Treatment of mandibular condylar process fractures: biological considerations.

The topic of condylar injury in adults has generated more discussion and controversy than any other in the field of maxillofacial trauma. It is an important subject because such injuries are common and complications of trauma to the temporomandibular joint (TMJ) are far-reaching in their effects. Why are there so many different methods to treat this injury? How can seemingly disparate treatment options all produce satisfactory outcomes in the majority of patients? The reason lies with the biological adaptations that occur within the masticatory system that are poorly understood, not readily qu

Mandibular angle fractures: two-miniplate fixation and complications.

BACKGROUND: Noncompression monocortical miniplate fixation of the mandibular angle is an accepted and reliable method for providing rigid internal fixation. High complication rates have been reported for internal fixation of angle fractures. OBJECTIVE: To analyze the outcome and complications in cases in which patients were treated with 2-miniplate fixation at the mandibular angle. DESIGN: A retrospective analysis of outcomes for a case series. SETTING: Treatment performed at a level 1 trauma-rated teaching hospital. METHODS: From May 1992 to September 2001, a total of 88 patients with ang

Treatment methods for fractures of the mandibular angle.

Fractures of the mandibular angle are plagued with the highest rate of complication of all mandibular fractures. Over the past 10 years, various forms of treatment for these fractures were performed on an indigent inner city population. Treatment included: 1) closed reduction or intraoral open reduction and non-rigid fixation; 2) extraoral open reduction and internal fixation with an AO/ASIF reconstruction bone plate; 3) intraoral open reduction and internal fixation using a solitary lag screw; 4) intraoral open reduction and internal fixation using two 2.0 mm mini-dynamic compression plates;

Complications of rigid internal fixation for mandibular fractures.

The application of rigid internal fixation to maxillofacial trauma is among the truly great advances that have been made in the field. The use of rigid internal fixation techniques has resulted in many advantages for the patient. Unfortunately, complications have occurred with this technology that are often devastating in their consequences for treatment outcomes. This article addresses some of the problems that stem from the use/misuse of rigid fixation devices in the treatment of mandibular fractures. It presents the most common complications and their management, including malunion, fibrous

Bilateral mandibular subcondylar fractures contributing to airway obstruction.

Bilateral mandibular fracture contributing to airway obstruction is a well-understood phenomenon. This is typically described as a rather immediate or acute process. After trauma, the production of a "flail mandible" with concomitant loss of support of tongue muscles is thought to result in obstruction of the upper airway. An unusual case in which the bilateral mandibular fractures contributed to an airway obstruction 10 days after the initial trauma is described. The cause is explained and methods of prevention and treatment are presented.

Patterns of mandibular fractures in Adelaide, South Australia.

Facial fractures are exceedingly common, and fractures of the mandible are the most common facial fracture. Over the past two decades a changing trend in the aetiology of these fractures has been apparent, with a decline in the percentage resulting from motor vehicle trauma, and an increase in the percentage resulting from assaults. A 3 year prospective study of 324 patients presenting to the Royal Adelaide Hospital with mandibular fractures was conducted and the patient groups, influence of alcohol, aetiology and type of fracture were examined and compared with other large series from around

Dental injuries in association with mandibular condyle fractures.

A total of 356 patients with mandibular condyle fractures were studied regarding associated dental injuries. One third of the patients had injured teeth, on average, 3.7 teeth had been injured per accident. Dental traumas were distributed equally between the anterior (incisors and canines) and posterior teeth (premolars and molars) in both jaws. In unilateral condylar fracture cases maxillary dental injuries involved more often premolars and/or molars of the fracture side than those of the nonfracture side. The mean number of dental traumas in the mandible was higher in cases of simultaneous m

Mandibular fractures. Their effect on growth and dentition.

Findings in 28 subjects having had childhood mandible fractures with long-term follow-up document asymmetric facial growth and dental abnormality in a high proportion. Facial growth abnormalities, five of which were moderate to severe, were detected by trained medical-dental examiners in 13 of the subjects (47%). Cephalometric abnormalities were detected in 18 (67%) of the 27 subjects so analyzed, with five (18%) meeting multiple roentgenographic criteria for abnormality. Frontal and lateral cephalometric abnormalities were found in nearly equal percentages among condylar-only, condylar-body,

Are you overlooking fractures of the mandibular condyle?

Fractures of mandibular condyle may be overlooked because attention is often focused on readily apparent soft tissue injury such as lacerations and abrasions. Clinical and radiographic signs are often subtle, but the injuries caused by rapid deceleration and listed in Table 2 will serve to alert the pediatrician to the possibility of such fractures. All children in these circumstances should be carefully examined for dental occlusion and the preauricular area palpated to help formulate a clinical diagnosis. Additional evidence is gained from the finding of deviation or limitation of the jaw up

Mandibular fractures: a review of 156 consecutive cases.

This report deals with a consecutive series of 156 mandibular fractures that were managed during a three-year period. The age and sex incidence are presented and the causative factors of the trauma are discussed. The fractures are classified according to anatomic site and laterality. The majority of the patients were treated with closed reduction techniques and the reasons for open reduction are tabulated. The results and complications of the treatment are enumerated. A conservative technique is described for open reduction of fracture-dislocations of the condylar head.

Relationship between mandibular condyle and angle fractures and the presence of mandibular third molars.

OBJECTIVES: We retrospectively evaluated the impact of mandibular third molars on the occurrence of angle and condyle fractures. MATERIALS AND METHODS: This was a retrospective investigation using patient records and radiographs. The sample set consisted of 440 patients with mandibular fractures. Eruption space, depth and angulation of the third molar were measured. RESULTS: Of the 144 angle fracture patients, 130 patients had third molars and 14 patients did not. The ratio of angle fractures when a third molar was present (1.26 : 1) was greater than when no third molar was present (0.19 : 1

Update on mandibular condylar fracture management.

PURPOSE OF REVIEW: Fractures of the mandibular condyle have provided a lasting source of controversy in the field of facial trauma. Concerns regarding facial nerve injury as well as reasonable functional outcomes with closed management led to a reluctance to treat with an open operative intervention. This article reviews how incorporating new technologies and surgical methods have changed the treatment paradigm. RECENT FINDINGS: Multiple large studies and meta-analyses continue to demonstrate superior outcomes for condylar fractures when managed surgically. Innovations, including endoscopic t

Bilateral mandibular fracture related to osteoradionecrosis.

Mandible is the most frequently affected bone during head and neck irradiation. Late changes in the mandible may manifest in the form of reduced bone density, dental caries, loss of spongiosa trabeculations, delayed healing following dental extraction, pathologic fractures, osteoradionecrosis, trismus, growth defects in children or second malignancies. Pathologic fractures of mandibular bone are rare and may be spontaneous or traumatic (following dental extraction). We report the case of a 55-year lady, who had undergone surgery and adjuvant radiotherapy for carcinoma oral tongue T2N0M0 on a c

[Facial nerve paralysis and mandibular fracture].

The authors describe three cases of peripheral facial nerve paralysis in patients with a mandibular fracture. In two cases, in which the onset of palsy was uncertain, the facial nerve injury was contralateral to the fractured side. Topodiagnostic tests showed neural damage at the third intrapetrosal portion and at the genicular ganglion. In one of the two patients tomography revealed a fracture line through the anterio-superior wall of the external auditory canal homolateral to the facial palsy. In the third subject palsy set in immediately after the trauma and was ipsilateral to the mandibula
