Tibial slope and medial meniscectomy significantly influence short-term knee laxity following ACL reconstruction.

PURPOSE: To determine demographic, anatomic, and surgical factors associated with static and dynamic Anterior Tibial Translation (ATT) following ACL reconstruction. The hypothesis was that both static and dynamic ATT would be greater in knees with high tibial slope or that required meniscectomy. METHODS: The authors prospectively enrolled 280 consecutive patients that had primary ACL reconstruction using hamstring autografts at one center for which preoperative tear type, meniscal tears, and medial tibial slope were documented. A total of 137 were excluded due to concomitant extra-articular t

Arthroscopic partial meniscectomy of a medial meniscus bucket-handle tear using the posteromedial portal.

Arthroscopic resection of irreparable bucket-handle tears of the medial meniscus is a commonly performed procedure. Adequate visualization of the posterior horn of the medial meniscus can be a challenging task with the conventional use of the anterior portal. An attempt to resect the posterior horn in a blind fashion may result in iatrogenic damage of the articular cartilage in the posterior compartment, over-resection of a remnant meniscus, or an insufficient resection of the torn fragment. We describe the use of the posteromedial portal for an accurate visualization and resection of the post

Early and intensive physiotherapy accelerates recovery postarthroscopic meniscectomy: results of a randomized controlled study.

The efficacy of an early, intensive, supervised rehabilitation program to accelerate knee strength recovery in the first 3 weeks postmeniscectomy by arthroscopy was evaluated using a randomized controlled trial design. The maximal voluntary isokinetic strength of 31 men, randomly allocated to either a treatment (EXP) or a control (CTL) group, was measured twice by a blind rater: preoperatively (pretest) and 3 weeks postsurgery (posttest), using a computer-controlled Kin-Com dynamometer (Chattecx Corporation, Chattanooga, TN). Strength deficits of the operated leg at the pretest and posttest we

Pes anserinus transplantation without a routine medial meniscectomy: a review of twenty-two cases.

Of 549 pes anserinus transplantations performed at the National Athletic Health Institute, Inglewood, California from 1971 through 1977, only 27 involved the transfer for anteromedial rotational instability in the presence of an intact medial meniscus. The 22 patients reviewed were evaluated with a questionnaire, physical and roentgenogram examinations, and hamstring and quadriceps muscle testing with Cybex II isokinetic dynamometer. Average length of follow-up was one year, four months. Acute anteromedial rotational instability was seen in 16 patients while six had chronic instability. All ha

Quality of recovery after arthroscopic meniscectomy with remimazolam or propofol anesthesia: A randomized controlled trial.

BACKGROUND: Anesthetic technique may influence postoperative quality of recovery (QoR). Remimazolam may offer hemodynamic stability and favorable early recovery characteristics, but its effect on patient-reported QoR compared with propofol remains unclear. This is particularly relevant in minor orthopedic surgery, where QoR is a principal outcome. This randomized controlled trial compared postoperative QoR between propofol-based and remimazolam-based total intravenous anesthesia (TIVA) using the QoR-40 questionnaire. MATERIALS AND METHODS: Eighty adult patients undergoing elective arthroscopi

Association of Quadriceps Strength and Psychosocial Factors With Single-Leg Hop Performance in Patients With Meniscectomy.

BACKGROUND: Clinicians use the single-leg hop test to assess readiness for return to sports after knee injury. Few studies have reported the results of single-leg hop testing after meniscectomy. Additionally, the contributions of impairments in quadriceps strength and psychosocial factors to single-leg hop performance are unknown. PURPOSE: To compare single-leg hop performance (distance and landing mechanics) between limbs and to examine the association of single-leg hop performance with quadriceps strength and psychosocial factors in patients with meniscectomy. STUDY DESIGN: Descriptive lab

Post-operative deformation and extrusion of the discoid lateral meniscus following a partial meniscectomy with repair.

PURPOSE: The purpose of this study was to investigate the morphology of the discoid lateral meniscus sequentially following a partial meniscectomy with repair using magnetic resonance imaging (MRI). METHODS: Nine patients with a symptomatic discoid lateral meniscus with a peripheral tear were enrolled in this study, and a partial meniscectomy with repair was performed arthroscopically. An MRI examination was performed 2 weeks after surgery (before weight bearing was permitted) and again 6 months after surgery (when sporting activities could resume). The width, height and distance of the disco

Dynamic contact mechanics of the medial meniscus as a function of radial tear, repair, and partial meniscectomy.

BACKGROUND: The menisci are integral to normal knee function. The purpose of this study was to measure the contact pressures transmitted to the medial tibial plateau under physiological loads as a function of the percentage of the meniscus involved by the radial tear or repair. Our hypotheses were that (1) there is a threshold size of radial tears above which contact mechanics are adversely affected, and (2) partial meniscectomy results in increased contact pressure compared with that found after meniscal repair. METHODS: A knee simulator was used to apply physiological multidirectional dynam

Delayed Presentation of Popliteal Artery Laceration Mimicking Baker Cyst After Arthroscopic Meniscectomy: A Case Report.

CASE: We report a case of delayed presentation of popliteal artery laceration after arthroscopic meniscectomy. Three weeks after the procedure, the patient developed severe calf pain, swelling over the popliteal fossa, and foot drop, despite a normal initial postoperative period. Clinical examination and radiological investigations suggested a massive hematoma in the popliteal fossa. A decision was made to explore the popliteal artery and drain the hematoma after consultation with the vascular surgeon. A laceration was noted, and it was repaired. CONCLUSION: Orthopaedic surgeons should mainta

Predictors of Successful Treatment 1 Year After Arthroscopic Partial Meniscectomy: Data from the OME Cohort.

UNLABELLED: Arthroscopic partial meniscectomy (APM) is one of the most common orthopaedic procedures. Understanding factors that predict better patient-reported outcomes is important for guiding patient and clinician decision-making. The purpose of this study was to evaluate predictors of pain and function after APM in a large, multisite, academic health system cohort. METHODS: We prospectively enrolled 665 patients who were ≥40 years of age and who had APM without any concomitant ligament or cartilage-resurfacing procedures. There were 486 subjects (73%) who completed baseline and follow-up

Does concomitant meniscectomy or meniscal repair affect the recovery of quadriceps function post-ACL reconstruction?

PURPOSE: The purpose of this study was to determine the effect of concomitant meniscal surgery on the recovery of quadriceps activation and strength at a time when individuals return to sport following anterior cruciate ligament (ACL) reconstruction. METHODS: Forty-six individuals that were cleared for participation following ACL reconstruction were invited to participate in this study. Participants were placed into groups according to surgical reports (ACL-only, n = 24; meniscal repair, n = 12; meniscectomy, n = 10). Quadriceps strength was quantified using isokinetic and isometric measures.

Disability, impairment, and physical therapy utilization after arthroscopic partial meniscectomy in patients receiving workers' compensation.

BACKGROUND: The treatment of meniscal tears in injured workers is associated with less favorable outcomes and higher utilization of clinical services. It was hypothesized that patients receiving Workers' Compensation who undergo arthroscopic meniscectomy can have excellent outcomes with physical therapy utilization below the national best-practices benchmarks. METHODS: The records of 155 injured workers who had undergone 164 primary arthroscopic meniscectomies were reviewed at least one year following claim closure.
