Brain-machine interface facilitated neurorehabilitation via spinal stimulation after spinal cord injury: Recent progress and future perspectives.

Restoration of motor function is one of the highest priorities in individuals afflicted with spinal cord injury (SCI). The application of brain-machine interfaces (BMIs) to neuroprostheses provides an innovative approach to treat patients with sensorimotor impairments. A BMI decodes motor intent from cortical signals to control external devices such as a computer cursor or a robotic arm. Recent BMI systems can now use these motor intent signals to directly activate paretic muscles or to modulate the spinal cord in a way that reengage dormant neuromuscular systems below the level of injury. Perceived competence in computer use as a moderator of musculoskeletal strain in VDU work: an ergonomics intervention case.

Musculoskeletal strain and other symptoms are common in visual display unit (VDU) work. Psychosocial factors are closely related to the outcome and experience of musculoskeletal strain. The user-computer relationship from the viewpoint of the quality of perceived competence in computer use was assessed as a psychosocial stress indicator. It was assumed that the perceived competence in computer use moderates the experience of musculoskeletal strain and the success of the ergonomics intervention. The participants (n = 124, female 58%, male 42%) worked with VDU for more than 4 h per week. They to

Artificial neural network prediction using accelerometers to control upper limb FES during reaching and grasping following stroke.

This work investigates arm acceleration as a control signal for functional electrical stimulation (FES) of the upper limb during reaching and grasping. We segment the reach and grasp motion into phases and present an artificial neural network (ANN) approach that estimates the phase of the reaching cycle from accelerometer signals. We then select the stimulator command that maximizes successful triggering without unnecessary risk to the patient's safety. Our results suggest that the algorithm successfully generalizes between sessions and patients but is less successful at generalizing between d

Management by consent in human-machine systems: when and why it breaks down.

This study examined the effects of conflict type, time pressure, and display design on operators' ability to make informed decisions about proposed machine goals and actions in a management-by-consent context. A group of 30 B757 pilots were asked to fly eight descent scenarios while responding to a series of air traffic control clearances. Each scenario presented pilots with a different conflict that arose from either incompatible goals contained in the clearance or inappropriate implementation of the clearance by automated flight deck systems. Pilots were often unable to detect these conflict

Analysis of using EMG and mechanical sensors to enhance intent recognition in powered lower limb prostheses.

OBJECTIVE: The purpose of this study was to determine the contribution of electromyography (EMG) data, in combination with a diverse array of mechanical sensors, to locomotion mode intent recognition in transfemoral amputees using powered prostheses. Additionally, we determined the effect of adding time history information using a dynamic Bayesian network (DBN) for both the mechanical and EMG sensors. APPROACH: EMG signals from the residual limbs of amputees have been proposed to enhance pattern recognition-based intent recognition systems for powered lower limb prostheses, but mechanical sen

Human-machine Interface using functional electrostimulation and inertial sensors for lower limb rehabilitation in spinal cord injury individuals: a proof of concept.

A spinal cord injury (SCI) is a neurological disorder that impairs motor and physiological functions and leads to a reduced quality of life and autonomy for the person affected. In this scenario, human-machine interfaces (HMIs) have emerged as an effective tool to leverage residual motor capabilities and benefit injured persons. This work aims to develop a closed-loop HMI system for lower-limb rehabilitation composed of an in-house multi-channel Functional Electrical Stimulation (FES), which is activated by considering gait and pedaling cycles measured by an Inertial Measurement Unit. Graph-Driven Simultaneous and Proportional Estimation of Wrist Angle and Grasp Force via High-Density EMG.

Myoelectric prostheses are generally unable to accurately control the position and force simultaneously, prohibiting natural and intuitive human-machine interaction. This issue is attributed to the limitations of myoelectric interfaces in effectively decoding multi-degree-of-freedom (multi-DoF) kinematic and kinetic information. We thus propose a novel multi-task, spatial-temporal model driven by graphical high-density electromyography (HD-EMG) for simultaneous and proportional control of wrist angle and grasp force. Twelve subjects were recruited to perform three multi-DoF movements, includin

Consumer perspectives on the usability and value of the iBOT(®) wheelchair: findings from a case series.

PURPOSE: The iBOT(®) is an advanced power wheelchair introduced in 2003 for people with mobility impairments to enhance their independence and community access. Although this innovation soon gained popularity, the production of this wheelchair was recently stopped owing to the lower-than-expected sale figures. In this scenario, this study examines the impact of this wheelchair on current consumers by highlighting its usability in multiple contexts and verifying the value associated with its ownership. METHOD: Seven active consumers of the iBOT(®) were interviewed using a mixed method approach

An independent SSVEP-based brain-computer interface in locked-in syndrome.

OBJECTIVE: Steady-state visually evoked potential (SSVEP)-based brain-computer interfaces (BCIs) allow healthy subjects to communicate. However, their dependence on gaze control prevents their use with severely disabled patients. Gaze-independent SSVEP-BCIs have been designed but have shown a drop in accuracy and have not been tested in brain-injured patients. In the present paper, we propose a novel independent SSVEP-BCI based on covert attention with an improved classification rate. We study the influence of feature extraction algorithms and the number of harmonics. Finally, we test online c

Development and quantitative performance evaluation of a noninvasive EMG computer interface.

This paper describes a noninvasive electromyography (EMG) signal-based computer interface and a performance evaluation method based on Fitts' law. The EMG signals induced by volitional wrist movements were acquired from four sites in the lower arm to extract users' intentions, and six classes of wrist movements were distinguished using an artificial neural network. Using the developed interface, a user can move the cursor, click buttons, and type text on a computer. The test setup was built to evaluate the developed interface, and the mouse was tested by five volunteers with intact limbs. The

Changes in neuromuscular activity during motor training with a body-machine interface after spinal cord injury.

Body machine interfaces (BMIs) are used by people with severe motor disabilities to control external devices, but they also offer the opportunity to focus on rehabilitative goals. In this study we introduced in a clinical setting a BMI that was integrated by the therapists in the rehabilitative treatments of 2 spinal cord injured (SCI) subjects for 5 weeks. The BMI mapped the user's residual upper body mobility onto the two coordinates of a cursor on a screen. By controlling the cursor, the user engaged in playing computer games. The BMI allowed the mapping between body and cursor spaces to be

Is effective force application in handrim wheelchair propulsion also efficient?

BACKGROUND: Efficiency in manual wheelchair propulsion is low, as is the fraction of the propulsion force that is attributed to the moment of propulsion of the wheelchair. In this study we tested the hypothesis that a tangential propulsion force direction leads to an increase in physiological cost, due to (1) the sub-optimal use of elbow flexors and extensors, and/or (2) the necessity of preventing of glenohumeral subluxation. METHODS: Five able-bodied and 11 individuals with a spinal cord injury propelled a wheelchair while kinematics and kinetics were collected. The results were used to per

Speech audiometry by a speech synthesizer. I. A preliminary report.

A preliminary report on speech test results with a portable, text-to-speech synthesizer is presented. The differentiation scores achieved at speed 80 words/min vary. So far the best mean differentiation scores in normal material are 75%. The increase of the presentation level improves the differentiation score, as does the decrease of word speed and training. The future and present uses of this system are discussed. These include: devices for the handicapped, e.g. to produce speech for the mute, man-machine communication through speech in industry control, data processing systems and uses in a

Impact of Contact Force Monitoring in Acute Pulmonary Vein Isolation Using an Anatomic Approach. A Randomized Study.

BACKGROUND: The impact of contact force (CF) monitoring in pulmonary vein (PV) isolation after a circumferential anatomic ablation (CAA) is unknown. We analyze the usefulness of CF monitoring in acute PV isolation and procedure parameters using a CAA. METHODS: Fifty patients with paroxysmal atrial fibrillation were randomized into CF-on (CF >10 grams; n = 25) or CF-off (CF blinded; n = 25) groups. We performed a first round of CAA with a ThermoCool(®) SmartTouch(®) catheter blinded to the LASSO(®) catheter (Biosense Webster, Diamond Bar, CA, USA), with radiofrequency (RF) lesions tagged with

Accuracy of a P300 speller for people with motor impairments: a comparison.

A Brain-Computer Interface (BCI) provides a completely new output pathway that can provide an additional option for a person to express himself/herself if he/she suffers a disorder like amyotrophic lateral sclerosis (ALS), brainstem stroke, brain or spinal cord injury or other diseases which impair the function of the common output pathways which are responsible for the control of muscles. For a P300 based BCI a matrix of randomly flashing characters is presented to the participant. To spell a character the person has to attend to it and to count how many times the character flashes. Although

Dusty: an assistive mobile manipulator that retrieves dropped objects for people with motor impairments.

People with physical disabilities have ranked object retrieval as a high-priority task for assistive robots. We have developed Dusty, a teleoperated mobile manipulator that fetches objects from the floor and delivers them to users at a comfortable height. In this paper, we first demonstrate the robot's high success rate (98.4%) when autonomously grasping 25 objects considered being important by people with amyotrophic lateral sclerosis (ALS). We tested the robot with each object in five different configurations on five types of flooring. We then present the results of an experiment in which 20

Limitations in fields of vision for simulated young farm tractor operators.

Farm tractors account for the majority of deaths and injuries among youths working on North American farms. A vehicle operator's field of vision is an important operational aspect for safe driving. However, very little is known about visual limitations of young tractor operators compared to adult operators. The main purpose of this study was to quantify limitations in fields of vision of children with different anthropometry. The study was based on assessment of 42 farm tractors in popular use in the USA. The results showed that youth operators typically had diminished fields of vision compare

Improving left hemispatial neglect using virtual reality.

BACKGROUND: In hemispatial neglect, space can be dissociated on the basis of "near" peripersonal vs "far" extrapersonal space. The clinical manifestations of neglect can be modified by having patients use a tool to explore "far" extrapersonal space. An explanation for this is that the use of a stick produced an extension of body space resulting in a remapping of "far" space as "near" space. OBJECTIVE: S: To determine whether the remapping of space can be generalized to the amelioration of the "affected" vs the "nonaffected" space, rather than being confined to the selective amelioration of "f
