Symptomatic torn discoid lateral meniscus in adults.

The purposes of this study were to report degenerative changes that coexist with a symptomatic torn discoid lateral meniscus in adults and to analyze the factors associated with the accompanied degenerative changes. From 1997 to 2008, 329 knees in the 305 patients were included. Associations between the status of the meniscus and the coexisting degenerative changes on the images and the arthroscopic findings were statistically analyzed. Marginal osteophyte was seen on conventional radiography in 118 patients (36%). Ninety patients (27%) had arthroscopically confirmed chondral lesion. Age, dura

Meniscus preservation; rationale, repair techniques and results.

It is inescapable that loss of part or all of the structure of the meniscus of the knee alters the biomechanics of knee function leading to a gradual deterioration of the chondral surface, which ultimately will result in osteoarthritis. The goal of the knee surgeon should therefore be to preserve as much as possible the function of the meniscus. Classic teaching is that many meniscal tears, which are peripheral and longtitudinal, will heal if sutured and protected during a slow healing process. However it is clear that other tear configurations can heal if given the opportunity by adequate pre

Open meniscus repair. Indications, technique, and results.

As noted through the work of Fairbank, total meniscectomy is not a benign operation. Cox et al found that meniscectomies in canine knees lead to gross and microscopic degenerative changes. They also noted that partial meniscectomies lead to less severe degenerative changes. They believed that there was a direct relationship between the degree of degenerative change and the amount of meniscus removed. McGinty et al reported early return to function with decreased morbidity and decreased complication rates after partial meniscectomy; this was compared to both open and arthroscopic techniques for

A new arthroscopic classification of degenerative medial meniscus root tear that correlates with meniscus extrusion on magnetic resonance imaging.

BACKGROUND: To determine a new classification system for medial meniscus root tears (MMRT) based on arthroscopic findings. METHODS: 24 knees (55%) belonged to the nondisplaced or overlapped group, and 20 knees (45%) to the widely displaced group. Absolute meniscal extrusion was defined as distance between outer edge of the articular cartilage of tibial plateau and meniscal outer edge. Relative extrusion was defined as extruded meniscus width divided by entire meniscal width, multiplied by 100. The proportion of knees with major (>3 mm) extrusion were compared in two groups, as were the severi

New meniscus repair technique for peripheral tears near the posterior tibial attachment of the posterior horn of the medial meniscus.

We introduce a suture technique to repair a peripheral tear near the posterior tibial attachment of the posterior horn. A suture hook was inserted through the posteromedial portal, and the peripheral capsular rim was penetrated from superior to inferior by the sharp hook. Both relay limbs were brought out through the posteromedial portal. The outer limb of the superior peripheral capsular rim was identified with a hemostat. An 18-gauge spinal needle loaded with a No. 0 polydioxanone suture (PDS) was introduced into the joint from the anteromedial portal; it was passed through the joint space u

Discoid LaterL Meniscus: A Concise Literature Update.

Discoid lateral meniscus (DLM) is the most frequent congenital variant of the lateral meniscus, which is prone to degeneration and tears, and frequently causes knee osteoarthritis. The purpose of this article has been to analyze the publications made during 2023 on DLM. The main conclusions of the analysis were as follows: MRI assessment might be helpful to diagnose DLM and detect the presence of instability: two main factors in the decision to perform surgery. Arthroscopic assessment should be utilized in conjunction with MRI findings for complete DLM diagnosis. Restoring the normal shape, re

The Lateral Meniscus Oblique Radial Tear (LMORT).

PURPOSE OF REVIEW: The lateral meniscus oblique radial tear (LMORT) of the posterior horn is a relatively new term for one of the more common types of lateral meniscal tears found in patients with acute anterior cruciate ligament (ACL) injuries. Given the importance of anatomical reduction and preservation of the lateral meniscus to maintain normal knee stability and slow the progression of early-onset osteoarthritis, LMORT classification and treatment guidelines have been formulated. This article provides a review of the prevalence, classification, biomechanics, surgical repair techniques, an

Arthroscopic Meniscus Ramp Repair: The Shoelace Technique.

BACKGROUND: A "ramp lesion" is described as an injury involving the peripheral attachment of the posterior horn of the medial meniscus. Ramp lesions are associated with increased loads on anterior cruciate ligament and leads to rotatory instability of knee. During anterior cruciate ligament reconstruction, failure to identify and treat ramp lesion leads to increased forces on the reconstructed graft and residual instability which ultimately increases chances for graft failure. It is important to identify the ramp lesions by looking at the posterior compartment and repair ramp lesion. Ramp lesi

Bilateral discoid medial Meniscus: Two case reports.

RATIONALE: Discoid meniscus is an anatomical congenital anomaly more often found in the lateral meniscus. A discoid medial meniscus is a very rare anomaly, and it is difficult to diagnose a bilateral discoid medial meniscus because not all the discoid medial menisci are symptomatic. Herein, we report 2 cases of bilateral discoid medial meniscus. PATIENTS CONCERNS: Case1: a 23-year-old man presented with complaints of pain and occasional clicking of the right knee for 6 years, the left knee was asymptomatic. Case 2: a 51-year-old woman had a 1-month history of pain and a feeling of "giving awa

Optical spectroscopic determination of human meniscus composition.

This study investigates the correlation between the composition of human meniscus and its absorption spectrum in the visible (VIS) and near infrared (NIR) spectral range. Meniscus samples (n = 24) were obtained from nonarthritic knees of human cadavers with no history of joint diseases. Specimens (n = 72) were obtained from three distinct sections of the meniscus, namely; anterior, center, posterior. Absorption spectra were acquired from each specimen in the VIS and NIR spectral range (400-1,100 nm). Following spectroscopic probing, the specimens were subjected to biochemical analyses to deter

Discoid meniscus presenting as juvenile rheumatoid arthritis.

Discoid meniscus is a mechanical lesion described as having little inflammatory reaction. We describe a child with lateral discoid meniscus misdiagnosed as juvenile rheumatoid arthritis (JRA). Synovitis responded to aspirin, but function gradually deteriorated with increasing flexion deformity until arthrotomy and meniscectomy. Histology showed intense inflammatory changes compatible with JRA. Clinical and laboratory clues to early diagnosis were the localized nature of the inflammation on physical examination, radiographs and bone scan.

Characteristics of Posterior Root Tears of the Lateral Meniscus.

Lateral meniscus posterior root tears are increasingly recognized as clinically important injuries that can compromise meniscal function and alter knee joint biomechanics, particularly in the setting of anterior cruciate ligament (ACL) injury. This review synthesizes and critically evaluates the current literature on the anatomy, biomechanics, classification, epidemiology, diagnosis, and management of lateral meniscus posterior root tears, with consideration of similarities and differences compared with medial meniscus posterior root tears. Particular attention is given to the anatomical relat

Micromechanical properties of the healthy canine medial meniscus.

AIMS: Knowledge of the micromechanical characteristics of the menisci is required to better understand their role within the stifle joint, improve early diagnosis of meniscal lesions, and develop new treatment and/or replacement strategies. The aim of the study was to determine the mechanical properties of the healthy medial canine meniscus and to evaluate the effect of regional (caudal, central, and cranial) and circumference (axial and abaxial) locations on these properties. METHODS: To study the micromechanical properties of the medial menisci in healthy (Beagle) dogs, the influence of reg

Discoid meniscus in human fetuses: A systematic review.

BACKGROUND: Discoid meniscus (DM) is a rare variant of regular knee anatomy. Compared to standard meniscus it is thicker and abnormal in shape; these characteristics make it more prone to tear. It is a congenital defect whose correct etiology is still debated and far from being clarified. The purpose of this systematic review is to evaluate evidences of DM in human fetuses in order to assess whether embryological development may have a role. METHODS: A systematic review was performed on PubMed, Scopus, and Embase with different combinations of the keywords "discoid meniscus", "embryology", "f

The Meniscus Tear: A Review of Stem Cell Therapies.

Meniscal injuries have posed a challenging problem for many years, especially considering that historically the meniscus was considered to be a structure with no important role in the knee joint. This led to earlier treatments aiming at the removal of the entire structure in a procedure known as a meniscectomy. However, with the current understanding of the function and roles of the meniscus, meniscectomy has been identified to accelerate joint degradation significantly and is no longer a preferred treatment option in meniscal tears. Current therapies are now focused to regenerate, repair, or
