Antiparasitic nitroimidazoles. 9. Synthesis of some 2-(4-dialkylaminomethylstyryl)-and 2-(4-amidinostyryl)5-nitro-1-vinylimidazoles.

A series of 2-styryl-5-nitro-1-vinylimidazoles carrying alkylaminomethyl or amidino functions in the 4 position of the styryl ring was prepared and evaluated for antitrypanosomal activity in mice infected with Trypanosoma rhodesiense. The alkylaminomethyl compounds were found inactive against Trypanosoma cruzi infections in mice. One compound, 2-(4-methylamidinostyryl)-5-nitro-1-vinylimidazole hydrochloride, showed antitrypanosomal activity comparable to the standard drugs suramin, pentamidine, diminazene, and melarsoprol when tested ip against T. rhodesiense infected mice and also showed some

Erythrina abyssinica prevents meningoencephalitis in chronic Trypanosoma brucei brucei mouse model.

Human African trypanosomiasis is prevalent in Sub-sahara African countries that lie between 14° North and 29° south of the equator. Sixty million people are at risk of infection. Trypanosoma brucei gambesience occurs in West and Central Africa while Trypanosoma brucei rhodesience occurs in East and Southern Africa. The neurological stage of the disease is characterized by neuroinflammation. About 10% of patients treated with the recommended drug, melarsoprol develop post treatment reactive encephalopathy, which is fatal in 50% of these patients, thus melarsoprol is fatal in 5% of all treated p

[Pharmaceutical development concerning diseases predominating in tropical regions: the concept of indigent drugs].

When the WHO certified the eradication of smallpox in 1981, there was a general impression that the fight against infectious diseases which began with Jenner and Pasteur was entering a phase of achievement: poliomyelitis, dracunculasis, leprosy, Chagas' disease and neonatal tetanus were also responding to eradication campaigns. However, in 1995, infectious diseases are still an important cause of mortality and morbidity and the rising incidence of emerging or re-emerging diseases remains a matter of great concern. Although this situation can be explained, at least partly, by the deterioration

Synthesis, characterization and in vitro antitrypanosomal activities of new carboxamides bearing quinoline moiety.

The reported toxicities of current antitrypanosomal drugs and the emergence of drug resistant trypanosomes underscore the need for the development of new antitrypanosomal agents. We report herein the synthesis and antitrypanosomal activity of 24 new amide derivatives of 3-aminoquinoline, bearing substituted benzenesulphonamide. Nine of the new derivatives showed comparable antitrypanosomal activities at IC50 range of 1-6 nM (melarsoprol 5 nM). Compound 11n and 11v are more promising antitrypanosomal agents with IC50 1.0 nM than the rest of the reported derivatives. The novel compounds showed s

Complement activation in patients with Gambian sleeping sickness.

Low levels of complement components C3, C4 and factor B were found in the sera of most patients with early and advanced Gambian trypanosomiasis. C3 levels increased after treatment with melarsoprol (Mel B). Nearly all patients had high serum IgM levels and a significants correlation was found betwen high serum IgM levels and low serum C3 levels. High concentrations of cryglobulins, which contained IgM and C3, were found in most of sera tested. Formation of large molecular weight IgM complexes may play an important part in producing the low C3 levels found in patients with Gambian sleeping sick

African trypanosomiasis in two travelers from the United States.

African trypanosomiasis is a rare but well-documented cause of fever in United States travelers returning from areas where it is endemic. We report two recently diagnosed cases that involved tourists who went on safari in Tanzania. Review of these and 29 other published cases indicates that disease in returning United States travelers is nearly always of the East African form, a fulminant illness for which prompt diagnosis is necessary. In the United States, timely and appropriate therapy for this disease has resulted in favorable outcomes for most patients. Chemoprophylaxis for East African t

Kenyan purple tea anthocyanins and coenzyme-Q10 ameliorate post treatment reactive encephalopathy associated with cerebral human African trypanosomiasis in murine model.

Human African trypanosomiasis (HAT) is a tropical disease caused by two subspecies of Trypanosoma brucei, the East African variant T. b. rhodesiense and the West African variant T. b. gambiense. Melarsoprol, an organic arsenical, is the only drug used to treat late stage T. b. rhodesiense infection. Unfortunately, this drug induces an extremely severe post treatment reactive encephalopathy (PTRE) in up to 10% of treated patients, half of whom die from this complication. A highly reproducible mouse model was adapted to assess the use of Kenyan purple tea anthocyanins and/or coenzyme-Q10 in bloc

The detection and treatment of human African trypanosomiasis.

Human African trypanosomiasis (HAT) is caused by the injection of Trypanosoma brucei (T. b.) gambiense or T. b. rhodesiense by Glossina, the tsetse fly. Three historical eras followed the exclusive clinical approach of the 19th century. At the turn of the century, the "initial research" era was initiated because of the dramatic spread of HAT throughout intertropical Africa, and scientists discovered the agent and its vector. Two entities, recurrent fever and sleeping sickness, were then considered a continuum between hemolymphatic stage 1 and meningoencephalitic stage 2. Treatments were develo

Isolation of Trypanosoma brucei gambiense from cured and relapsed sleeping sickness patients and adaptation to laboratory mice.

BACKGROUND: Sleeping sickness due to Trypanosoma brucei (T.b.) gambiense is still a major public health problem in some central African countries. Historically, relapse rates around 5% have been observed for treatment with melarsoprol, widely used to treat second stage patients. Later, relapse rates of up to 50% have been recorded in some isolated foci in Angola, Sudan, Uganda and Democratic Republic of the Congo (DRC). Previous investigations are not conclusive on whether decreased sensitivity to melarsoprol is responsible for these high relapse rates. Therefore we aimed to establish a parasi

Role of calcium in trypanocidal drug action.

The synergistic effect of serum on the drug combination of salicylhydroxamic acid plus glycerol, which is active against Trypanosoma brucei, is due to diffusible calcium ions. The synergistic activity can be removed by dialysis of the serum or by addition of calcium chelating agents. A buffer containing calcium can mimic the synergistic activity of serum. This finding may have important implications in the clinical management of African trypanosomiasis in humans. Calcium also has a synergistic effect on melarsoprol, the only drug available for treating sleeping sickness patients with central n

[East African sleeping sickness (Trypanosoma rhodesiense infection) in 2 Swiss travelers to the tropics].

We report on two Swiss travellers who acquired African sleeping sickness (Trypanosoma rhodesiense) the same day while visiting the Akagera park in Rwanda. The first patient developed clinical signs of sleeping sickness 8 days after being bitten by a tsetse fly. Trypanosomes were demonstrated in the blood and CSF. The other patient fell ill 13 days after the bite and trypanosomes were found only in blood samples. The first patient (cerebral trypanosomiasis), was treated with melarsoprol. He developed an allergic reaction under treatment but made a good recovery 4 months later. The second patien

[Complement: a sign of infestation of sheep by Trypanosoma brucei brucei in an experimental model: its evolution following treatment].

Broad variations in complement levels are seen during the course of many infectious diseases, particularly human and animal african trypanosomiasis. In this study, sheep complement is measured by means of its haemolytic activity expressed in 50% haemolytic complement units per ml (HCU50/ml). In healthy sheep, the average value is 23.3 HCU50/ml. Complement levels dramatically decrease five days after subcutaneous inoculation of Trypanosoma brucei brucei and remain undetectable until death of non treated infected animals. Eight infected sheep are treated with graded dosages of melarsoprol when c

Hepatic mixed-function oxidase activity in mice infected with Trypanosoma brucei gambiense or treated with trypanocides.

Three days after mice were inoculated with Trypanosoma brucei gambiense, total hepatic cytochrome P-450 levels were decreased 14% from control values, while the specific mixed-function oxidase activity (per nmole of cytochrome P-450) was inhibited almost 40% in infected animals. Furthermore, drugs used to treat trypanosomiasis (Suramin and Melarsoprol B) are themselves potent in vitro inhibitors of hepatic mixed-function oxidase activity. These two trypanocides also produced a decrease in mixed-function oxidations and an increase in pentobarbital sleeping time when administered intraperitoneal

Human sleeping sickness in the Gboko endemic area of Nigeria.

Human infection with Trypanosoma gambiense in the Gboko endemic area was first reported in May, 1974 although T. gambiense sleeping sickness had been present there since the turn of the century. The disease is associated with the presence of the tsetse Glossina tachinoides and Glossina palpalis which is plentiful and widespread throughout the division as well as in thickets along the streams in the area. No successful attempt has been made to control the tsetse vector in the Division. The incidence and geographical distribution of cases of T. gambiense sleeping sickness in the Gboko area are d
