Electronic phenotypes to distinguish clinician attention to high body mass index, hypertension, lipid disorders, fatty liver and diabetes in pediatric primary care: Diagnostic accuracy of electronic phenotypes compared to masked comprehensive chart review.

BACKGROUND/OBJECTIVES: Electronic phenotyping is a method of using electronic-health-record (EHR) data to automate identifying a patient/population with a characteristic of interest. This study determines validity of using EHR data of children with overweight/obesity to electronically phenotype evidence of clinician 'attention' to high body mass index (BMI) and each of four distinct comorbidities. METHODS: We built five electronic phenotypes classifying 2-18-year-old children with overweight/obesity (n = 17,397) by electronic/health-record evidence of distinct attention to high body mass inde

A double-masked randomized crossover study comparing the effect of latanoprost/timolol and brimonidine/timolol fixed combination on intraocular pressure and ocular blood flow in patients with primary open-angle glaucoma or ocular hypertension.

PURPOSE: Ocular blood flow dysregulation has been implicated in the pathogenesis of glaucoma. Whereas the effect of single antiglaucoma substances on ocular blood flow has been addressed in various experiments, evidence for fixed combinations is sparse. In the present study, we set out to compare the effects of latanoprost 0.005%/timolol 0.5% (LT) fixed combination and brimonidine 0.2%/timolol 0.5% (BT) fixed combination on intraocular pressure (IOP) and ocular blood flow. METHODS: In the present study, which followed a randomized, double-masked 2-way crossover design, 16 patients with primar

A phase III, multicentre, randomised, investigator-masked, cross-over, comparative, non-inferiority trial evaluating the efficacy and tolerability of generic preservative-free Latanoprost (Polpharma S.A.) compared to Xalatan® (Pfizer) in patients with ocular hypertension or primary open-angle glaucoma.

BACKGROUND: Primary open-angle glaucoma (POAG), often associated with increased intraocular pressure (IOP), can lead to permanent damage of the optic nerve, concomitant visual field loss, and blindness. Latanoprost, a prostaglandin F2α analogue, reduces IOP and is used to treat glaucoma. In this clinical trial, we evaluated the efficacy of Latanoprost Polpharma, a generic preservative-free latanoprost 0.05 mg/ml eye drops solution, in lowering IOP when compared to the originator Xalatan® (latanoprost 0.005% ophthalmic solution, Pfizer). METHODS: This was a Phase III, multicentre, randomized,

Neighborhood Deprivation, Perceived Stress, and Pregnancy-Related Hypertension Phenotypes a Decade Following Pregnancy.

BACKGROUND: Hypertensive disorders in pregnancy and other adverse pregnancy outcomes (APOs) increase the risk of developing chronic hypertension and cardiovascular disease. Perceptions of stress and neighborhood context also influence blood pressure (BP) fluctuations. We examined if APOs, higher perceived stress, and neighborhood deprivation were associated with hypertension phenotypes a decade after pregnancy in untreated individuals. METHODS: Participants were 360 individuals who gave birth between 2008 and 2009 and participated in a research study 8-10 years following pregnancy. Standardiz

Proportion of US Adults Recommended Out-of-Clinic Blood Pressure Monitoring According to the 2017 Hypertension Clinical Practice Guidelines.

The 2017 Hypertension Clinical Practice Guidelines recommend out-of-clinic BP monitoring to screen for white coat and masked hypertension among adults not taking antihypertensive medication and white coat effect and masked uncontrolled hypertension among adults taking antihypertensive medication. We estimated the percentage of US adults meeting criteria for out-of-clinic BP monitoring by the American College of Cardiology/American Heart Association guideline using the 2011 to 2014 National Health and Nutrition Examination Survey (n=9623). Among US adults not taking antihypertensive medication,

Urinary catecholamines during stress and ambulatory blood pressure in children born full term but small for gestational age: a pilot study.

OBJECTIVES: Increased sympathetic activity is proposed to be a mechanism of high blood pressure in children born small for gestational age. Ambulatory blood pressure monitoring is a form of blood pressure measurement that can detect high blood pressure outside the hospital in patients with normal office blood pressure. This condition is called masked hypertension. There are limited data on association between ambulatory blood pressure and urinary catecholamines during exposure to stress in children born small for gestational age. METHODS: Nineteen children born small for gestational age and 1

Blood Pressure Phenotypes in Patients Recently Diagnosed With Type 2 Diabetes Mellitus and Matched Controls: A Study in Urban Mozambique.

Accurate characterization of blood pressure (BP) phenotypes is essential for cardiovascular risk assessment, particularly among type 2 diabetes mellitus (T2DM) patients. We aimed to evaluate the prevalence of different BP phenotypes using multiple BP assessment methods, in Black Africans, with and without T2DM. Recently diagnosed T2DM patients (n = 151) and age- and sex- matched controls (1:1) without T2DM had their BP assessed using standard office BP measurement (OBPM), unattended automated office BP measurement (u-AOBP), 24 h ambulatory BP monitoring (ABPM), and home BP monitoring (HBPM). Patterns of Ambulatory Blood Pressure in Normotensive Patients With Type 2 Diabetes Mellitus Attending a Tertiary Care Hospital in South India.

UNLABELLED: Systemic hypertension (HTN) is one of the most common comorbidities in diabetes mellitus (DM). Diabetic patients with HTN have a several-fold increased risk of cardiovascular disease (CVD) as compared to normotensive nondiabetic controls. Routine office blood pressure (BP) measurement does not help diagnose hypertensive phenotypes such as masked hypertension, white coat hypertension, etc. Hence, ambulatory blood pressure monitoring (ABPM) is a valuable tool for diagnosing these conditions, as it can help identify adverse cardiovascular complications. AIMS: To study the patterns of

Blood Pressure Pattern and Target Organ Damage in Patients With Chronic Kidney Disease.

In patients with chronic kidney disease, data on blood pressure (BP) pattern and its association with target organ damage, which indicates elevated cardiovascular risk, are sparse. In 305 treated hypertensive chronic kidney disease patients, we assessed BP pattern, left ventricular mass (magnetic resonance imaging), intima-media thickness (ultrasound), 24-hour-pulse wave velocity and 24-hour-central augmentation index (Mobil-O-Graph). Controlled hypertension (normal office and ambulatory BP) was found in 41% and sustained uncontrolled hypertension (elevated office and ambulatory BP) in 30% of

Routine use of ambulatory blood pressure monitoring in potential living kidney donors.

BACKGROUND AND OBJECTIVES: Most transplant centers exclude prospective living kidney donors with hypertension from donation. Centers routinely identify hypertension using BP measured in the clinic, but it is not clear that clinic BP accurately detects the presence or absence of hypertension in potential donors. We therefore conducted a prospective study to determine the impact of routine ambulatory BP monitoring on diagnosis of hypertension in potential donors and the value of other baseline characteristics in predicting ambulatory BP results. Ambulatory blood pressure monitoring in Egyptian children with nephrotic syndrome: single center experience.

BACKGROUND: Hypertension (HTN), especially masked hypertension, is one of the cardiovascular consequences of nephrotic syndrome. Masked hypertension cannot be identified during routine follow-up visits and adversely effects the patients' cardiac function. The purpose of this study was to use ambulatory blood pressure monitoring (ABPM) to evaluate the blood pressure status of children with nephrotic syndrome. METHODS: Ninety children with nephrotic syndrome (NS) participated in this cross-sectional study, which was carried out at Cairo University Children Hospital's nephrology clinic (CUCH). A

Redefining the phenomenon: risk of preeclampsia in white-coat hypertension during the second half of pregnancy in a high-risk cohort.

White-coat hypertension (WCH) during pregnancy has been linked to adverse maternal and fetal outcomes, although available evidence is inconsistent, particularly during the second half of gestation. This variability may be partly explained by heterogeneity in blood pressure (BP) phenotype definitions, especially regarding the inclusion of different ambulatory BP monitoring (ABPM) periods. We conducted a retrospective cohort study including pregnant women evaluated between 20 and 34 weeks of gestation within a high-risk pregnancy setting. Office BP and 24-h ABPM were obtained using a standardize
