Management of maxillary sinus perforation at the time of immediate implantation with sinus-socket osteotomy (SSO) technique.

Immediate implantation related to socket and vital anatomy of the maxillary sinus area requires careful consideration. One major complication that may occur during immediate replacement of posterior teeth is maxillary sinus membrane perforation. To date, no specific technique has been proposed to manage this problem so that implant insertion remains possible during the same surgery. In this paper, we introduce a method to manage this type of perforation and while performing simultaneous implantation. We used sinus-socket osteotomy (SSO), a modified osteotomy technique for socket preparation th

Soft tissue changes following maxillary osteotomies in cleft lip and palate and non-cleft patients.

A retrospective study of 25 patients with unilateral cleft lip and palate and 25 patients with hypoplastic maxillae without a cleft was carried out to evaluate the effects of maxillary osteotomies at the Le Fort I level on the lip and nose profile. The pre-surgical cephalometric tracing was superimposed twice on the post-surgical cephalogram, on the cranial structures and the anterior maxillary structures, for landmark movement measurement. Results showed statistically significant correlations between soft and hard tissue movement in the cleft group. On average, the ratios of horizontal nasal

Maxillary advancement in cleft palate patients. A modification of the Le Fort I osteotomy and preliminary results.

A technique for maxillary advancement at the Le Fort I level in cleft palate patients is described. A partial thickness palatal flap which leaves the greater palatine vessels in situ allows the maxilla to be repositioned anteriorly without displacing the soft palate. This leaves velopharyngeal function undisturbed whilst maintaining a good blood supply to the maxilla. This operation has been carried out on 7 patients and preliminary observations on the results have been made using speech assessment, nasendoscopy, Doppler flow studies and an assessment of skeletal relapse. Six of the patients h

Reduction genioplasty enhances quality of life in female patients with prognathism and maxillary hypoplasia undergoing bimaxillary osteotomy.

The structure of the chin determines facial attractiveness and is directly linked to quality of life (QoL). In patients with prognathism and maxillary hypoplasia, bimaxillary osteotomy (BIMAX) with mandibular setback does not always lead to a more slender chin or improved aesthetics. The aim of the present study was to evaluate whether QoL differed between females undergoing BIMAX alone (group I; n=30) and those undergoing BIMAX combined with reduction genioplasty (group II; n=30). Presurgical and postsurgical evaluations included cephalography, photogrammetry, and the Oral Health Impact Profi

Ruptured Pseudoaneurysm of the Maxillary Artery and Its Branches Following Le Fort I Osteotomy: Evidence-Based Guidelines.

BACKGROUND: Although rare, pseudoaneurysms (PA) can develop following Le Fort I osteotomy and lead to life-threatening hemorrhage. However, the typical presentation of a PA following a Le Fort I osteotomy is not well characterized. Evidence-based guidelines are not currently available for evaluation of PA following Le Fort I osteotomy. METHODS: A case report is presented of a 27-year-old man who underwent Le Fort I advancement and subsequently developed a bleeding PA. A comprehensive search of journal articles was performed using the MEDLINE/PubMed database between 1964 and April 2016. Keywor

Equivalence of virtual and physical model surgery on digitally fabricated models for occlusal planning in segmental maxillary osteotomy.

Maxillary segmentation is often necessary to achieve optimal occlusion in orthognathic surgery, and although virtual surgical planning provides improved diagnostic precision and workflow efficiency, many surgeons continue to rely on conventional model surgery for establishing occlusion in segmented cases, with limited evidence directly comparing these approaches. This in-vitro study evaluated archived, de-identified preoperative intraoral scans of patients who underwent segmental maxillary orthognathic surgery at the University of Texas Health Science Center at San Antonio, including only case

Postoperative maxillary stability after Le Fort I osteotomy using a u-HA/PLLA system: three-dimensional analysis by surface superimposition based on virtual Le Fort I osteotomy.

The postoperative stability achieved with Le Fort I osteotomy (LFI) using bioabsorbable systems remains controversial. A new method - multipoint measurement method - was devised for detailed three-dimensional examination of postoperative stability following LFI, and the stability after LFI when using SuperFIXSORB-MX made of u-HA/PLLA was investigated. Thirty-one patients who underwent LFI using SuperFIXSORB-MX were evaluated retrospectively. The patients were divided into four malocclusion types: open bite, mandibular retrognathia, mandibular protrusion, and facial asymmetry. Seven maxillary r

Camouflage treatment of posterior bite collapse in a patient with skeletal asymmetry by using posterior maxillary segmental osteotomy.

Orthodontic treatment of posterior bite collapse due to early loss of molars and the consequent drift of adjacent teeth is complicated. When the posterior bite collapse occurs in patients with facial asymmetry, both transverse and vertical compensation are necessary for camouflage orthodontic treatment. In such cases, posterior maxillary segmental osteotomy (PMSO) can be an effective alternative procedure that simplifies the orthodontic treatment and shows long-term stability through dental compensation within the alveolar bone housing. This case report aimed to describe the orthodontic treatm

Malpositioned maxillary anterior implants correction using segmental osteotomy with three-dimensional guide and micro-saw: a technical note.

The maxillary anterior region, often referred to as the "esthetic zone," presents unique challenges in implant dentistry, especially when complications such as implant malposition occur. Malpositioned implants can lead to esthetic concerns, including gingival recession or implant fixture exposure, often resulting from facial placement following immediate implantation after tooth extraction. This case report outlines the successful correction of a facially malpositioned maxillary anterior implant using segmental osteotomy facilitated by three-dimensional (3D) technology and a micro-saw, combine

Identifying Predictors of Acquired Velopharyngeal Insufficiency in Cleft Lip and Palate Following Maxillary Osteotomy Using Multiple Regression Analyses.

BACKGROUND: Maxillary osteotomy is typically undertaken to correct abnormal facial growth in cleft lip and palate. The surgery can cause velopharyngeal insufficiency resulting in hypernasality. This study aims to identify valid predictors of acquired velopharyngeal insufficiency following maxillary osteotomy by using a range of perceptual and instrumental speech investigations and multiple regression. METHODS: A prospective study was undertaken consisting of a consecutive series of patients with cleft lip and palate (N = 20) undergoing maxillary osteotomy by a single surgeon. Participants wer

Concurrent bilateral total temporomandibular joint replacement surgery and conventional maxillary osteotomy utilizing virtual surgical planning web-based technology.

INTRODUCTION: We describe the case of a 31-year-old lady who presented in 1999 with significant temporomandibular joint (TMJ) dysfunction following a previous vertical subsigmoid osteotomy. The previous surgery had displaced both condyles from their glenoid fossae, and a persistent malocclusion resulted. Despite initial conservative management, both condyles remained out of their fossae and she re-presented 10 years later. On presentation, she had significant progressive right TMJ dysfunction with severe pain. A class III malocclusion was apparent with a 5-mm anterior open bite and jaw deviati
