[Medication errors reporting in drug clinical trials: Role of the clinical research pharmacist?].

The investigational drugs circuit has specific risks, and medication errors may occur in clinical trials, possibly associated with adverse reactions. These risks must therefore be managed. In fact, there are few reports of medication errors during clinical trials. In a context of regulatory interpretation difficulties on this subject, we conducted a national survey that highlighted the heterogeneity of the methods used by academic sponsors to collect, code and report medication errors and the need to develop a culture of reporting these errors in clinical trials. This is why the REVISE group (

"My son was castrated as a result of a medical error. Is it OK to raise him as a eunuch?".

A 12-year-old boy lost both testes after testicular torsion. He is now 14, and his father wants to know if the boy should immediately start supplemental testosterone or if he might reasonably choose to live as a eunuch. The boy does not yet express any strong opinion except that he is embarrassed about his weight gain. We advised the father that there is no need to rush the decision as the boy could at least delay testosterone therapy until his teens or early 20s and still go through male puberty with little risk of adverse health effects. We seek to know if others endorse our endocrinological

Using simulation to identify sources of medical diagnostic error in child physical abuse.

Little is known regarding sources of diagnostic error at the provider level in cases of possible child physical abuse. This study examines medical diagnosis as part of medical management and not as part of legal investigation. Simulation offers the opportunity to evaluate diagnostic accuracy and identify error sources. We aimed to identify sources of medical diagnostic error in cases of possible abuse by assessing diagnostic accuracy, identifying gaps in evaluation, and characterizing information used by medical providers to reach their diagnoses. Eight femur fracture simulation cases, half of

[Medication errors with concentrated potassium intravenous solutions: Data of the literature, context and prevention].

Accidental direct intravenous injection of a concentrated solution of potassium often leads to patient death. In France, recommendations of healthcare agencies to prevent such accidents cover only preparation and intravenous infusion conditions. Accidents continue to occur in French hospitals. These facts demonstrate that these recommendations are insufficient and ineffective to prevent such deaths, especially those occurring during a catheter flushing. This article reviews the measures able to reduce the number of accidents. Countries which removed concentrated ampoules from ward stocks obser

Causes of medication administration errors in hospitals: a systematic review of quantitative and qualitative evidence.

BACKGROUND: Underlying systems factors have been seen to be crucial contributors to the occurrence of medication errors. By understanding the causes of these errors, the most appropriate interventions can be designed and implemented to minimise their occurrence. OBJECTIVE: This study aimed to systematically review and appraise empirical evidence relating to the causes of medication administration errors (MAEs) in hospital settings. DATA SOURCES: Nine electronic databases (MEDLINE, EMBASE, International Pharmaceutical Abstracts, ASSIA, PsycINFO, British Nursing Index, CINAHL, Health Managemen

What do hospital staff in the UK think are the causes of penicillin medication errors?

BACKGROUND: Medication errors are a potential major threat to patient's health, and allergic reactions occurring in patients with known allergies are an important preventable form of adverse drug event. The use of penicillin antibiotics in patients who are allergic to penicillin, in particular, is a major concern. AIM: To survey staff attitudes and beliefs to incidents involving penicillin allergic patients who are prescribed and administered penicillin antibiotics. SETTING: A 650 bed teaching hospital in England. METHOD: Using individual and (focus) group interview proceedings with a purpo

Medical error: a 60-year-old man with delayed care for a renal mass.

Mr B, a 60-year-old man with back pain, was not informed of an incidental finding of a renal mass suggestive of cancer on a magnetic resonance imaging scan. Failure and delays in test follow-up are a frequent problem in medicine, occurring in more than 5% of significantly abnormal ambulatory test results. Rather than simply blaming involved clinicians, systems for managing tests need to be reengineered using methods from reliability sciences. These begin with investigations into the systemic causes of the failures, then application of approaches such as heightened situational awareness, closed

Enhancing Coping and Communication Strategies Following Medical Errors: A Video Case Scenario Workshop for Pediatric Residents.

INTRODUCTION: Medical errors or adverse events can lead to complex emotions and a decrease in clinical self-confidence in health care workers. Limited training exists to teach residents communication strategies about disclosure, as well as coping and debriefing skills following medical errors. METHODS: As part of a required rotation for first-year pediatric residents, we developed a 1-hour virtual workshop that included a video case scenario, facilitated discussions, and expert input to teach communication skills and coping strategies following medical errors. The effectiveness of the worksho

Factors associated with the prevalence of refractive errors among medical students in the United Arab Emirates.

BACKGROUND: Refractive errors (REs) remain a leading cause of visual impairment worldwide. Medical students frequently report a high burden of REs, possibly attributable to genetic influences and lifestyle factors like extended near-work activities and limited outdoor exposure. Limited data exist on the prevalence and determinants of REs among medical students in the United Arab Emirates (UAE). This study aimed to determine the prevalence of REs in this population and to identify the key potential risk factors associated with them. METHODS: A cross-sectional survey was conducted among medical
