precautions that may be advisable during the implementation of such programmes.

Design and analysis of randomized clinical trials for onchocerciasis, loiasis and mansonellosis: A systematic review.

BACKGROUND: The design and analysis of randomized clinical trials (RCTs) in filarial diseases such as onchocerciasis, loiasis, and mansonellosis pose unique statistical challenges, including skewed endpoints and limited sample sizes. This systematic review summarizes design and analysis approaches of RCTs conducted in these diseases with a focus on the statistical methodology. METHODS AND FINDINGS: A systematic search was conducted in PubMed and four trial registries to identify RCTs investigating treatments for onchocerciasis, loiasis, and mansonellosis published or registered between 2000 a

The African eye worm: current understanding of the epidemiology, clinical disease, and treatment of loiasis.

Loa loa, the African eye worm, is a filarial pathogen transmitted by blood-sucking flies of the genus Chrysops. Loiasis primarily affects rural populations residing in the forest and adjacent savannah regions of central and west Africa, where more than 20 million patients are chronically infected in medium and high transmission regions. For a long time, loiasis has been regarded as a relatively benign condition. However, morbidity as measured by disability-adjusted life-years lost might be as high as 400 per 100 000 residents, and the population attributable fraction of death is estimated at 1

Preclinical animal models for onchocerciasis and loiasis: A systematic review of applications in drug screening.

BACKGROUND: Onchocerciasis and loiasis are co-endemic filarial neglected tropical diseases in Central and West Africa. While ivermectin-based mass drug administration has reduced onchocerciasis burden, it can trigger severe neurological adverse events in individuals with high Loa loa microfilaraemia. This limitation highlights the urgent need for safe macrofilaricidal therapies and appropriate preclinical models. METHODOLOGY/PRINCIPAL FINDINGS: We conducted a systematic review following PRISMA guidelines to evaluate animal models used for preclinical drug screening in onchocerciasis and loias

A historical note on an imported case of loiasis in Rio de Janeiro, Brazil, 1964.

Loiasis is a filarial disease transmitted by the Chrysops spp. tabanid flies in West and Central Africa. It is most commonly diagnosed by the clinical manifestations of Calabar swellings (transient localized inflammatory edema) or, most dramatically, by the appearance of a migrating worm through the conjunctival tissues or the bridge of the nose. We report the case of a 35-year-old resident in the city of Rio de Janeiro who displayed a moving Loa loa in the bulbar conjunctival tissue two years after returning from a six-month trip to Uganda. Surgical removal of the worm was performed.

Loiasis--a neglected and under-estimated affliction: endemicity, morbidity and perceptions in eastern Cameroon.

Loiasis is a neglected disease that may have great social and economic impact in some endemic areas. This study was designed to update the geographical distribution of loiasis and assess the frequency and perceptions of the clinical signs of the disease in the Eastern province of Cameroon. The investigation covered 32 villages and involved 4146 respondents. Human infection with Loa loa was endemic in all the study villages but the prevalence of microfilaraemia generally decreased from south to north. All of the study villages had local names for eye worm and Calabar swellings that varied in me

Markers of Loa loa infection in permanent residents of a loiasis endemic area of Gabon.

Different markers of infection were analyzed in 56 permanent residents of a Loa loa endemic village in Gabon. The population was divided into those with parasitological evidence of L. loa infection and those with no history of loiasis over the period of observation (c. 5 years). 26.7% of villagers had L. loa microfilariae, 33.9% had an ocular passage of an adult worm, and 17.8% had calabar oedema. Several other clinical symptoms were present in both groups of individuals, but none was considered to be pathognomonic for L. loa infection. Most of the villagers were polyparasitized, with Plasmodi

Distinct loiasis infection states and associated clinical and hematological manifestations in patients from Gabon.

BACKGROUND: Loiasis-a filarial disease endemic in Central and West Africa-is increasingly recognized as significant individual and public health concern. While the understanding of the disease characteristics remains limited, significant morbidity and excess mortality have been demonstrated. Here, we characterize clinical and hematological findings in a large cohort from Gabon. METHODS: Loiasis-related clinical manifestations and microfilaremia, hemoglobin and differential blood counts were recorded prospectively during a cross-sectional survey. For analysis, participants were categorized int

[Comparative analysis of 2 diagnostic methods of human loiasis: IgG4 serology and nested PCR].

By evaluating the diagnostic methods developed in our laboratory, the prevalence of loaiosis was estimated among 201 individuals from the province of Haut Ogooué in Gabon using IgG4 serology and nested-PCR. The study showed that the prevalence of loaiosis was higher than that described using standard microscopy. IgG4-based ELISA (Enzyme Linked Immunosorbant Assay) using crude extract of Loa loa microfilariae showed that 80% (35/44) of microfilaraemic individuals (MF') and 56% (88/157) of amicrofilaraemics (AMF) presented antibodies. By contrast, L. loa specific DNA amplified by nested-PCR was

Delayed-Onset Loiasis: A Case of Loa loa Infection Diagnosed Six Years Post-exposure.

Loiasis, caused by Loa loa, is a filarial infection endemic to Central and West Africa, increasingly recognized in non-endemic regions due to global migration and travel. Diagnosis in non-endemic regions can be challenging due to delayed symptom onset. We report a case of a 40-year-old woman presenting with ocular discomfort and a migrating subcutaneous parasite six years after residing in Gabon. Laboratory evaluation revealed eosinophilia and microfilariae on peripheral blood smear, confirming loiasis. Initial treatment with albendazole led to inflammatory symptoms, highlighting the risk of p
