Current State and Future Direction of Postpartum Hemorrhage Risk Assessment.

In the United States, postpartum hemorrhage is a leading preventable cause of maternal mortality and morbidity. To reduce morbidity from postpartum hemorrhage, risk assessment is an important starting point for informing decisions about risk management and hemorrhage prevention. Current perinatal care guidelines from the Joint Commission recommend that all patients undergo postpartum hemorrhage risk assessment at admission and after delivery. Three maternal health organizations-the California Maternal Quality Care Collaborative, AWHONN, and the American College of Obstetricians and Gynecologis

Factors associated with antenatal and delivery care in Sudan: analysis of the 2010 Sudan household survey.

BACKGROUND: Every day, globally approximately a thousand women and girls needlessly die as a result of complications during pregnancy, childbirth or the 6 weeks following delivery. The majority of maternal deaths are avoidable and could be prevented with proven interventions to prevent or manage complications during pregnancy and child birth. The aim of this study was to examine factors associated with underutilization of maternal health services in Sudan. METHODS: Data was obtained from the Sudan Household Health Survey 2010(SHHS). The SHHS collected data from 5730 women, aged 15-49 years an

Low-Value Diagnostic Imaging Use in the Pediatric Emergency Department in the United States and Canada.

IMPORTANCE: Diagnostic imaging overuse in children evaluated in emergency departments (EDs) is a potential target for reducing low-value care. Variation in practice patterns across Canada and the United States stemming from organization of care, payment structures, and medicolegal environments may lead to differences in imaging overuse between countries. OBJECTIVE: To compare overall and low-value use of diagnostic imaging across pediatric ED visits in Ontario, Canada, and the United States. DESIGN, SETTING, AND PARTICIPANTS: This study used administrative health databases from 4 pediatric E…

Defining spine care: a framework for scope, standards, and bidirectional care transitions.

Modern spine care is increasingly fragmented across operative and nonoperative disciplines, with inconsistent terminology, variable procedural standards, and uneven referral thresholds. These limitations can lead to duplicated services, delayed escalation when structural disease is present, premature escalation when operative targets are absent, and prolonged episodes of low-value care. This manuscript presents an implementation-oriented conceptual framework designed to improve transition reliability across the spine-care continuum. The framework was developed through targeted narrative synthe…

Watchful waiting as a strategy to reduce low-value spinal imaging: study protocol for a randomized trial.

BACKGROUND: Patients with acute low back pain frequently request diagnostic imaging, and clinicians feel pressure to acquiesce to such requests to sustain patient trust and satisfaction. Spinal imaging in patients with acute low back pain poses risks from diagnostic evaluation of false-positive findings, patient labeling and anxiety, and unnecessary treatment (including spinal surgery). Watchful waiting advice has been an effective strategy to reduce some low-value treatments, and some evidence suggests a watchful waiting approach would be acceptable to many patients requesting diagnostic test

Diet quality, risk factors and access to care among low-income uninsured American adults in states expanding Medicaid vs. states not expanding under the affordable care act.

BACKGROUND: The Affordable Care Act (ACA) Medicaid expansion varies in availability across states. PURPOSE: We compared characteristics of low-income uninsured residents in both Medicaid nonexpanding and expanding states with respect to their dietary quality, health risk factors, and access to care. METHODS: Data from the 2007-2012 National Health and Nutrition Examination Survey was matched with the Kaiser Family Foundation Medicaid expansion data. Bivariate and multivariate regressions were estimated to assess differences across expanding and non-expanding states. RESULT: The non-expansio

Absence of recognition of low alkaline phosphatase level in a tertiary care hospital.

SUMMARY: Low serum total alkaline phosphatase level (ALP), the hallmark for hypophosphatasia (HPP), must be recognized to provide appropriate care of the patients and to avoid antiresorptive treatment. The prevalence of persistent low ALP in a clinical setting is 0.13% and the recognition is very low (3%). INTRODUCTION: A low serum total alkaline phosphatase level is the hallmark for the diagnosis of hypophosphatasia. Although very rare, HPP must be recognized to provide appropriate treatment of non-union fractures and to avoid potentially harmful drugs, such as antiresorptive treatments. The

Predictive value of methicillin-resistant Staphylococcus aureus (MRSA) nasal swab PCR assay for MRSA pneumonia.

Pneumonia due to methicillin-resistant Staphylococcus aureus (MRSA) is associated with poor outcomes and frequently merits empirical antibiotic consideration despite its relatively low incidence. Nasal colonization with MRSA is associated with clinical MRSA infection and can be reliably detected using the nasal swab PCR assay. In this study, we evaluated the performance of the nasal swab MRSA PCR in predicting MRSA pneumonia. A retrospective cohort study was performed in a tertiary care center from January 2009 to July 2011. All patients with confirmed pneumonia who had both a nasal swab MRSA

Subgrouping patients with sciatica in primary care for matched care pathways: development of a subgrouping algorithm.

BACKGROUND: Sciatica is a painful condition managed by a stepped care approach for most patients. Currently, there are no decision-making tools to guide matching care pathways for patients with sciatica without evidence of serious pathology, early in their presentation. This study sought to develop an algorithm to subgroup primary care patients with sciatica, for initial decision-making for matched care pathways, including fast-track referral to investigations and specialist spinal opinion. METHODS: This was an analysis of existing data from a UK NHS cohort study of patients consulting in pri

Palliative care experts' relative importance of end-of-life care quality indicators: Findings from a discrete choice experiment.

BACKGROUND: End-of-life care delivery is shaped by subject matter experts who influence treatment decisions, policies, and programs and set guidelines that inform end-of-life care practices. However, little is known about what these experts view as most important when delivering high quality end-of-life care. AIM: To quantify the relative value that experts place on 13 key indicators of care quality at end-of-life and to assess whether preferences vary across high- and low/middle income countries. DESIGN: Cross-sectional survey using a discrete choice experiment that asks respondents to trad

Improved Value of Individual Prenatal Care for the Interdisciplinary Team.

OBJECTIVE: Innovative models of prenatal care are needed to improve pregnancy outcomes and lower the cost of care. We sought to increase the value of traditional prenatal care by using a new model (PodCare) featuring a standardized visit schedule and coordination of care within small interdisciplinary teams in an academic setting. METHODS: Prenatal providers and clinic staff were divided into four "Pods". Testing and counseling topics were assigned to visits based on gestational age. Interdisciplinary weekly Pod meetings provided coordination of care. A retrospective chart review was performe

Low-Value Prostate-Specific Antigen Screening in Older Males.

IMPORTANCE: The US Preventive Services Task Force guidelines advise against prostate-specific antigen (PSA) screening for prostate cancer in males older than 69 years due to the risk of false-positive results and overdiagnosis of indolent disease. However, this low-value PSA screening in males aged 70 years or older remains common. OBJECTIVE: To characterize the factors associated with low-value PSA screening in males 70 years or older. DESIGN, SETTING, AND PARTICIPANTS: This survey study used data from the 2020 Behavioral Risk Factor Surveillance System (BRFSS), a nationwide annual survey c

Practice of Kangaroo Mother Care Among Low-Birth-Weight Infants Discharged from Health Facilities and Its Outcome in Mekelle City, Tigray, Northern Ethiopia.

BACKGROUND: Kangaroo mother care is a proven intervention shown to be effective in reducing neonatal mortality among low-birth-weight infants. The paucity of evidence regarding the practice at home can be highlighted. This study aimed to assess the practice and outcome of kangaroo mother care at home among mothers having low-birth-weight infants discharged from two hospitals in Mekelle, Tigray, Ethiopia. METHODS: A prospective cohort study was conducted among 101 paired mothers and low-birth-weight neonates discharged from Ayder and Mekelle Hospitals. Non-probability purposive sampling was us

Variation in medical claim denials: safety-net providers are hardest hit.

INTRODUCTION: Medical claim denials impose substantial costs on providers but may also reduce low-value care, resulting in lower expenditures and better quality. Little is known about how denial rates vary across payer and provider segments, including for safety-net providers. METHODS: Using a large, multipayer claims dataset from 2019, we examined initial denials, appeal patterns, and final denials by payer segment, service type, and provider safety-net status. RESULTS: Medicaid managed care had the highest initial denial rate for professional claims (15.1%), while Medicare Advantage had th…
