[Treatment options for bipolar mania].

The diagnosis and treatment of bipolar mania are extremely challenging. Therapeutic intervention for mania has traditionally relied on the use of lithium or divalproex as a first-line treatment option. However, due to the limited therapeutic range of these agents, typical neuroleptics have often been used. Although these have demonstrated efficacy in mania, they are often associated with significant side effects, especially extrapyramidal symptoms. Thus, atypical antipsychotics are increasingly preferred in the treatment of bipolar mania. In this review, data from controlled studies for severa

Risperidone treatment of bipolar mania.

OBJECTIVE: To review clinical trial evidence regarding the efficacy and safety of risperidone for the treatment of bipolar mania. DATA SOURCES: Articles were identified through searches of PubMed (1950-August 2005), EMBASE (1988-August 2005 week 37), and International Pharmaceutical Abstracts (1970-August 2005) databases using the key words risperidone, atypical antipsychotics, and bipolar mania. Additional references were found through review of bibliographies of identified articles. PubMed searches for efficacy and safety were limited to clinical trials. STUDY SELECTION AND DATA EXTRACTION

[Adjunct treatments in acute mania].

When treating patients with manic states, the physician has to deal with at least two main Issues. First, the therapeutic decision has to be rapid because of the unpredictability of its immediate course. This concerns often results in polypharmacy with adjunct treatments. However, the therapeutic choice has to be cautious since part of the treatment will be maintained for prophylaxis. According to recent guidelines, the use of monotherapy with mood stabilisers during acute manic states concerns only few patients with mostly hypomania to moderate mania. Up to date, antipsychotics and benzodiaze

Mania in six preschool children.

At least nine cases of apparent preschool manic-depressive illness have been previously reported in the literature. In each of these children, a strong family history of affective illness was noted. In this report, the case histories of six preschool children ages 3 to 5 years with bipolar illness are summarized. These six were obtained from a sample of 36 consecutively hospitalized preschool children. Thus 17% of these hospitalized preschool children had bipolar illness. All had irritable mood, strong family history of affective illness, and previous presentation with symptoms of attention de

Loading strategies in acute mania.

Treatment of acute mania has been greatly influenced by loading strategies. Loading has potential benefits, including rapid symptom reduction in mania and a shortened length of stay. Disadvantages include an increased likelihood of adverse effects of the medications. Loading strategies for lithium, valproic acid (divalproex sodium), carbamazepine, oxcarbazepine, olanzapine, and haloperidol decanoate in the treatment of acute mania are discussed. Recent studies highlight this treatment option for selected patients. It is the unique properties of the medications that influence their use in loadi

Clinical EEG findings in mania.

Clinical EEG findings from 202 hospitalized manic patients repeated during 131 recurrences of mania were described. Results were considered in the light of current issues in the literature including the incidence of EEG abnormalities and minor variations, relationships between EEG and family history, EEG lateralization and longitudinal course of illness. The majority of patients had normal EEGs or mild nonspecific deviations compatible with effects of psychoactive medications. More definitive EEG abnormalities were observed in 16-percent. Microsleep occurred in 19 percent and small sharp spike

Secondary mania: diagnosis and treatment.

Increasing evidence indicates that subtypes of bipolar disorder differ not only in symptomatology and associated clinical features, but by differences in age at onset, illness course, and response to treatment. Secondary manic states differ from typical bipolar states and are often especially difficult to treat. Although the correction of the underlying organic factors (toxic, metabolic, or infectious) may effectively reverse the manic presentation, many organic factors are not reversible (trauma, stroke, and aging), and the presence of these etiologic factors can complicate traditional antima

[Dysphoric mania and mixed states].

Mixed states and depressive or dysphoric mania have been described by Kraepelin at the beginning of the century. They recently became the focus of interest and research. This type of bipolar disorder is frequent (30-40%), severe, difficult to treat. They can be associated with neurological and developmental disorders, substance abuse and other conditions. These patients are less responsive to lithium than patients with pure mania. Anticonvulsant drugs appear to be more efficacious. They can be prescribed with lithium. ECT proved to be useful in case of non response. The nature of these conditi

Mania after withdrawal of isocarboxazid.

Two cases of mania following the discontinuation of treatment with isocarboxazid are described. Although this phenomenon has been reported by several investigators to occur after cessation of tricyclic antidepressant therapy, there exists only one isolated report of hypomania after discontinuation of monoamine oxidase inhibitors. Possible mechanisms for the occurrence of hypomania and mania after withdrawal from antidepressants are discussed.

Dysphoric mania, mixed states, and mania with mixed features specifier: are we mixing things up?

Various terms have been used to describe mania when it is accompanied by depressive symptoms. In this article, we attempt to define and discuss 3 of these terms: dysphoric mania, mixed state, and mania with mixed features specifier. We conclude that whatever term is used, it is important to be aware that mania is more often unpleasant than pleasant, and that the unpleasantness is not limited to depression.

Antipsychotic drugs in mania: factors predicting use of antipsychotic medication following inpatient treatment of mania.

Inpatients treated for mania are frequently discharged on antipsychotics. There has been disagreement in clinical guidelines concerning their use, and there is little knowledge of the factors predicting this. The aim of the present study was to identify those factors predicting discharge on typical antipsychotic medication after hospital admission for mania. Data were collected retrospectively from the hospital notes of 74 patients who had 100 consecutive psychiatric hospital admissions for mania during the 1990s. Univariate and multivariate analyses were used to estimate odds ratios. The rate

Delirious mania intractable to treatment.

We present the case of a 68-year-old lady with a background diagnosis of bipolar disorder, who developed significant episodes of intractable delirium during each of her last three inpatient psychiatric admissions, where she was admitted with mania and psychosis. The case demonstrates diagnostic and management difficulties secondary to this delirium. We discuss the probable cause of this delirium and the various management strategies utilised in an effort to ameliorate her condition.

[Mania induced by antibiotic therapy].

We report two cases of adults who developed mania after taking the antibiotic clarithromycin. Clarithromycin is a frequently used antibiotic, but it can lead to a rare but significant psychiatric complication in the form of a manic episode. Mania is commonly associated with bipolar disorder, but the causes can be pharmacological, metabolic or neurologic, particularly when it occurs in patients who themselves or whose families have no past history of psychiatric illness. New-onset mania calls for detailed clinical and laboratory testing and neuro-imaging so that somatic causes can be ruled out.

Unipolar mania: a distinct entity?

BACKGROUND: Whether or not unipolar mania is a separate nosological entity remains a subject of dispute. This review discusses that question in light of recent data. METHODS: Unipolar mania studies in the PUBMED database and relevant publications and cross-references were searched. RESULTS: There seems to be a bipolar subgroup with a stable, unipolar recurrent manic course, and that 15-20% of bipolar patients may be unipolar manic. Unipolar mania may be more common in females. It seems to have a slightly earlier age of illness onset, more grandiosity, psychotic symptoms, hyperthymic temperam
