Entrapment neuropathies of the lower extremity.

Neuropathies that affect the lower limbs are often encountered after trauma or iatrogenic injury or by entrapment at areas of anatomic restriction. Symptoms may initially be masked by concomitant trauma or recovery from surgical procedures. The nerves that serve the lower extremities arise from the lumbosacral plexus, formed by the L2-S2 nerve roots. The major nerves that supply the lower extremities are the femoral, obturator, lateral femoral cutaneous, and the peroneal (fibular) and tibial, which arise from the sciatic nerve, and the superior and inferior gluteal nerves. An understanding of

The lower extremity in Morquio syndrome.

BACKGROUND: The modalities and results of surgical intervention in the lower extremity in children with Morquio syndrome type A [mucopolysaccharidosis-IV (MPS-IVA)] have not been well described. The aims of this study are to define the lower extremity deformities, and describe the results of intervention in MPS-IVA patients. METHODS: Retrospective chart and radiograph review of 23 MPS-IVA patients with a minimum follow-up of >2 years. Patients were divided into no intervention and surgical groups. Demographic data, surgical details, clinical results, and complications were recorded. Standard

Lower extremity surgery in muscular dystrophy.

INTRODUCTION: The natural progression of neuromuscular diseases results in inevitable musculotendinous contractures, most often in spite of early treatment. Surgery corrects this. The aim of this study was to evaluate the results of tendon surgery in these cases. MATERIALS AND METHODS: Twenty children with muscular dystrophy underwent hip surgery (tensor fascia lata tenectomy, rectus femoris and sartorius tenotomy), knee surgery (gracilis, semitendinosus and semimembranosus tenotomies) and/or ankle surgery (lengthening or tenotomy of the Achilles tendon with or without posterior tibial tendon

Fatal lower extremity varicose vein rupture.

Varicose vein rupture is a rare cause of death, although varicosities are a common pathology. We present three cases of sudden death due to varicose vein rupture. After a review of the literature, the case circumstances and the findings of imaging examination, performed in two cases, are presented. One of them had undergone a post-mortem computed tomography angiography (PMCTA), and one a PMCTA as well as a post-mortem magnetic resonance (PMMR) imaging prior to conventional autopsy. One of the cases presented herein is, to our knowledge, the youngest known fatality due to varicose vein rupture.

Lower extremity changes experienced during pregnancy.

The anatomic and physiologic changes occurring with pregnancy result in a variety of symptoms affecting the lower extremity. The purpose of this investigation is to provide a comprehensive look at the lower extremity changes experienced during pregnancy and correlate symptoms with underlying etiology in a literature review. In this retrospective study, 100 postpartum women were interviewed regarding the lower extremity changes experienced in pregnancy. The interview included dermatologic, vascular, neurologic, and musculoskeletal portions. Results demonstrate more than 50% of women reported fa

Lower extremity amputation: a contemporary series.

We sought to identify the results achieved with lower extremity amputations performed by both community and university-based surgeons as well as from multiple disciplines (orthopedic/general/vascular surgeons) serving a predominantly nonurban population. A review of 411 consecutive patients undergoing 508 non-traumatic lower extremity amputations at Charleston Area Medical Center from January 1999 to December 2003 was conducted. Amputations were performed most frequently at the below knee level (50.9%). Perioperative mortality (30-day) for the cohort was 11%. Mortality increased with more prox

Cycling injuries of the lower extremity.

Cycling is an increasingly popular recreational and competitive activity, and cycling-related injuries are becoming more common. Many common cycling injuries of the lower extremity are preventable. These include knee pain, patellar quadriceps tendinitis, iliotibial band syndrome, hip pain, medial tibial stress syndrome, stress fracture, compartment syndrome, numbness of the foot, and metatarsalgia. Injury is caused by a combination of inadequate preparation, inappropriate equipment, poor technique, and overuse. Nonsurgical management may include rest, nonsteroidal anti-inflammatory drugs, cort

Lower extremity injuries in youth sports.

The lower extremity is the most commonly injured anatomic area in sports. An appropriate history and physical examination is sufficient to diagnose most injuries. By knowing the position where a joint has the greatest and least bony stability, one can predict the likelihood of bony and ligament injury. Such information is helpful in determining which structures are most susceptible to injury as well as identifying when radiographs or other diagnostic studies are indicated. An accurate diagnosis is crucial for planning effective therapy and in determining the need for surgical referral. An unde

[Necrotizing fasciitis of the lower extremity].

Three cases of a necrotizing fasciitis, a rare but life- and limb-threatening infection, were treated in the Department of Surgery at the University Hospital of the Ruhr University Bochum (MHH) between October 1992 and January 1994. We compare our experiences with regard to bacteriological etiology, risk factors, localization, and place of entry, course, therapy and mortality with those in the literature.

Management of the compromised lower extremity.

Musculoskeletal injury associated with vascular compromise is a limb-threatening situation. When these injuries are associated with open fractures, the risk of limb loss is increased. In the case of an open fracture of the tibia associated with vascular compromise distal to the popliteal artery trifurcation, limb salvage is rarely indicated. Attempts at limb salvage in open tibial fractures are associated with a high risk of complication, prolonged recovery and poor function. Early amputation is more effective than delayed amputation.

Nerve grafting for lower-extremity injuries.

Nine patients were evaluated following nerve grafting of segmental injuries to the peroneal or sciatic nerve. Using a device that quantitated ankle dorsiflexion strength, the average return in strength was 11% recovery of the contralateral leg (correlating to motor grade M2). Four patients had a functional degree of motor recovery (grade M3 or M4), which corresponded to an average ankle dorsiflexion strength of 21% of the contralateral leg. The return of protective sensation occurred in all patients. Even though nerve grafts were required, repair of the peroneal and sciatic nerves provided eno

Reconstruction of the burned lower extremity.

Many of the long-term sequelae and deformities are preventable by early wound closure and aggressive physical managements. Concurrent and associated injuries and medical problems may place a patient with lower extremity burn injury into different clinical subsets. The standard care may have to be modified to accommodate these differences. The scars, the deformities, and the morbidity have decreased dramatically with early aggressive care and physical management. When contractures and deformities are recognized early in the rehabilitation phase, many of these can be satisfactorily corrected by

Digital venography of the lower extremity.

A digital 33-cm fluoroscopic system equipped with conventional spot-film and digital or video hard-copy capabilities was evaluated for its usefulness in diagnosing deep venous thrombosis of the lower extremities. The impact of different fluoroscopic field sizes, spatial resolution, and contrast variance was measured by using phantoms. The results indicate that the physical characteristics of the digital system are acceptable for lower-extremity venography. Digital fluoroscopic hard copy was compared with conventional spot films in 22 examinations. The digital examinations were as accurate as t
