Excessive fetal movements are a sign of fetal compromise which merits further examination.

Changes in fetal movement are associated with increased risk of stillbirth after 28 weeks of pregnancy. The majority of studies have focussed on maternal perception of reduced fetal movements, which is associated with stillbirth via placental dysfunction. Recent studies have also described an association between a single episode of excessive fetal movements and late stillbirth. We present a hypothesis that a sudden episode of excessive fetal activity indicates fetal compromise relating to underlying disturbance of the in utero environment, which if it persists can lead to fetal death. The orig

Can Ultrasound Performed in Prolonged Second Stage of Labor Predict the Difficulty and Success Rates of Operative Vaginal Delivery?

OBJECTIVE: To evaluate different sonographic methods for the prediction of the difficulty and the success of operative vaginal delivery (OPD). MATERIALS AND METHODS: A prospective study was performed on 45 term singleton uncomplicated pregnancies with prolonged 2nd stage of delivery with cephalic presentation. Measurements of the fetal head, relations between the fetal head and maternal pelvic parameters during rest and during maternal pushing were taken using translabial ultrasound. RESULTS: 29 cases of OPD were successful and 4 cases failed ending in cesarean section. The passage of the bi

The roles of maternal one-carbon metabolism and placental imprinted gene expression in placental development and somatic growth in a longitudinal birth cohort.

OBJECTIVES: One-carbon nutrients and imprinted genes both play critical roles in placental development and somatic growth. We aimed to examine (1) the impact of maternal one-carbon nutrition on placental imprinted gene expression, placental development, and infant growth and (2) interactions between one-carbon nutrients and imprinted genes in placental development and infant growth. METHODS: Women were interviewed prenatally about demographics and their alcohol, smoking, and drug use during pregnancy in a prospective longitudinal cohort study examining developmental effects of prenatal alcoho

History of perinatal loss and maternal-fetal attachment behaviors.

BACKGROUND: Maternal-fetal attachment (MFA) is an important requirement for optimal maternal-infant adaptation. Current studies showed conflicting findings about whether a history of perinatal loss (fetal/neonatal death) affects maternal attachment in pregnancy. RESEARCH QUESTION: "Does a history of perinatal loss affect maternal-fetal attachment behaviors?" METHODS: One hundred women with and without a history of PL were recruited using a convenience method of sampling, from prenatal care services affiliated to Shahid Behesti University of Medical Sciences. Data collected by questionnaires

Maternal history of maltreatment interacts with DNA methylation patterns to predict infant temperament.

BACKGROUND: Maternal childhood maltreatment (CM) has been associated with subsequent difficult infant temperament. Further, maternal CM and maladaptive infant outcomes have each been linked, separately, to increased methylation in umbilical cord blood of CpG sites in genes related to the stress response and inflammatory markers. Researchers have not yet examined the nature of the interactions of these factors or whether DNA methylation (DNAm) mediates or moderates the association of maternal CM with infant temperament. OBJECTIVE: We tested whether DNAm mediates or moderates the association be

Consequences of perturbations of fetal fuels in ovine pregnancy.

Maternal or fetal substrate infusions into chronically catheterized fetal lambs have recently been performed in order to examine the effects of excessive fetal substrate presentation on fetal metabolism and metabolic rate. The degree of maternal hyperglycemia in this animal model has been shown to relate to the degree of fetal hyperglycemia and to the magnitude of fetal glucose utilization. Series of direct fetal glucose infusions were performed designed to mimic the development of moderate maternal hyperglycemia. These studies suggest that fetal glucose excess stimulates fetal oxidative metab

Is Feto-Maternal Transfusion after Cesarean Delivery Different in Singleton and Twin Pregnancy?

Background: The aim of the study was to investigate if feto-maternal transfusion was related to the size of the fetal-maternal interface, and, therefore, was larger in twin pregnancy in comparison with singleton pregnancy. Methods: Blood samples from women with singleton (n = 11), and monochorionic (n = 11) and dichorionic (n = 13) twin gestations were tested. Flow cytometry tests with hemoglobin F, glycophorin A, and hemoglobin F and carbonic anhydrase simultaneous staining were used to detect fetal red blood cells and maternal F cells. Results: In all cases, the volume of feto-maternal trans

Catecholamine physiology in the ovine fetus. III. Maternal and fetal response to acute maternal exercise.

A study was made of the effects of maternal exercise on fetal plasma concentrations of catecholamines in nine ewes with chronically catheterized singleton fetuses at 125 to 137 days' gestation. The ewes were subjected to acute treadmill exercise of 2.5 mph for 45 minutes with continuous recording of maternal and fetal blood pressures. Samples of arterial blood were obtained for measurement of catecholamines, glucose, and blood gases. Changes in blood flow in fetal organs in response to maternal exercise were assessed by injection of radioactive microspheres. The maternal plasma catecholamine r

Intrapartum Fetal Heart Rate Anomalies in Acute Fatty Liver of Pregnancy: A Case Report and Pathophysiological Insights.

INTRODUCTION: Acute fatty liver of pregnancy (AFLP) is a rare but potentially life-threatening obstetric condition characterized by maternal hepatic dysfunction and risk of rapid clinical deterioration. In this setting, fetal hypoxia may develop through multiple mechanisms, including maternal metabolic acidosis, hypovolemia, and placental fatty acid accumulation. These factors may impair oxygen transfer, partly through alterations in the Bohr-Haldane effect, ultimately compromising fetal oxygenation. CASE PRESENTATION: We report a case of AFLP complicated by fetal heart rate abnormalities in

Embryo-fetal toxicity assessment of vonoprazan in rats and rabbits.

Vonoprazan is a new potassium-competitive acid blocker to treat acid-related diseases. However, its safety during pregnancy is unclear. The aim of the study was to investigate the potential reproductive toxicity on the embryo-fetal development of vonoprazan. Vonoprazan acetate was administered by intravenous injection to pregnant rats (0, 2, 6 and 20 mg kg-1 day-1 ) and rabbits (0, 1.2, 3.6 and 12 mg kg-1 day-1 ) during the organogenetic period (gestation day 6-15 [rats] and 6-18 [rabbits]). Maternal reproductive endpoints were evaluated, together with effects on fetal growth and morphological

Dissociation between maternal and fetal toxicity of methyl isocyanate in mice and rats.

The contribution of maternal hormonal changes and pulmonary damage on the fetal toxicity of methyl isocyanate (MIC) was studied in mice and rats. Exposure to MIC decreased maternal plasma progesterone levels in mice that lost but not in mice that retained pregnancy. Fetal toxicity of MIC was not related to changes in maternal plasma corticosterone levels. Neither chronic administration of progesterone nor the suppression of pulmonary edema with dexamethasone decreased fetal toxicity of MIC. Embryos exposed in utero or in vitro to MIC vapor exhibited a concentration-dependent decrease in growth

Narrative review of the relationship between the maternal-fetal interface immune tolerance and the onset of preeclampsia.

BACKGROUND AND OBJECTIVE: The establishment of maternal-fetal interface immune tolerance is essential for successful pregnancy. Studies have shown that spontaneous abortion, recurrent abortion, and fetal growth restriction are related to maternal-fetal interface immune dysfunction. Preeclampsia is an idiopathic condition related to pregnancy which manifests as hypertension, proteinuria, and other target organ damage after 20 weeks of gestation. Although the etiology of preeclampsia is still unknown, its pathogenesis is thought to be related to genetics, environment, and metabolism. In recent y

The regulated cell death at the maternal-fetal interface: beneficial or detrimental?

Regulated cell death (RCD) plays a fundamental role in placental development and tissue homeostasis. Placental development relies upon effective implantation and invasion of the maternal decidua by the trophoblast and an immune tolerant environment maintained by various cells at the maternal-fetal interface. Although cell death in the placenta can affect fetal development and even cause pregnancy-related diseases, accumulating evidence has revealed that several regulated cell death were found at the maternal-fetal interface under physiological or pathological conditions, the exact types of cel

[Supplements during pregnancy: fad or necessity?].

Needs for vitamins and micronutriments, enzyme cofactors in various metabolic processes, are notably increased during pregnancy in order to satisfy the requirements of fetal and placental growth. Limited maternal stores and an often inadequate dietary intake lead to deficiency, the materno-foetal consequences of which are beginning to be discerned: supplements thus appear to be useful. While there is hardly any discussion regarding iron and calcium, the authors feel that it should be broader, including water-soluble vitamins, magnesium and trace elements, in such a way as to deal with widely e

The effect of retinoic acid on chondrogenesis in the fetal hamster tibia in vivo.

Chondrogenesis in the fetal hamster tibia was examined 24, 36 and 48 h after maternal administration of retinoic acid at a known teratogenic dose (80 mg/kg) on day 10 of gestation. Twenty-four hours after treatment the tibial chondroblasts were more closely packed with less intervening fibrillar or granular matrix than in the controls. By 36 h post treatment, an intercellular matrix containing wide clumped fibrils unassociated with matrix granules had begun to accumulate, while the condroblasts had a relatively poorly developed Golgi apparatus and smooth, non-scalloped cell outlines. Cells fre

Teratogenesis and low copper status resulting from triethylenetetramine in rats.

The teratogenicity of triethylenetetramine (TETA) was studied using the Sprague-Dawley rat. TETA was fed during pregnancy at levels of 0 (control), 0.17, 0.83, or 1.66% in a complete purified diet. The frequency of resorptions and the frequency of abnormal fetuses at term increased with increasing levels of the drug. Maternal and fetal tissue copper levels were significantly lower in the TETA groups than in controls, with levels decreasing in a dose-related manner. Maternal kidney and fetal liver zinc levels increased within the TETA groups in a dose-related manner. Maternal liver iron was inc

Alterations in maternal-fetal gut and amniotic fluid microbiota associated with fetal growth restriction.

BACKGROUND: Fetal growth restriction (FGR) is typically characterised as the fetus' inability to reach its inherent growth potential. A growing body of evidence points to the important role of the maternal gut microbiota in FGR development. However, comprehensive research on changes in maternal-fetal gut and intrauterine microbiota related to FGR is lacking. METHODS: In this case-control study, we sequenced bacterial 16S rRNA from 35 maternal faecal, 35 meconium, and 31 amniotic fluid samples collected from 19 pregnant women diagnosed with FGR and 16 healthy controls. We identified putative b

Fetal cerebral oxygenation: the role of maternal hyperoxia with supplemental CO2 in sheep.

OBJECTIVE: We tested the hypothesis that supplemental CO2 can enhance the effect of maternal oxygen administration on fetal cerebral oxygenation. STUDY DESIGN: In near-term fetal sheep (n = 6), we instrumented the cerebral cortex with tissue PO2-laser Doppler flow probes, and placed arterial catheters. Following a 30-minute control period, the ewe breathed 50% O2 for 15 minutes, followed by added 6% CO2 for 15 minutes. We examined fetal cortical tissue PO2, cerebral blood flow (CBF), and fetal and maternal blood gases and related variables. RESULTS: In response to maternal O2 administration,

The influence of fetal sex on antenatal maternal glucose and insulin dynamics.

The 'Developmental Origins of Health and Disease' (DOHaD) hypothesis postulates that exposures during critical periods of development and growth, including maternal hyperglycemia, can have significant consequences for short- and long-term health in offspring. The influence of fetal status on maternal (patho)physiology is less well understood but gaining attention. Fetal sex specifically may be an independent risk factor for a range of adverse pregnancy outcomes, including increased gestational diabetes mellitus (GDM) frequency with male fetuses in multi-ethnic populations. Fetal sex has been t
