Treatment of midline cleft of the mandible-A 2-stage approach.

Midline clefts of the lower lip and mandible are rare congenital anomalies. They may present as complete or incomplete in nature along with associated bifid tongue and ankyloglossia. Some may present with cleft of lower lip, cleft of mandible, fissured tongue, absent hyoid bone, cleft of manubrium sterni, or an extremely rare anomaly known as Tessier cleft no. 30. We present this unique case of midline cleft of the lower lip (incomplete) and mandible (complete) with initial soft-tissue surgery for the lower lip. The hard-tissue surgery for this case is deferred at a later date, and the reasons

Thymopharyngeal duct cyst: a form of cervical thymus.

The various types of cervical thymus may present as a neck mass, usually laterally, from the angle of the mandible to the manubrium. Since it is rare to diagnose the entity preoperatively, the differential diagnosis includes the more common branchial cleft cyst, thyroglossal duct cyst, cystic hygroma, cystic dermoid, and lesions of the salivary gland, thyroid, parathyroid, and cervical lymph nodes. Because cervical thymic tissue in various forms has been reported so frequently, we feel the entity should be considered in the clinical differential diagnosis of lateral neck mass, especially in th

Straight Back Syndrome.

Straight back syndrome is a rare condition usually associated with mitral valve prolapse and "pseudo-heart disease" caused by a very narrow anterior-posterior chest due to the loss of the normal dorsal curvature of the thoracic spine. More rarely, the narrowed upper chest may compress the trachea causing extrinsic tracheal obstruction. If severe enough, this requires operative repair by effectively opening up the upper chest by a variety of techniques to allow the trachea enough room to avoid compression.

Manubriosternal joint dislocation: an unusual risk of trampolining.

BACKGROUND: Manubriosternal joint (MSJ) dislocation is a rare but potentially serious injury that can be associated with concurrent injuries to ribs, lungs, or myocardium. Two distinct types of MSJ dislocation have been described in the literature: type I, involving posterior dislocation of the sternum due solely to direct trauma; and type II, involving the sternum being pushed anteriorly as a result of indirect trauma. Until now, the relationship between the nature of the forces acting on the sternum and the type of MSJ dislocation that results has been absolute, whereby indirect forces never

[X-ray examination of the sterno-manubrial joint in ankylosing spondylitis and Reiter's disease (author's transl)].

In 19 patients with ankylosing spondylitis, 11 presented some lesions of the manubrio-sternal joint. In 3 cases these lesions were more advanced than the sacro-iliac alterations which were very slight or even doubtful. In 8 patients with severe Reiter's disease, 7 developed an ankylosing spondylitis and 2 presented involvement of the manubrio-sternal joint 8 weeks, respectively 10 weeks after the appearance of the first symptoms.

Anatomical and Functional Results of Medial and Lateral Surface of Temporal Muscle Fascia in Type 1 Tympanoplasties.

AIM: The aim of this study is to investigate whether there is a difference in the anatomical success of the graft and the functional gain of hearing as a result of the medial or lateral surface application of the temporalis muscle fascia graft. MATERIALS AND METHODS: Seventy-three patients who underwent tympanoplasty (myringoplasty) type 1 surgery due to tympanic membrane perforation between December 2017 and December 2019 were included in this study. The gender and age of the patients during this study were determined. Preoperative tympanic membrane perforation types were grouped as central,

Early development of the malleus and incus in humans.

It is widely accepted by developmental biologists that the malleus and incus of the mammalian middle ear are first pharyngeal arch derivatives, a contention based originally on classical embryology that has now been backed up by molecular evidence from rodent models. However, it has been claimed in several studies of human ossicular development that the manubrium of the malleus and long process of the incus are actually derived from the second arch. This 'dual-arch' interpretation is commonly presented in otolaryngology textbooks, and it has been used by clinicians to explain the aetiology of

Double cervical cyst derived from a single thyroglossal duct tract.

Thyroglossal duct cysts are one of the most common congenital cervical pathologic findings in children. This type of cyst can be located anywhere between the base of the tongue and the sternal manubrium. We report the case of a patient with a double thyroglossal cyst, one located in the hyoid region and the other in the thyroid gland. The 2 cysts were connected by a permeable tract, which confirms that an involution failure of the embryonic remains of the thyroglossal duct was responsible for the development of the cyst. Intrathyroid cysts of the thyroglossal duct, though uncommon, should be i

Retrosternal abscess after trigger point injections in a pregnant woman: a case report.

INTRODUCTION: Although retrosternal abscess is a well known complication of sternotomy and intravenous drug abuse, to date it has not been described as a consequence of trigger point injections. There are reported cases of serious complications as a result of this procedure including epidural abscess, necrotizing fasciitis, osteomyelitis and gas gangrene. CASE PRESENTATION: A 37-year-old African-American woman, who was 20 weeks pregnant, presented to our emergency room with complaints of progressively worsening chest pain and shortness of breath over the course of the last two months. Surgery in the cervicothoracic junction with an anterior low suprasternal approach alone or combined with manubriotomy and sternotomy: an approach selection method based on the cervicothoracic angle.

OBJECT: The authors propose an easy MR imaging method to measure and categorize individual anatomical variations within the cervicothoracic junction (CTJ). Furthermore, they propose guidelines for selection of the appropriate approach based on this new categorization system. METHODS: In the midsagittal section of the cervicothoracic MR imaging studies obtained in 95 Chinese patients, a triangle was drawn among 3 points: the suprasternal notch (SSN), the midpoint of the anterior border of the C7/T1 intervertebral disc, and the corresponding anterior border in the CTJ at the level of the SSN. Early sternal erosion and luetic aneurysms of thoracic aorta: report of 6 cases and analysis of cause-effect relationship.

Six patients with luetic aneurysm of the ascending aorta eroding the sternum are presented. The erosion was an early and principal presentation and the site of erosion and location and morphology of aneurysm were identical in all six patients. The erosion mainly affected the right half of the manubrium and medial end of right clavicle. The aneurysms arose from the junction of the ascending and transverse arches of the thoracic aorta and had narrow opening close to the origin of the innominate artery. The identical presentation, aetiology, angiographic location and morphology, corroborated furt

Multiple valve surgery for a patient with presternal oesophageal reconstruction.

A 73-year old woman presented with progressive exertional dyspnoea. Echocardiography revealed severe regurgitation of the aortic, mitral and tricuspid valves, indicating the need for multiple valve surgery. The patient had a past history of oesophageal cancer that had been treated with chemoradiotherapy followed by oesophagectomy with presternal reconstruction using a gastric tube and a pedicled jejunum covered by the rectus abdominal muscle flap. She underwent aortic and mitral valve replacement with prostheses and tricuspid ring annuloplasty through a lower partial median sternotomy to avoid

Seizure induced polytrauma; not just posterior dislocation of the shoulder.

A 61-year-old woman sustained multiple fractures secondary to the tonic clonic muscular contractions of a seizure. Her injuries included: bilateral proximal humerus posterior fracture dislocations, manubrium fracture, unstable sixth thoracic vertebrae crush fracture, bilateral acetabular fractures and a left femoral neck fracture. Seizures are a rare but recognised cause of fracture. Patients with osteoporosis are more susceptible to more serious fractures to long bones, spine and pelvis. The lack of history of high-energy trauma can lead to a delay in diagnosis. Rarely, the combination and ma

[Amyloid tumor of bone].

An osseous plasmocytoma was masked by an excessive tumor-like deposit of amyloid to such an extent that the primary disease was only diagnosed after histological examination of autopsy material from a 62 year old woman. The 20 cm in diameter tumor originated from the manubrium sterni. Other tumors were found in the ribs, right clavicle, thoracic and lumbal spine and in the left femur. A massive layer of amyloid was found in these tumors by microscopy. Plasma cells infiltrated the area between the amyloid plaques and the macroscopically tumor-free vertebral bodies. All other organs were free fr

Clavicular Distraction Osteoplasty With Sternoclavicular Reconstruction for a Medial Clavicle Fracture: A Case Report.

CASE: An 80-year-old man with osteoporosis sustained a complex, comminuted medial clavicle (MC) fracture that failed nonoperatively. A novel reconstruction involved MC resection, midclavicle distraction osteoplasty (DO) with iliac crest bone graft, and sternoclavicular (SC) joint reconstruction using a semitendinosus tendon allograft. By 1 year, the patient achieved successful healing and functional recovery. CONCLUSION: Patients with excessive MC bone loss may require a clavicular lengthening to approximate the MC to the manubrium. Restoring clavicular length through a DO in conjunction with

Atypical Bone Scintigraphy Findings in SAPHO Syndrome: A Case-Based Comparison With Typical Imaging Patterns.

SAPHO syndrome (synovitis, acne, pustulosis, hyperostosis, and osteitis) is a rare autoinflammatory disorder characterized by diverse skeletal and dermatologic manifestations. Bone scintigraphy plays a pivotal role in diagnosis, with the classic "bullhead" sign reflecting anterior chest wall involvement. Nevertheless, atypical scintigraphic patterns, particularly spinal involvement, can pose diagnostic challenges. We report 2 cases: one demonstrating typical tracer uptake in the manubrium and bilateral sternoclavicular joints, and another exhibiting predominant spinal osteitis. These cases ill

Sternal Tumor Resection and Complex Chest Wall Reconstruction of a Solitary Plasmacytoma: A Report of a Rare Case.

Localized sternal plasmacytoma is a rare and aggressive oncologic condition. Surgical resection followed by radiotherapy offers the highest chance of cure. Radical resection of the chest wall is technically feasible and is associated with improved outcomes. Subsequent reconstruction using meshes, plates, and both autologous and allogenic tissues allows for the restoration of chest wall function and may prolong survival. Patients with solitary plasmacytoma require close follow-up to monitor for local recurrence or distant disease progression. We present a case of a 63-year-old man diagnosed wit

[Osteoarticular infections associated with catheterization of the subclavian vein].

Access to a central way through catheterization of the subclavian vein is a widely used technique. Not uncommonly, the procedure is followed by infective complications among which clavicular osteomyelitis and septic sternoclavicular arthritis represent a rare eventuality. We report two cases of staphylococcic bacteremia produced after subclavian vein catheterization. Both patients presented septic sternoclavicular arthritis and osteomyelitis of the sternal manubrium. The isolated microorganisms were Staphylococcus aureus and Staphylococcus epidermidis methicillin-resistant, respectively. The c
