A critical audit of the surgical management of intractable epistaxis using sphenopalatine artery ligation/diathermy.

An audit of the local practice of sphenopalatine artery (SPA) ligation/diathermy was undertaken following its introduction to the unit in April 1998. The authors looked to the literature for evidence of what was to be taken as a successful result and were surprised at the lack of published data on its efficacy or lack thereof. Fivehundredsixtythree patients were admitted for inpatient management of epistaxis over a two-year period. Ten of these patients required surgical control of epistaxis and went on to have either sphenopalatine artery ligation or diathermy. One had concomitant anterior et

Enhancing Cognitive Impairment Assessment and Management in Hip Fracture Patients: A Two-Cycle Clinical Audit.

Cognitive impairment significantly affects elderly hip fracture patients, dramatically worsening their outcomes, yet healthcare teams consistently fail to screen for it. Despite national guidelines mandating routine cognitive assessment, compliance remains alarmingly low. This audit confronted this critical gap between evidence and practice in fragility hip fracture care.  Methods: We conducted a prospective two-cycle audit at Northwick Park Hospital in Harrow, England, to transform cognitive assessment practices. Cycle 1 (July-November 2023) exposed baseline failures in 30 patients aged ≥65 w

Ocular sequelae of epidermal necrolysis: French national audit of practices, literature review and proposed management.

Stevens-Johnson Syndrome (SJS) and toxic epidermal necrolysis (TEN) are serious and rare diseases, most often drug-induced, and their incidence has been estimated at 6 cases/million/year in France. SJS and TEN belong to the same spectrum of disease known as epidermal necrolysis (EN). They are characterized by more or less extensive epidermal detachment, associated with mucous membrane involvement, and may be complicated during the acute phase by fatal multiorgan failure. SJS and TEN can lead to severe ophthalmologic sequelae. There are no recommendations for ocular management during the chroni

Advanced clinical prioritisation in an Irish, tertiary, chronic pain management service: an audit of outcomes.

BACKGROUND: Advanced clinical prioritisation (ACP) pathways could potentially improve the interface between primary and secondary care, instigating appropriate treatment pathways with improved efficiencies. Telemedicine is a key component of ACP pathways. AIMS: Telephone consultations for new referrals (as part of a pilot ACP pathway for chronic pain) were trialled to try (a) improve efficiency of outpatient clinics, (b) expedite assessment/treatment, and (c) reduce the number of face-to-face attendances. An audit of this activity was undertaken. METHODS: The 100 longest waiting new referral

Major lower limb amputation audit - introduction and implementation of a multimodal perioperative pain management guideline.

Lower limb amputation is a frequent surgical intervention. It is well known to be associated with postoperative pain. Optimisation of perioperative pain has been shown to reduce the risk of chronic pain. There are no national guidelines for the perioperative pain management of lower limb amputations. Following a baseline audit, we devised a multimodal perioperative pain management guideline, which included the insertion of a local anaesthetic perineural catheter. All patients undergoing an elective or emergency above, through and below knee amputation were reviewed prior and following the impl

First-line management of metastatic castrate-resistant prostate cancer patients: Audit of real-life practices.

BACKGROUND: No reliable guidelines are available for choosing the best option between docetaxel and new hormonal therapies (NHTs) (i.e., abiraterone and enzalutamide) in first-line therapy for metastatic castration-resistant prostate cancer (mCRPC) patients. We performed an audit of real-life practices. METHODS: We built an online questionnaire and distributed it with the help of French oncology networks. This questionnaire was sent to 481 physicians who treat patients with mCRPC. All of the answers were declarative, individual, and anonymized. A descriptive analysis was done. A univariate lo

A novel audit methodology to improve detection of planning and delivery errors in passive motion management.

BACKGROUND AND PURPOSE: End-to-end dosimetry audits, including respiratory motion, typically compare film measurements acquired in a moving frame directly to planned dose distributions that do not resolve the applied motion. Inherent motion-induced dose differences cannot be distinguished from error-induced differences in this analysis. An alternate comparison approach is proposed utilising both motion-coupled and decoupled film measurements to overcome the motion-induced differences, and improve error detection. MATERIALS AND METHODS: Reference internal target volume-based plans were created

A clinical audit and cost analysis of latent TB management at a tertiary referral centre in Ireland.

INTRODUCTION: It is estimated that 24.8% of the world's population has latent TB. The World Health Organization's (WHO) End TB Strategy states that the systematic identification and management of LTBI in groups of people at high risk of reactivation is an essential part of TB elimination in low-incidence countries. AIM: Our aims were to evaluate the effectiveness of LTBI management at our tertiary referral outpatient department (OPD) and to identify how our service could be improved. METHODS: We included all patients seen in the infectious diseases outpatient service who were referred queryi

Postpartum hemorrhage in Suriname: A national descriptive study of hospital births and an audit of case management.

BACKGROUND: Postpartum hemorrhage (PPH) is the leading cause of direct maternal mortality globally and in Suriname. We aimed to study the prevalence, risk indicators, causes, and management of PPH to identify opportunities for PPH reduction. METHODS: A nationwide retrospective descriptive study of all hospital deliveries in Suriname in 2017 was performed. Logistic regression analysis was applied to identify risk indicators for PPH (≥ 500ml blood loss). Management of severe PPH (blood loss ≥1,000ml or ≥500ml with hypotension or at least three transfusions) was evaluated via a criteria-based au

Evaluation of the management of optic neuritis: audit on the neurological and ophthalmological practice in the north west of England.

The management of acute optic neuritis by neurologists and ophthalmologists in the north west of England was assessed in the light of the Optic Neuritis Treatment Trial (ONTT) recommendations. A questionnaire on a fictitious case of typical unilateral optic neuritis was mailed to all consultant ophthalmologists and neurologists working in the North West and Merseyside Health Authorities. They were then asked to comment on management of the case. Fifty two out of 86 ophthalmologists and 20 out of 28 neurologists replied. The overall response rate was 63%. Sixty five per cent of neurologists and

Optimizing the Management of Pediatric Bronchiolitis: A Multi-cycle Clinical Audit at a Regional Hospital in Oman.

Background Bronchiolitis is a major cause of lower respiratory tract infection and hospital admission among infants. Although international guidelines consistently recommend supportive care as the primary management approach, significant variation in clinical practice persists, often resulting in avoidable investigations, treatments, and healthcare expenditure, contributing to unnecessary costs and potential patient harm from avoidable interventions. Objective The objective of this study is to evaluate the effectiveness of a multi-cycle clinical audit in improving adherence to evidence-based b

A Closed-Loop Audit: The Assessment of Red Flags and Management of Acute Conjunctivitis in Primary Care.

Background Acute red eye is a common presenting complaint to primary care. Although conjunctivitis is the most common cause, some conditions can be sight-threatening. Diagnosing such conditions can be difficult given the broad differential diagnoses and limited specialist equipment. The National Institute for Health and Care Excellence (NICE) has published five "red flags," which may indicate the need for urgent ophthalmological assessment: reduced visual acuity (VA), copious discharge, marked eye pain/photophobia, contact lens use and recent trauma. For patients not requiring specialist refer
