Effect of different surgical repair modalities on regeneration of the rabbit mandibular nerve.

A study was designed to evaluate the ability of the rabbit mandibular nerve to regenerate when exposed to crush and resection injuries, as well as to determine how differently sized resection injuries healed when repaired with either autogenous grafts or laminin-lined collagen tubulization. The nerve demonstrated a regenerative capacity over a 1-cm defect, with morphology and function that approximated normals, but could not span a 2-cm gap defect unaided. Crush injuries produced findings that were inferior to both those in normal nerves and in those with resections. In 1-cm defects, both graf

Longitudinal study of risk for facial nerve injury in mandibular condyle fracture surgery: marginal mandibular branch-traversing classification of percutaneous approaches.

OBJECTIVE: This study aimed to longitudinally assess the risk of facial nerve injury (FNI) in the surgical repair of mandibular condylar neck and subcondylar fractures (CN/SCFs) and to explore its predictors. MATERIALS AND METHODS: In a retrospective cohort study, the outcome was defined as FNI at 1 week and 1, 3, and 6 months postoperatively. Potential predictors included age, sex, etiology, fracture site and pattern (dislocation/non-dislocation), concomitant facial fractures, interval to surgery, surgeons' experience, plate types, and the marginal mandibular branch-traversing approach (deep

Age and Risk of Inferior Alveolar Nerve Injury After Removal of Lower Third Molars Close to the Mandibular Nerve.

BACKGROUND: Extraction of lower third molars (3 Ms) carries a risk of inferior alveolar nerve (IAN) injury. Anatomical proximity is a well-known risk factor, although the effect of patient age remains insufficiently explored. PURPOSE: The study purpose was to measure the association between age and IAN injury among patients whose 3 Ms are close to the mandibular canal. STUDY DESIGN, SETTING, SAMPLE: This case-control study included patients at increased risk for IAN injury due to close proximity of the lower 3 Ms to the mandibular canal, as assessed by preoperative cone-beam computed tomogra

Communication between the transverse cervical nerve (C2,3) and marginal mandibular branch of the facial nerve: a cadaveric and clinical study.

Several branches of the facial nerve are known to anastomose with branches of the cervical plexus, other cranial nerves, and the trigeminal nerve. Communication between the sensory transverse cervical nerve (C2, 3) and marginal mandibular nerve is, however, less well known, and in a previous study of 86 neck dissections we reported a 2.3% incidence of anastomoses between them. In this prospective study, we meticulously searched for more examples using both formalin-fixed cadavers and neck dissections. A total of 102 necks were included (both sides of 36 cadavers (n=72 necks), and 30 patients w

Ansa Cervicalis to Marginal Mandibular Nerve Transfer, a Feasible Option to Improve Lower Lip Function in Facial Nerve Paralysis.

Facial nerve palsy affecting the lower face may compromise essential functions such as speech and eating. The associated facial asymmetry can contribute to social isolation. Facial nerve reconstruction using a combination of interposition nerve grafts and/or nerve transfers offers significant improvement. However, these options are limited by insufficient donor nerves and unwanted synkinesis if a single donor nerve is used for the entire hemi-face. To ameliorate both limitations, we perform an ansa cervicalis to marginal mandibular nerve transfer to restore tone and symmetry to the lower third

Functional Mandibular Reconstruction With Integrated Nerve Repair: A Pathway to Dental Implant Rehabilitation.

Functional reconstruction of large mandibular defects, especially in young patients, presents a significant clinical challenge. The ideal approach should not only restore skeletal contour but also address nerve deficits and facilitate final occlusal rehabilitation, all while minimizing morbidity. This report describes a comprehensive, multi-staged strategy for such a case. A 22-year-old male patient was diagnosed with a large cemento-ossifying fibroma (COF) located in the molar region of the left mandible. He underwent a segmental mandibulectomy, resulting in a 40-mm defect. Immediate reconstr

Focused Investigation of Facial Nerve Dysfunction After Mandibular Distraction Osteogenesis for Robin Sequence.

Facial nerve dysfunction (FND) is a well-recognized but poorly documented complication of mandibular distraction osteogenesis (MDO) for Robin sequence (RS). This study aims to document the authors' experiences with FND and identify risk factors associated with this adverse event. A retrospective review of a prospectively gathered database was performed to identify patients with RS who underwent MDO at the authors' institution from March 2016 to June 2023. The authors included all infants with at least 3 months of follow-up after device removal. Data collected included patient demographics, inc

Inferior alveolar nerve function after open reduction and internal fixation of mandibular fractures.

PURPOSE: Mandibular fractures are amongst the most common facial fractures and are usually treated by open reduction and internal fixation (ORIF). Inferior alveolar nerve (IAN) injuries are seen frequently in mandibular fractures as well as after ORIF of these fractures due to the exposition and the close proximity of the nerve during fracture reduction. Therefore the continuity of the IAN can be disrupted. Permanent injury to the IAN can result in diminished quality of life. This retrospective study was designed to objectively analyse the incidence and the outcome of pre- and postoperative me

Unexpected temporary mental nerve deficit after extraction of mandibular first molar: case report.

UNLABELLED: Temporary neurologic deficit affecting the distribution of the left mental nerve was reported in a warfarinized, diabetic patient who had a history of previous CVA with right-sided body weakness. The neurologic deficit was reported following routine extraction of a left mandibular first molar. Recovery of sensation began six weeks after the procedure. Possible mechanisms and preventive measures are discussed. CLINICAL RELEVANCE: The reader should understand that mental paraesthesia can be an unexpected outcome in the most routine of mandibular extractions.

The use of phentolamine mesylate to evaluate mandibular nerve damage following implant placement.

High success rates and long-term predictability of implant therapy have been well documented in the literature. However, complications in implant treatment can arise and include sensory disturbances, especially in the posterior mandible in areas close to the inferior alveolar nerve. Treatment efficacy of sensory disturbances caused by implant placement in this area relies on the expeditious diagnosis of an induced paresthesia. Recently, phentolamine mesylate has been introduced as a reversal agent of local anesthesia with the ability to decrease the requisite time for a patient to return to no

Incidence of lingual nerve trauma and postinjection complications in conventional mandibular block anesthesia.

Trauma to the lingual nerve is potential risk factor for patients receiving mandibular block anesthesia. This paper describes the results of a patient survey of 9,587 conventional mandibular blocks received by 2,289 adults. The incidence of lingual nerve trauma and postinjection complications is reported along with an associated analysis of the duration of complications, procedures at the time of incident, the side of occurrence (right or left), and gender ratios. In addition, recommendations for patient evaluation, treatment, and follow-up care are discussed.

A novel extraoral ultrasound-guided approach for mandibular nerve block in Rahmani sheep.

BACKGROUND: Regional anesthesia is the favored choice in ruminant animals compared to general anesthesia, primarily due to its high safety margin and reduced demand for cost-intensive equipment in addition to its field applicability. Ultrasound-guided nerve block has gained significant attention in the field of veterinary medicine. This study utilized twenty-seven sheep of the native Rahmani breed to both demonstrate and evaluate the effectiveness of the ultrasound guided inferior alveolar nerve block technique. METHODS: The research comprised three phases: Phase 1 involved an anatomical exam
