Wearable devices for improving medication adherence in older adults with chronic conditions: A systematic review.

BACKGROUND: Medication adherence remains a major public health challenge, especially among older adults managing multiple chronic conditions and complex therapeutic regimens. Poor adherence is linked to increased mortality, complications, hospital readmissions, higher healthcare costs, and reduced quality of life. Wearable devices (WDs), integrated into everyday accessories and equipped with sensors and reminder systems, may offer a practical strategy to support continuous monitoring and improve adherence. Given the still-limited evidence in this area, this review evaluates the impact of weara

A Transition of Care Service to Reduce Hospital Readmissions for High-Risk Cardiology Patients (RECARD): Protocol for a Pre-Post Interventional Trial.

BACKGROUND: Transition of care (TOC) is a high-risk period for medication errors as patients move between multiple health care professionals, settings, and changing medication regimens. Ineffective communication systems further increase the likelihood of medication harm and hospital readmission. A TOC service with tailored, patient-specific support may help reduce these risks. Cardiovascular medicines significantly contribute to medication harm, and patients recovering from acute myocardial infarction (AMI) or cardiothoracic surgery (CS) are vulnerable due to complex regimens and frequent medi

Effects of hospital pharmacist-community pharmacist collaboration on solving pharmacotherapy problems in stroke patients with post-discharge transitions: non-randomized comparative study.

BACKGROUND: Seamlessly providing appropriate drug therapy to patients recovering from stroke is essential for preventing recurrence, regardless of changes in their medical care environment. To address this, several initiatives have been undertaken to improve communication between hospital and community pharmacists, including the use of discharge medication summaries. However, we previously revealed discrepancies between the contents of prescriptions upon discharge and outpatient prescriptions from other institutions. Therefore, in the present study, we aimed to examine the effect of bidirectio

Measuring the operational impact of digitized hospital records: a mixed methods study.

BACKGROUND: Digitized (scanned) medical records have been seen as a means for hospitals to reduce costs and improve access to records. However, clinical usability of digitized records can potentially have negative effects on productivity. METHODS: Data were collected during follow-up outpatient consultations in two NHS hospitals by non-clinical observers using a work sampling approach in which pre-defined categories of clinician time usage were specified. Quantitative data was analysed using two-way ANOVA models and the Mann-Whitney U test. A focus group was held with clinicians to qualitativ

[10 years of emergency medical care at the traumatology center of the university clinic in Essen].

10 years of experience with emergency treatment at a University Center sufficiently proved the necessity of an emergency physician at the site of the accident. Because of its inefficiency, the separate and individual displacement of the doctor and the emergency vehicle to the site of the accident has been abandoned in 1976. The professional qualifications of the emergency team are as important as the technical equipment. Specific attention has to be paid to the education of nursing -, ambulance personnel and doctors belonging to the team. Very early and efficient shock therapy is essential in

Use of serum drug concentrations in outpatient clinics.

A four-month retrospective study was conducted to analyze the use of serum drug concentrations (SDCs) in the outpatient clinics of a teaching hospital. The general internal medicine, asthma and allergy, cardiology, and neurology clinics were selected as frequent users of SDCs. Serum theophylline, digoxin, quinidine, procainamide, carbamazepine, phenytoin, and phenobarbital concentrations obtained during patient clinic visits were audited by trained pharmacy students using medical records and computer-generated laboratory data. The SDCs were evaluated for (1) the appropriateness of how they wer

An Investigation of Drug-Drug Interaction Alert Overrides at a Pediatric Hospital.

OBJECTIVES: Drug-drug interactions (DDIs) can result in patient harm. DDI alerts are intended to help prevent harm; when the majority of alerts presented to providers are being overridden, their value is diminished. Our objective was to evaluate the overall rates of DDI alert overrides and how rates varied by specialty, clinician type, and patient complexity. METHODS: A retrospective study of DDI alert overrides that occurred during 2012 and 2013 within the inpatient setting described at the medication-, hospital-, provider-, and patient encounter-specific levels was performed at an urban, qu

INTRODUCTION: Despite major policy initiative, Germany's hospital digital transformation remains slow. METHODS: This study analysed data from the IT-Report Healthcare (2017, 2023 and 2025) to evaluate changes in five domains related to the Hospital Future Act (KHZG): clinical documentation, order entry, clinical decision support (CDSS), medication management and discharge management. A panel of 49 hospitals was examined using linear mixed models (LMM). RESULTS: While most hospitals applied for funding in documentation and discharge management, by 2025, most projects also in these areas were

Evaluation of alerts for potassium-increasing drug-drug-interactions.

Electronic alerts for preventing hyperkalaemia during potassium-increasing drug-drug-interactions (DDIs) are often overridden due to their low specificity. Treatments of 76,467 inpatients were retrospectively analysed to establish more specific alerts. Alerting concepts for identifying DDIs that induced hyperkalaemia (serum potassium ≥5.5 mEq/l were compared. The positive predictive value (PPV) of alerts was 2.9% if they were triggered at onset of each potassium-increasing DDI. The PPV increased to 5.1% if alerts at onset were suppressed for serum potassium levels of <4.0 mEq/l. The PPV ros

How can a standard software package for data management in anesthesia be achieved?

Collecting data for administrative, statistical, medical, and organizational purposes is becoming increasingly important in anesthesia. In 1986 the Swiss Society for Anesthesiology decided to create a program that would be compatible for different computers and would expedite data collection. The system developed was called Information System for Operations (ISOP), which was written in the database and programming system Massachusetts General Hospital Utility Multi Programming System (MUMPS). It was installed in eight hospitals and met the initial requirements, but the individual requirements

Medication Adherence Barriers and Their Relationship to Health Determinants for Saudi Pediatric Dialysis Patients.

Medication adherence is critical for the treatment and improved outcomes of chronic diseases. However, there is little research on the medication adherence of pediatric dialysis patients in Saudi Arabia. This study examines medication adherence barriers and their relationship to health determinants among Saudi children on dialysis, to enhance treatment success. We conducted a hospital-based, cross-sectional survey of pediatric dialysis patients using a simple random sampling technique. There is a trend of higher medication adherence for peritoneal dialysis patients compared with hemodialysis p

Challenges And Opportunities For Improving Patient Safety Through Human Factors And Systems Engineering.

Despite progress on patient safety since the publication of the Institute of Medicine's 1999 report, To Err Is Human, significant problems remain. Human factors and systems engineering (HF/SE) has been increasingly recognized and advocated for its value in understanding, improving, and redesigning processes for safer care, especially for complex interacting sociotechnical systems. However, broad awareness of HF/SE and its adoption into safety improvement work have been frustratingly slow. We provide an overview of HF/SE, its demonstrated value to a wide range of patient safety problems (in par
