PURPOSE: Kienböck disease is characterized by avascular necrosis of the lunate bone, which is usually progressive without treatment. This study examined lunate core decompression for its treatment potential.
METHODS: We surgically treated 20 patients with stage 1 to 3b Kienböck disease with lunate core decompression. We evaluated pain, range of motion, functional disability, and radiographic indices in these patients at baseline and 5 years after surgery.
RESULTS: The mean age of the patients was 29 years; 16 were men. Ten patients had Lichtman stage 1 disease, 6 had stage 2 disease, 3 had s

There are various biomechanical procedures for the treatment of lunate necrosis in the literature, but only a few deal with the possible revascularisation of the bone. We used the second dorsal metacarpal vascular bundle implantation technique in different stages of Kienböck's disease. Since 1990, twelve patients were treated by this method. In stage I-II according to Lichtman, we implanted the vessels in the lunate after excochleation and cancellous bone grafting, in stage III we used a modified form of the Graner operation and revascularised the proximalised part of the capitate. In the firs

In conclusion, this study combined three different technologies to simultaneously monitor scaphoid, lunate, and global wrist motion in three dimensions and concurrently collect data on the pressure distribution in the radiocarpal and ulnocarpal joints. This information was collected dynamically in real time while the wrist was moved in reproducible, physiologic cycles of motion. The scaphoid and lunate flex and extend as well as pronate and supinate while the wrist moves in the plane of flexion and extension. There is minimal radial and ulnar deviation of these carpal bones during this motion.

In the diagnostic work-up of Madelung deformity conventional radiographic imaging is often used, assessing the three-dimensional deformity in a two-dimensional manner. A three-dimensional approach could expand our understanding of Madelung deformity's complex wrist anatomy, while removing inter- and intra-rater differences. We measured previous two-dimensional-based and newly developed three-dimensional-based parameters in 18 patients with Madelung deformity (28 wrists) and 35 healthy participants (56 wrists). Madelung deformity wrists have increased levels of ulnar tilt, lunate subsidence, lu

We have used this operation for the management of pain due to osteoarthritic changes at the radio-scaphoid and capitate-lunate joints. It allows correction of the carpal height by reposition of the flexed lunate, preserves the load bearing capitate-lunate axis, and decompresses the radio-scaphoid joint. We have carried out the procedure in 25 cases. Perilunate instability with osteoarthritic changes were present in 14 wrists; the remainder were consequent to non-union of fractures of the scaphoid with osteoarthrosis, or following complex fracture dislocations. At review 10 patients were free o

BACKGROUND: The purpose of this study was to use micro-computed tomography to demonstrate the intraosseous vascularity of the lunate within a three-dimensional orientation to identify areas of greatest perfusion and define vascular "safe zones" for surgical intervention.
METHODS: Fourteen upper extremities were injected with a lead-based contrast agent. The lunates were harvested and scanned using a micro-computed tomography scanner. The intraosseous vascularity was incorporated into a three-dimensional image. Vessel number, diameter, distribution, and pattern were evaluated and analyzed. Vas

PURPOSE: To determine the amount of scaphoid and lunate translation that occurs in normal cadaver wrists during wrist motion, and to quantify the change in ulnar translation when specific dorsal and volar wrist ligaments were sectioned.
METHODS: We measured the scaphoid and lunate motion of 37 cadaver wrists during wrist radioulnar deviation and flexion-extension motions using a wrist joint motion simulator. We quantified the location of the centroids of the bones during each motion in the intact wrists and after sectioning either 2 dorsal ligaments along with the scapholunate interosseous li

Lunate dislocation is a circumambient disruption of the lunate bone. It is an uncommon but devastating wrist injury that can have negative effect on lifestyle with significant morbidity. This injury can go unrecognized by physicians resulting in complications including but not limited to carpal tunnel syndrome and Kienbock's disease. We present two cases of type 4 lunate dislocation that presented within 24 hours of injury; one had fallen off a moving motorcycle while the other had been involved in a motor vehicle accident. The second developed carpal tunnel syndrome. Radiographs showed typica

PURPOSE: Treatment options for advanced stage Kienböck disease primarily involve lunate excision, either in the context of proximal row carpectomy or in association with partial wrist fusion. This study aimed to investigate the in vivo three-dimensional carpal kinematics following arthroscopic lunate excision.
METHODS: We analyzed carpal translation and rotation of the affected wrist in five patients who underwent arthroscopic lunate excision for stage IIIC Kienböck disease. Our dynamic computed tomography image-based analysis focused on determining the centroid position of the scaphoid, triq

INTRODUCTION: Kienböck's disease is a rare and progressive osteonecrotic condition of the lunate that, in the absence of appropriate management, can result in irreversible structural deterioration of the wrist, collapse of the carpal architecture, and secondary osteoarthritis. Despite the existence of multiple therapeutic approaches, a standardized, evidence-based treatment algorithm has yet to be established. Treatment strategies are primarily determined by the salvageability of the lunate, which is classically considered viable when it preserves both its morphological integrity and structura

Kienböck's disease is a rare disorder that presents with wrist pain and limitation of motion and is caused by avascular necrosis of the lunate bone. Dialysis patients occasionally present with wrist pain. However, Kienböck's disease is rarely reported in dialysis patients. We report a case of 52-year-old woman with a 28-year history of hemodialysis who presented with acute wrist pain. T1-weighted magnetic resonance imaging showed diffuse low intensity of the lunate bone, consistent with the diagnosis of Kienböck's disease. Because this disease can lead to chronic debilitating wrist pain, promp

OBJECTIVE: Transposition of the pisiform bone is an operative treatment for lunatomalacia. The postoperative viability of the transposed pisiform bone is difficult to assess. The purpose of the study was to evaluate the utility of MRI for postoperative assessment of viability of the pisiform and lunate bones.
DESIGN AND PATIENTS: Six patients who underwent transposition of the pisiform for treatment of lunatomalacia, were assessed pre- and postoperatively with conventional radiographs (including tomography), CT and MRI.
RESULTS: Conventional radiographs, conventional tomograms and CT were al

We present the first two cases of juvenile patients, with advanced-stage lunatomalacia (Lichtman stage IIIB) in the literature, who were successfully treated with vascularized bone transfers. A corticoperiosteal metacarpal II bone graft was used in both patients, accompanied by an external monorail fixateur that was mounted for 6 weeks to achieve temporary wrist distraction. At 1-year follow-up, the patients had no residual pain, full wrist range of motion, and proper radiologic remodeling of the collapsed lunate. Surgically diminished compression forces at wrist level, temporary gain of joint

Twenty patients with Kienböck's disease who had undergone a joint levelling procedure, were reviewed at a mean of 11 years following surgery. Clinical and radiological evaluation revealed good long-term results with all patients complaining of less pain than before surgery and with statistically significant increases in range of motion and grip strength. Radiologically a significant number of patients were found still to have lunate fractures and fragmentation. In addition, 13 patients have developed new bone formation in the lunate fossa, and 12 show osteoarthritic changes, particularly at th

This study evaluated the effects of sectioning the scapholunate interosseous ligament, radioscaphocapitate ligament, and scaphotrapezial ligament on the kinematics of the scaphoid and lunate. Eight cadaver upper extremities were placed in a wrist joint simulator and moved in continuous cycles of flexion-extension and radial-ulnar deviation. Positional data of the scaphoid and lunate were obtained in the intact state, after the scapholunate ligament was cut; after the scapholunate and scaphotrapezial ligaments were cut; after the scapholunate, scaphotrapezial, and radioscaphocapitate ligaments

Models of scaphoid non-union with static dorsi-flexed intercalated segment instability were produced in five frozen arms from cadavers or subjects following accidents by repetitive mechanical loading of the wrist joints longitudinally after a bone defect has been made at the mid-portion of the scaphoid. We designed four models of reduction: anatomical reduction; reduction with a shortened scaphoid; anatomical reduction but with the radio-lunate ligament sectioned, and a shortened scaphoid with the radio-lunate ligament sectioned. Results suggested that anatomical reduction with rigid fixation

PURPOSE: The use of vascularized bone grafts for the treatment of Kienböck's disease may prevent ongoing lunate collapse and provide relief of wrist symptomatology. This study examines our experience with the use of the 4 + 5 extensor compartmental artery (ECA) bone graft for the treatment of Kienböck's disease.
METHODS: A retrospective review was performed of all patients having pedicled vascularized bone grafts for Kienböck's disease between 1991 and 2002. Only those patients who had reconstruction with a 4 + 5 ECA graft were included in the study. Presurgical and postsurgical measurements

PURPOSE: To evaluate 5 defined measurement techniques that are applicable to x-rays of Madelung's deformity: ulnar tilt, lunate subsidence, lunate fossa angle, palmar tilt, and palmar carpal displacement. The measurements rely on the longitudinal axis of the ulna and the carpal bones to determine drawing lines and avoid the distorted distal radius and its deformed lunate fossa. The reliability and reproducibility of the measurements is determined.
METHODS: Forty-eight sets of posteroanterior and lateral x-ray views of the wrist of subjects with the clinical diagnosis of Madelung's deformity w
