Silicone migration to the pleural space associated with silicone-gel augmentation mammaplasty.

We present a case report of silicone particles found in the pleural fluid of a patient 20 years after bilateral augmentation mammaplasty with silicone-gel implants. The patient's history is notable for bilateral replacements of implants and multiple closed capsulotomies several years subsequent to the original augmentation procedure. A ruptured left implant was found in 1991 when she first experienced pain in the upper back. A left pleural effusion developed subsequently. Analysis of the pleural effusion fluid by scanning electron microscopy suggested the presence of silicone. All laboratory r

Effects of Reduction Mammaplasty Operations on the Spinal Column: Clinical and Radiological Response.

BACKGROUND: Women with macromastia suffer from bodily disproportions like increased spinal curvature owing to the mass effect caused by severely large breasts. In such cases, the erector spinae muscles generate an overcompensatory pressure to maintain a normal posture, resulting in neck, back, and lumbar pain. This study aimed to objectively show the improvement of physical symptoms after reduction mammaplasty and evaluate psychological and physical changes of patients. METHODS: Pre- and postoperative cervical, thoracic, and lumbar bidirectional (anteroposterior and lateral) radiographs were

Anthropometric measurements and their value in predicting complications following reduction mammaplasty and abdominoplasty.

Body mass index (BMI) is the traditional way of assessing patients for body contouring surgery and has been shown to have a predictive value for surgical complications. In recent years, there have been discussions about accuracy of BMI as an indicator of obesity, and other obesity indices have been developed. This study aimed to measure and compare different anthropometric variables and examine their association with surgical morbidity. Prospective measurements of height, weight, waist, and percentage body fat were obtained preoperatively in 60 patients undergoing body contouring surgery. Demo

Impact of systemic tranexamic acid on postoperative drainage in large-volume reduction mammaplasty.

BACKGROUND: Optimizing clinical outcomes and postoperative recovery plays a key role in surgical procedures. We evaluated the effects of systemic tranexamic acid (TXA) administration on postoperative drainage in patients undergoing large-volume reduction mammaplasty. METHODS: A retrospective cohort analysis was performed by including 104 patients (208 breasts) who underwent large-volume reduction mammaplasty, defined as resection weights ≥ 500 g per breast. Overall, 59 patients (118 breasts) received intraoperative intravenous TXA, whereas 45 patients (90 breasts) served as a historic control

Risk factors for complications after reduction mammaplasty: a systematic review and meta-analysis.

BACKGROUND: Reduction mammaplasty (RM) is a commonly performed plastic surgery procedure, but various postoperative complications have been noted. This study aims to identify and quantify the association between risk factors and the occurrence of postoperative complications following RM. METHODS: We systematically reviewed PubMed, Medline, Cochrane Library and Web of Science for relevant publications, extracting suspected risk factors and associated postoperative complications. Meta-analyses were then conducted to evaluate their associations. RESULTS: We included 61 articles encompassing 71,

Asystolic Cardiac Arrest Associated With Unstable Bradycardia During Augmentation Mammaplasty: A Case Report.

Cardiac arrest is a rare but reported complication during breast augmentation surgery. It is even more rare in a reportedly healthy patient without preexisting cardiac disease. The authors present the case of a healthy 34-year-old female who underwent elective bilateral augmentation mammaplasty and experienced unanticipated asystolic cardiac arrest intraoperatively following general anesthesia supplemented by a regional pectoral (pec I) nerve block. The performing plastic surgeon provided cardiopulmonary resuscitation while the anesthesiologist initiated a rescue protocol per Advanced Cardiac

Pregnancy-associated gigantomastia recurrence and ectopic breast after reduction mammaplasty: A case report.

It has been reported that majority of cases of gigantomastia, also known as breast hypertrophy and macromastia, occur during either pregnancy or puberty. Gigantomastia is a rare disorder that does not have a clear etiology or well-established risk factors. We present a 26-year-old female patient who appeared to have pregnancy-associated gigantomastia recurrence, large accessory breast and, ectopic breast tissue at external genital three years after bilateral breast reduction surgery. The patient successively underwent bilateral mastectomy and vulvar tumor resection. Breast hypertrophy and prog

An autoprosthesis technique for better breast projection in free nipple graft reduction mammaplasty.

BACKGROUND: Reduction mammaplasty for macromastia provides relief from uncomfortable symptoms and improves self-confidence and the ability to participate in sports activities. Reduction mammaplasty using the free nipple graft technique may result in bottoming-out deformity and a lack of upper-pole projection. We describe a modified breast reduction technique that combines the Graf and Thorek methods. METHODS: We operated on 26 patients with gigantomastia using this novel technique. Preoperative markings were planned according to the classic Thorek amputation technique using a Wise pattern. A

Clinical outcome, quality of life, patients' satisfaction, and aesthetic results, after reduction mammaplasty.

Macromastia is a common indication for breast reduction within the public health care system in Sweden. To reduce the waiting time, a project was launched to operate on patients during a three-month period at a local hospital in Stockholm (Nacka Hospital). The operations were done by specialists and residents from the Karolinska University Hospital, using a medial flap technique. The aim of this prospective study was to evaluate the results of this project, focusing on clinical outcome, quality of life, patients' satisfaction, and aesthetic results. Personal and preoperative clinical data and

Septum-based mammaplasty: a surgical technique based on Würinger's septum for breast reduction.

BACKGROUND: During the past 7 years, the senior author (M.H.) has been performing septum-based mammaplasty. The aim of this article is to report the safety and ease of breast shaping by using this technique. METHODS: A series of 110 consecutive patients underwent septum-based breast reduction performed by a single surgeon. This technique uses a lateral or medial pedicle based on Würinger's horizontal septum, which carries the main nerve supply to the nipple in addition to intercostal perforators. RESULTS: Mean nipple-to-sternal notch distance was 33 cm (range, 22 to 45 cm). Mean resection wa

Successful revision surgery after postsurgical pyoderma gangrenosum following reduction mammaplasty: A case report.

BACKGROUND: Postsurgical pyoderma gangrenosum (PSPG) is a rare neutrophilic dermatosis that occurs following surgical procedures and frequently mimics postoperative infection. Misdiagnosis may lead to repeated surgical debridement and disease progression due to pathergy. Reports describing safe reoperation following PSPG remain limited, creating uncertainty regarding subsequent reconstructive procedures. CASE: We report the case of an 18-year-old woman with well-controlled perinatal HIV who developed PSPG following bilateral reduction mammaplasty. On postoperative day (POD) 2 she developed re

Nontobacco Nicotine Dependence Is Associated With Increased Early Postoperative Complications Following Reduction Mammaplasty.

BACKGROUND: Within the past decade, nontobacco nicotine dependence (NTND) has become more prevalent. However, the impact of nontobacco nicotine products on breast surgery remains less explored. Despite limited evidence, plastic surgeons routinely advise nicotine cessation regardless of source, which underscores the need to define the specific risks associated with NTND. OBJECTIVES: The authors of the present study sought to assess the effect of preoperative NTND on postoperative complications and revisions in patients who underwent reduction mammaplasty. METHODS: The Research Network on TriN

Balancing the Scales: Caution in Reduction Mammaplasty Recommendations for Obesity Class III Patients.

INTRODUCTION: The prevalence of obesity has risen worldwide, posing a problem to surgeons as obesity is a well-known risk factor for surgical outcomes. While prior studies have suggested performing reduction mammaplasty (RM) in patients with obesity, the variance in outcomes and quality of life (QoL) for obesity classes are ill-defined. We investigated whether obesity classes should be considered for RM by examining the surgical outcomes and QoL across different weight classes, aiming to pinpoint when outcomes become less favorable. METHODS: Patients undergoing RM by nine surgeons from 2016 t

Closed Incisional Negative Pressure Therapy Significantly Reduces Early Wound Dehiscence after Reduction Mammaplasty.

BACKGROUND: Closed incisional negative pressure therapy (ciNPT) has been shown to improve surgical outcomes. Functional reduction mammaplasty has a wound dehiscence rate of 25% and higher in most series, requiring extra care and delayed secondary healing. We aimed to determine if shifting from standard care dressings to ciNPT reduced early dehiscence after breast reduction. METHODS: This multisurgeon retrospective study compared consecutive patients undergoing primary breast reduction dressed with ciNPT to similar patients with standard dressing materials. Perioperative management was otherwi

"Medial Hemi-Mastopexy": An Innovation to Avoid Medial Pole Emptiness in Reduction Mammaplasty.

INTRODUCTION: Therapeutic mammaplasty is a well-established advanced oncoplastic technique for larger tumors in large breasts. However, the larger the breasts are, the greater is the lower outer parenchymal displacement due to weight resulting in relative emptiness in the medial/cleavage part. Therefore, the default practice of excising the medial wing parenchymal component of Wise pattern (WP) incision may either maintain or worsen such anatomy. METHODS: We present a technical innovation to improve the standard WP technique: instead of excision, the medial wing of the WP incision is deepithe

Proliferative Lesions Found at Reduction Mammaplasty: Incidence and Implications in 995 Breast Reductions.

BACKGROUND: Reduction mammaplasty relieves symptomatic macromastia. Pathologic specimens occasionally reveal unsuspected proliferative lesions or carcinoma. Few studies examine the incidence, risk factors, and outcomes in this population. METHODS: A retrospective review was performed between 2000 and 2012. The pathologic condition was categorized as benign, proliferative, or cancer. RESULTS: Five hundred seventy-two patients underwent 995 reduction mammaplasties. Cancer was detected in 23 specimens (2.3 percent) and proliferative lesions were detected in 137 (13.8 percent). Compared with pat
