Radiographic Comparison of Open and Minimally Invasive Distal Chevron Metatarsal Osteotomy in Patients With Hallux Valgus.

This study aimed to compare radiographic outcomes of open and minimally invasive surgery (MIS) in patients with hallux valgus. We reviewed data of patients with hallux valgus who underwent open or minimally invasive distal chevron osteotomy at our institution. Radiographic assessment was completed preoperatively, immediate postoperatively, and one year postoperatively using eight weight bearing parameters . The classic distal chevron osteotomy method was used for open surgery and a modified method that added percutaneous K-wire fixation to the minimal invasive Chevron-Akin (third-generation MI

Intra‑ and Interobserver Reliability of Yamaguchi's Method for the Assessment of First Metatarsal Pronation in Hallux Valgus Deformity.

Pronation of the first metatarsal is a risk factor for the formation and progression of the hallux valgus deformity. Recently, Yamaguchi et al published a study that showed how the round sign increases on digitally reconstructed radiography taken from a CT scan when pronation is applied. In this study, the shape of the lateral edge of the first metatarsal head was evaluated on weightbearing dorsoplantar radiographs. Yamaguchi's signs were presented to observers after the visual presentation of each foot as an image. The best-fit circle was drawn using the PACS drawing function. Ninety radiogra

How I Use a Three-Dimensional Approach to Correct Hallux Valgus with a Distal Metatarsal Osteotomy.

The role of uniplanar osteotomy in correction of multiplanar deformity in hallux valgus is a developing and promising concept. Careful consideration should be given to the literature when considering the concept of preexisting pronation of the metatarsal. Recent weight-bearing computed tomography studies contradict the existing literature and state that there is little or no rotation of the metatarsal; therefore, a multiplanar osteotomy should aim to correct the rotation caused by soft tissue imbalance at the tarsometatarsal and metatarsophalangeal joints rather than in the metatarsal itself.

[Intermediate results following subcapital metatarsal osteotomy in the therapy of hallux valgus with metatarsus primus varus].

71 feet were investigated after subcapital modified osteotomy of the metatarsus according to Kramer for an average follow-up period of 19 months. Mean improvement of the Hallux valgus angle was 10 degrees whereas the intermetatarsal angle was improved by 4 degrees on average. The high degree of satisfaction of patients (83%) with the outcome of this surgical method suggest its application in cases of medium metatarsus primus varus within hallux valgus deformations when the metatarso-phalangeal joint is not affected. In patients aged 50 years and older as well as in cases of MT I varus exceedin

Axial wedge osteotomy of the first metatarsal head (an adjunctive procedure for correction of hallux abducto valgus).

The authors have introduced an adjunctive procedure for correction of the osseous axial eversion of the first ray with hallux abducto valgus deformity. They believe that recurrence of bunion deformity is reduced with correction of this osseous abnormality. Consecutive illustrations are presented with recognition of the individual surgeon's ability to modify the procedure to allow for correction of deformities associated with hallux abducto valgus.

Incidence and Risk Factors for Short-Term Reoperations After Open First-Metatarsal Osteotomy for Hallux Valgus.

BACKGROUND: Despite the satisfactory results of hallux valgus surgery in general, the incidence of complications has been considerably high. This study evaluated the incidence and risk factors of short-term reoperations after surgical correction of hallux valgus deformity with first metatarsal osteotomy. METHODS: We conducted a retrospective review of 685 consecutive open first-metatarsal osteotomies performed from 2013 to 2018 in a single university-hospital region. The number, indications, and type of reoperation were collected. The association between reoperation and patient's age, sex, BM

A Refined Minimally Invasive Distal First Metatarsal Osteotomy for Moderate Hallux Valgus Treatment: The BC Procedure.

During the last 2 decades, there was an increasing interest in mini-invasive procedures for hallux valgus correction. In this scenario the Bösch technique appears to be a reproducible distal metatarsal osteotomy (DMO) to achieve a proper correction. Our DMO variant, called BC, was planned to combine the stability and predictability of the chevron osteotomy, with the power of correction, low surgical time and mini-invasive approach of the Bösch-SERI technique. The purpose of this investigation is to describe the surgical technique and report the results of this modified procedure at a minimum 2

The role of weightbearing computed tomography in assessing first metatarsal pronation in hallux valgus: A systematic review.

BACKGROUND: Hallux valgus is a multiplanar deformity often involving first metatarsal pronation. Weightbearing computed tomography may improve assessment compared to standard radiographs, which are limited to two-dimensional views. METHODS: A systematic review was conducted according to PRISMA guidelines. PubMed and Cochrane databases were searched for studies evaluating weightbearing computed tomography in assessing HV components, including metatarsal pronation and sesamoid position. Study quality was assessed using the Newcastle-Ottawa Scale. RESULTS: Fourteen studies (706 feet) were inclu

Analysis of hypermobility of the first metatarsal in hallux valgus deformity: A weight bearing computed tomography study.

BACKGROUND: The hypermobility of the first ray has been investigated as a possible contributing factor to hallux valgus, but its precise relationship with the condition has not yet been fully clarified. Weight-bearing computed tomography allows us to assess the foot in a three-dimensional and loaded manner, helping us better understand hypermobility. This study aims to evaluate the hypermobility in the sagittal plane of the first metatarsal in individuals with hallux valgus and hypermobility using weight-bearing computed tomography. METHODS: In this prospective study patients with hallux valg

3D Analysis of Metatarsal Torsion by Computed Tomography in Normal, Hallux Valgus, and Hallux Rigidus Feet.

BACKGROUND: One factor contributing to rotational deformity of the first metatarsal in hallux valgus is torsion of the metatarsal itself. Hallux rigidus also involves reduction of the longitudinal arch, but metatarsal torsion has not been discussed. We hypothesized that metatarsal torsion may be a morphologic change unique to hallux valgus. We compared 3-dimensional (3D) torsion of the first to fifth metatarsals between feet with hallux valgus, feet with hallux rigidus, and healthy control feet to investigate differences in the effects on pathologic conditions. METHODS: Participants were wome

Association of First Metatarsal Pronation Correction With Patient-Reported Outcomes and Recurrence Rates in Hallux Valgus.

BACKGROUND: The purpose of this study was to determine if a postoperative decrease in first metatarsal pronation on 3-dimensional imaging was associated with changes in patient-reported outcomes as measured by the Patient-Reported Outcomes Measurement Information System (PROMIS) physical function, pain interference, and pain intensity domains or recurrence rates in patients with hallux valgus (HV) who undergo a first tarsometatarsal fusion (modified Lapidus procedure). METHODS: Thirty-nine consecutive HV patients who met the inclusion criteria and underwent a modified Lapidus procedure had pr

Comparison of contact surface areas of metatarsal diaphyseal osteotomies for correction of hallux valgus: Experimental study.

OBJECTIVE: This study aimed to determine, pre-correction, the potential change in the osteotomy-site bony contact surface area that would occur during standard metatarsal diaphyseal procedures with the Baran-Unal modification of Mau osteotomy and then to compare it, post-correction, with the actual osteotomy-site bony contact surface area changes for a standard degree of deformity correction. METHODS: A total of 30 standard, same sized, biomechanically equivalent, left first metatarsal sawbones were included in this experimental study. They were divided equally into five groups for each of th

Correction of moderate and severe hallux valgus deformity with a distal metatarsal osteotomy using an intramedullary plate.

More than 200 surgeries have been described for hallux valgus correction. The distal V-shape chevron-type osteotomy was described with no more than 50% translation of the distal fragment in relation to the metatarsal shaft. This concept of high, powerful correction for distal chevron osteotomy fixed by the Mini MaxLock Extreme ISO gives the surgeon the possibility of a mini-invasive rapid solution. A new technique of fixation is based on the Murawski and Beskin concept, because a powerful correction can be performed with a minimally invasive approach, but, in contrast to other studies, with st

Treatment of hallux valgus with increased distal metatarsal articular angle: use of double and triple osteotomies.

The treatment of the congruent hallux valgus deformity requires special consideration for a successful outcome to be obtained. The distal metatarsal articular angle is of critical importance in this deformity. The goal of correction is to achieve a realigned first ray and preserve the congruent first metatarsophalangeal articulation. In patients with an increased distal metatarsal articular angle and congruent joint, the use of double and triple first ray osteotomies must be used to achieve satisfactory correction.

The Mayday distal first metatarsal osteotomy for hallux valgus: a review after the introduction of a new instrument.

We present the short-term follow-up of 55 symptomatic hallux valgus deformities in 38 patients, treated operatively with a modification of the spike distal first metatarsal osteotomy, as described by Gibson and Piggott in 1962. The age range of the patients was 17 to 72 years at the time of surgery. The postoperative follow-up period was 12 to 55 months. Excellent and good clinical and radiographic results were recorded in 96.2% of our patients. Two of the patients (3.8%) were dissatisfied; one of them complained of metatarsalgia after the procedure, and the other had stiffness of the metatars
