A 33-yr-old African-American male with known human immunodeficiency virus (HIV) positivity underwent CT guided fine-needle aspiration biopsy of an anterior mediastinal mass. The aspirate was composed of a dimorphic population of cells that included small mature lymphoid cells and scattered cohesive groups of large epithelial cells in equal numbers. The neoplasm stained strongly for low weight molecular cytokeratin, epithelial membrane antigen (EMA), leukocyte common antigen (LCA), and Leu-7 which was consistent with a diagnosis of thymoma. Subsequent biopsies determined the neoplasm to be a ma

Primary mediastinal tumors in children are extremely rare, consisting of under 0.1% of neoplasms in this age group. Of these, teratomas represent 8%-16% of the cases. Clinical presentation depends primarily on the tumor's size, location, malignancy, and growth rate. Large tumor masses may compress structures in the thorax and cause respiratory distress syndrome, superior vena cava syndrome, or cardiac tamponade. This report describes a neonate with respiratory distress and heart failure shortly after birth due to a large mediastinal tumor mass. The child underwent successful surgery and has no

Giant cell tumor of bone is rarely seen in the rib, where it may present as a mediastinal mass. The diagnosis of giant cell tumor of bone is generally straightforward by fine-needle aspiration or needle core biopsy, but sampling problems may lead to confusion with other neoplasms or inflammatory processes. Here, we report a case of giant cell tumor of rib presenting as a mediastinal mass in a 36-year-old man. Because of inadequate sampling and inaccurate clinical information, the tumor was initially mistaken for thymoma. When the mass failed to respond to conventional chemotherapy, additional

CT and MR imaging allow earlier diagnosis and more specific characterization of anterior mediastinal masses than is possible with plain film radiographs. This review describes state-of-the-art CT and MR imaging of the anterior mediastinum. After a discussion of CT and MR imaging and indications for their use, normal and abnormal CT and MR findings in the anterior mediastinum are reviewed. Abnormalities include benign and malignant neoplasms, cysts, and mediastinal thyroid. Several masses such as thymolipomas, goiters, cysts, and lymphangiomas often do not require removal and now can be diagnos

Clinically detectable pulmonic stenosis resulting from compression of the pulmonary artery or right ventricular outflow tract by mediastinal tumors is uncommon. This report describes the occurrence of pulmonic stenosis secondary to a mediastinal tumor in a patient with Hodgkin's disease and in two patients with non-Hodgkin's lymphomas. Review of 35 published reports reveals that teratomas (ten cases) and Hodgkin's disease (eight cases) are the neoplasms most frequently reported to cause extrinsic pulmonic stenosis. Chest pain and dyspnea were the symptoms most commonly reported and a pulmonic

NUT midline carcinomas (NMCs) comprise a group of highly aggressive tumors that have been reported primarily in the head, neck, and mediastinum of younger individuals. These tumors overexpress the nuclear protein in the testis (NUT), most commonly due to a chromosomal translocation that fuses the NUT gene on chromosome 15 with the BRD4 gene on chromosome 19. Although the earliest recognized cases were described in the thymus or mediastinum, an extensive survey for NMCs among malignant thymic or other mediastinal neoplasms has not been reported. We examined NUT expression in 114 cases of poorly

The most common neoplasms of the anterior mediastinum are thymomas, lymphomas, and germ cell tumors. Surgical exploration was the routine approach to the diagnosis and management of these tumors. This is no longer true. The appropriate initial treatment of these neoplasms varies from surgical resection to radiation therapy to systemic chemotherapy. Except for the small well-encapsulated anterior mediastinal mass, it is imperative that a definitive tissue diagnosis be obtained before initiating treatment. An overview of these tumors and the specific approaches to their treatment are detailed.

Thymomas are relatively rare tumours of the anterior mediastinum, constituting approximately 10-15% of all mediastinal tumours. In contrast to other neoplasms, they rarely present distant metastases. We describe a case of thymoma with long survival and skin metastases diagnosed by two-colour flow cytometry.

Chordoma is a rare and slow-growing malignant neoplasm that arises from the embryonic notochord. It is rare to see a thoracic chordoma presenting as a posterior mediastinal mass with pleural seeding in a child. The authors report a chordoma of the thoracic spine with posterior mediastinal extension and pleural seeding in a 2-year-old boy who presented with asymmetric bilateral hand temperature and lower limb paralysis. The clinical course progressed rapidly, resulting in death.

Primary synovial sarcoma of the mediastinum is an exceedingly rare neoplasm. We describe a 31-year-old woman who had an incidental diagnosis of mediastinal mass. Histopathology and immunohistochemistry analysis confirmed the diagnosis of primary mediastinal synovial sarcoma. The patient underwent concurrent chemotherapy and radiotherapy, with minimal response radiologically. Resection was subsequently performed, with negative margins. The histopathologic examination revealed the diagnosis with a limited pathologic response. Because of the rarity of primary mediastinal synovial sarcoma, the opt

Descending necrotizing mediastinitis is a severe infection of the mediastinum. This syndrome manifests as fever and chest pain following cough and sputum production. A 49-year-old woman presented with fever and a 14-day history of pneumonia. CT showed mediastinal abscesses with a giant calcified mediastinal lymph node (21 × 18 mm) and pneumonia. Bronchoscopy by EBUS-TBNA under general anesthesia was performed. The pathogen found in the puncture culture was Streptococcus constellatus, and antibiotics (mezlocillin/sulbactam 3.375 IVGTT q8h) was administered. A proximal right main bronchial neopl

BACKGROUND: Mediastinal tumours in children are rare. Around 25% of them can be malignant. The thymoma is an uncommon neoplasm, and during adulthood it corresponds to 30% of anterior mediastinum tumours. The peak incidence is between 55-65 years.
CLINICAL CASE: A case of lymphocytic thymoma case is reported in a 4 year old patient with no previous or associated symptomatology. There was only a volume increase on the anterior neck region. The neck radiography and neck and chest tomography confirmed an anterior mediastinal mass surrounding the aorta and vena cava, as well as multiple mediastina

We report a case of a thymic neoplasm in an 18-year-old woman who presented with chest discomfort and an anterior mediastinal mass. The surgically resected tumor showed abundant adipose tissue admixed with thymic tissue and numerous medium-caliber blood vessels. We consider this tumor a rare variant of thymolipoma and designate it as thymohemangiolipoma. Because of its benign nature, it should be distinguished from other mediastinal lesions.

Fat-containing mediastinal masses, particularly mediastinal liposarcomas, are rare neoplasms that can grow to large sizes before becoming symptomatic and may be incidentally found on radiology examinations. In this case report, a 67-year-old male with a history of prostate cancer status post prostatectomy presented for an F-18-fluciclovine PET/CT for a rising, clinically detectable PSA and indeterminate pelvic lymph nodes seen on multiparametric MRI of the prostate. No local tumor recurrence or metastatic disease from prostate cancer was identified, but the PET/CT demonstrated a mixed soft tis

The mediastinum is among the most frequent anatomic region in which germ cell tumors (GCT) arise, second only to the gonads. Mediastinal GCT (mGCT) account for 16 % of all mediastinal neoplasms. Although the morphology and (according to all available data) the molecular genetics of mediastinal and gonadal GCT are identical, a number of unique aspects exist. There is a highly relevant bi-modal age distribution. In pre-pubertal children of both sexes, mGCT consist exclusively of teratomas and yolk sac tumors. The prognosis is generally favorable with modern treatment. In post-pubertal adults, vi

Background/Objectives: Contrast-enhanced endobronchial ultrasound (CE-EBUS) is a minimally invasive technique that combines traditional endobronchial ultrasound (EBUS) with a contrast agent (sulfur hexafluoride), enhancing the visualization of blood flow in mediastinal and hilar lymph nodes. This study aimed to assess the use of CE-EBUS in patients with advanced neoplasms and hilar or mediastinal lymphadenopathy, particularly to improve diagnostic accuracy and expedite sample collection. Methods: A retrospective observational study was conducted from April 2021 to December 2023, involving 49 p

IgG4-related disease (IgG4-RD) rarely presents as an anterior mediastinal mass and may radiologically resemble thymic neoplasms. We report a 70-year-old man with an incidentally detected anterior mediastinal tumour on chest CT. The lesion measured 33 mm, was well-circumscribed, homogeneous and partially calcified. Tumour markers and serum IgG4 were within the normal range. The patient underwent thoracoscopic resection under the diagnosis of thymoma. Histopathological examination revealed dense lymphoplasmacytic infiltration and abundant IgG4-positive plasma cells, confirming IgG4-related scler

Anterior mediastinal neoplasms are known to be causes of pressure symptoms on the heart and great vessels. This report describes a case of true thymic hyperplasia, a rare non-neoplastic condition, and speculates that pressure from this hyperplastic thymus may have produced a cardiac murmur.

Neuroendocrine neoplasms (NENs) of the thorax comprise a spectrum of pulmonary and mediastinal tumors ranging from well-differentiated, indolent lesions to highly aggressive carcinomas. Imaging, especially computed tomography and positron emission tomography, plays a central role in noninvasive tumor detection, characterization, staging, and treatment planning. Diagnosis of thoracic NENs prior to definitive histopathology is challenging, owing to their variable and often nonspecific imaging appearance, as well as the exceptional rarity of several subtypes. Because these tumors are often discov
