Cardiovascular peripheral effector mechanism in postflight orthostatic intolerance: a simulation study.

Orthostatic intolerance (OI) following exposure to microgravity or head-down bed rest is frequently observed and is thought to be multifactorial origin. Although hypovolemia is considered as the primary cause of OI, the role played by other factors, such as the lowered vasoconstrictor responsiveness (VCR) of resistance vessels, the enhanced vasoconstriction response of cerebral vessels, and the depressed myocardial contractility need to be elucidated. It is difficult to assess experimentally how each of these changes would affect orthostatic tolerance and how these factors interact with each o

Regional changes in muscle mass and strength following 20 days of bed rest, and the effects on orthostatic tolerance capacity in young subjects.

To this day, many studies have suggested that prolonged bed rest (BR) affects on muscle mass and strength not only in gravity muscles but also in ungravity muscles. However, it is still unclear whether the decrease in regional muscle strength after BR is due to the alterations in the corresponding muscle mass, or not. On the other hand, if BR decreases the mass of antigravity muscles (UGM) as well as muscle strength and then increases tissue compliance of the antigravity muscles, orthostatic tolerance capacity will be decreased by the reduction in cardiac output (CO) in spite of the increase i

Daily use of countermeasures to prevent headward fluid shifts throughout 30 days of strict head-down tilt bed rest.

Strict 6° head-down tilt bed rest (HDTBR) induces a chronic headward fluid shift similar to weightlessness that may underlie the development of spaceflight-associated neuro-ocular syndrome. We tested the efficacy of two headward fluid shift countermeasures, using either 25 mmHg lower body negative pressure (LBNP, 2 × 3 h daily, n = 12) or use of venoconstrictive thigh cuffs (VTC) following 1 h of moderate aerobic cycle exercise (Ex + CUFF, 6 h daily for 6 of 7 days, n = 12) across 30 days of HDTBR. The study also included a seated posture group (Upright, 2 × 3 h daily, n = 11), which was condu

Effects of exercise and inactivity on intravascular volume and cardiovascular control mechanisms.

Exercise, inactivity and confinement have been used as effective tools to assess the contributions of vascular volume and baroreflexes to orthostatic hypotension associated with exposure to microgravity. Prolonged exposure to bedrest, physical inactivity, or wheelchair confinement removes baroreceptor unloading caused by regular upright standing and induces attenuation of cardiovascular baroreflex responses. The magnitude of reduced baroreflex sensitivity following bedrest or wheelchair confinement is related to the degree of orthostatic hypotension. Reduction in vascular volume caused by bedr

Changes in Optic Nerve Head and Retinal Morphology During Spaceflight and Acute Fluid Shift Reversal.

IMPORTANCE: Countermeasures that reverse the headward fluid shift experienced in weightlessness have the potential to mitigate spaceflight-associated neuro-ocular syndrome. This study investigated whether use of the countermeasure lower-body negative pressure during spaceflight was associated with changes in ocular structure. OBJECTIVE: To determine whether changes to the optic nerve head and retina during spaceflight can be mitigated by brief in-flight application of 25-mm Hg lower-body negative pressure. DESIGN, SETTING, AND PARTICIPANTS: In the National Aeronautics and Space Administratio

Insight into mechanisms of reduced orthostatic performance after exposure to microgravity: comparison of ground-based and space flight data.

Since the beginning of human spaceflight, the value of understanding mechanisms of physiological adaptation to microgravity became apparent to life scientists who were interested in maintining crew health and developing countermeasures agains adverse effects of the mission. However, several characteristics associated the the logistics of spaceflight presented significant limitations to the scientific study of human adaptation to microgravity. Because space missions are so infrequent and involve minimal numbers of crewmembers, meaninful statistical analysis of data are limited. Reproducibility

Impact of moderate isocapnic hyperthermia on dynamic cerebral autoregulation and its directional sensitivity.

The effects of heat exposure on dynamic cerebral autoregulation (dCA), the capacity of the cerebrovasculature to buffer rapid changes in arterial pressure, and its directional sensitivity, defined as the asymmetric cerebrovascular response to increases versus decreases in mean arterial pressure (MAP), remain incompletely understood. This uncertainty is largely attributable to concomitant heat-induced reductions in arterial carbon dioxide. We hypothesized that moderate isocapnic hyperthermia would impair dCA, particularly at higher frequencies of MAP oscillations, while preserving directional s

Naloxone decreases tolerance to hypotensive, hypovolemic stress healthy humans.

OBJECTIVE: In animal studies, naloxone, an opioid receptor antagonist, improves tolerance to hemorrhagic shock. The purpose of this study was to determine whether naloxone would augment tolerance to hypotensive hypovolemic stress (lower body negative pressure [LBNP]) in healthy human males. DESIGN: This study was a repeated measures design. SETTING: The experiments were conducted in a laboratory setting. SUBJECTS: Eight healthy male subjects were tested. The subjects' ages were 30 +/- 4.0 yrs, height = 177 +/- 7.0 cm, and weight = 75.5 +/- 3.5 kg (mean +/- SEM). INTERVENTIONS: Subjects und

Sympathoadrenal-circulatory regulation of arterial pressure during orthostatic stress in young and older men.

Our purpose was to test the hypothesis that human aging alters sympathoadrenal-circulatory control of arterial blood pressure during orthostasis. Plasma catecholamine and hemodynamic adjustments to two different forms of orthostatic stress, lower body suction (-10 to -50 mmHg) and standing, were determined in 14 young (26 +/- 1 yr) and 13 older (64 +/- 1) healthy, normally active men. During quiet supine rest, cardiac output tended to be lower and systemic vascular resistance higher in the older men, but no other differences were observed. On average, arterial blood pressure was well maintaine

Association between vasovagal hypotension and low sympathetic neural activity during presyncope.

Recent studies suggest that an underlying mechanism for susceptibility of patients and astronauts to presyncope includes hypoadrenergic responses to orthostatic stress. However, data used to reach this conclusion are open to various interpretations. In this report, maintenance of sympathetic neural activity (MSNA; peroneal nerve microneurography) during -60 mmHg lower body negative pressure (LBNP) was associated with maintenance of orthostatic tolerance, and disappearance of MSNA was associated with hypotension and pre-syncope. However, MSNA was substantially higher during progressive increase

New evidence for baroreflex and respiratory chemoreflex-mediated cerebral sympathetic activation in humans.

The brain is highly innervated by sympathetic nerve fibers; however, their physiological purpose is poorly understood. We hypothesized that unilateral cerebral norepinephrine (NE) spillover, an index of cerebral sympathetic nerve activity (SNA), would be elevated when engaging the baroreflex [via lower-body negative pressure (LBNP; -20 and -40 Torr)] and respiratory chemoreflexes [via carbon dioxide (CO2) administration (+8 Torr)], independently and in combination. Twelve young and healthy participants (five females) underwent simultaneous blood sampling from the right radial artery and intern

Compensatory reserve index during central hypovolemia and volume loading in healthy young and older hyperthermic adults: A pilot study.

The compensatory reserve index (CRI), derived from machine learning algorithms from peripherally obtained photoplethysmography signals, provides a non-invasive assessment of cardiovascular stability, that may be useful clinically. Briefly, the CRI device provides a value between 0 and 1, with 1 reflecting full compensable capabilities and 0 reflecting little to no compensable capabilities. However, the CRI algorithm was developed in younger to middle aged adults, such that it is unknown if older age modulates CRI responses to cardiovascular challenges. In young and older subjects, we compared

Influence of vagal control on sex-related differences in left ventricular mechanics and hemodynamics.

Left ventricular (LV) twist mechanics differ between men and women during acute physiological stress, which may be partly mediated by sex differences in autonomic control. While men appear to have greater adrenergic control of LV twist, the potential contribution of vagal modulation to sex differences in LV twist remains unknown. Therefore, the present study examined the role of vagal control on sex differences in LV twist during graded lower body negative pressure (LBNP) and supine cycling. On two separate visits, LV mechanics were assessed using two-dimensional speckle-tracking echocardiogra

Does the avoidance of nasogastric decompression following elective abdominal colorectal surgery affect the incidence of incisional hernia? Results of a prospective, randomized trial.

PURPOSE: In a previous, prospective, randomized study of the use of nasogastric tubes in patients undergoing elective abdominal colorectal surgery, we found that patients who did not have nasogastric (NG) decompression postoperatively had a significantly higher rate of abdominal distention, nausea, and vomiting. Patients from that study have now been followed for a median duration of 5.3 years to evaluate whether this elevation in perioperative intra-abdominal pressure would subsequently lead to an increased incidence of incisional hernia. RESULTS: Of the 251 patients who received NG decompre

Giant VAC in a patient with extensive necrotizing fasciitis.

The authors present a case of an extensive fulminant necrotizing fasciitis of the left flank, thigh, and lower parts of the leg treated with debridement, split-thickness skin grafting, and a giant negative pressure wound dressing covering 0.53 m( 2) or 18% of the body surface. To the authors' knowledge, this is the largest split-thickness grafted body surface successfully treated with negative pressure wound dressing documented in the literature.

The orthostatic tolerance of service personnel of the Household Division of the British Army.

Reflex syncope is common among UK Armed Forces (UKAF) personnel in the Household Division, particularly during ceremonial duties involving prolonged orthostatic stress. This study aimed to (1) assess orthostatic tolerance (OT) using head-up tilt combined with lower body negative pressure (HUT/LBNP), (2) evaluate the retrospective diagnostic accuracy of HUT/LBNP based on syncope trigger, and (3) examine test repeatability. Seventy-three service personnel were categorized into ORTHOSTATIC (27 %), NON-ORTHOSTATIC (16 %), or CONTROL (56 %) groups based on prior syncopal history. OT was measured to

Responses of cerebral blood velocity and tissue oxygenation to low-frequency oscillations during simulated haemorrhagic stress in humans.

NEW FINDINGS: What is the central question of this study? Do low-frequency oscillations in arterial pressure and cerebral blood velocity protect cerebral blood velocity and oxygenation during central hypovolaemia? What is the main finding and its importance? Low-frequency oscillations in arterial pressure and cerebral blood velocity attenuate reductions in cerebral oxygen saturation but do not protect absolute cerebral blood velocity during central hypovolaemia. This finding indicates the potential importance of haemodynamic oscillations in maintaining cerebral oxygenation and therefore viabil

Pressure-volume characteristics of the intact and disrupted pelvic retroperitoneum.

Hemorrhage is a major cause of mortality in pelvic fractures. Bleeding can be controlled in hypotensive patients by direct ligation, angiographic embolization, pelvic packing, and acute external fixation. Acute application of an external fixator can reduce pelvic volume and reduce bleeding fractures to effect tamponade. This therapy assumes that the pelvis represents a closed space, which clearly is not true anatomically. However, the premise may hold functionally. This study explored the relationship between pressure and volume in the intact and disrupted pelvic retroperitoneum. In cadaveric
