Pediatric Distal Tibia Physeal Injury Presenting with Acute In-toeing: A Case Report and Literature Review.

Ankle fractures are among the most common fractures sustained in the pediatric population. Given the frequency of physeal involvement of the distal fragment, complications including growth arrest, overgrowth, and rotational deformities are not uncommon. This case report describes a 12-year-old adolescent boy who presented after an acute right ankle injury sustained while playing. He noted right ankle pain, swelling, and in-toeing of his foot. Radiographs of the ankle demonstrated a distal tibia Salter-Harris type II fracture that appeared nondisplaced. However, a CT scan of the ankle demonstra

Distal metatarsal osteotomy for adolescent hallux valgus.

This is a review of 55 metatarsal osteotomies for hallux valgus and metatarsus primus varus in patients under 15 years of age. The results were analysed after the patients were reviewed clinically and radiologically. Complications of the osteotomies and the factors that caused them are discussed. Poor results, irrespective of the type of osteotomy, were due to excessive shortening of the first metatarsal and/or dorsal tilting of the metatarsal head. In both instances there was persistent metatarsalgia.

Metatarsus adductus setting in adult patients: Results of a treatment algorithm with shortening arthrodesis tarsometatarsal joints two and three.

BACKGROUND: This study proposes a treatment algorithm based on the correction of symptomatic metatarsus adductus (MA) associated with hallux abducto-valgo (HAV) through tarsometatarsal joint 2-3 (TMTJ) shortening arthrodesis. Our hypothesis is that the proposed algorithm leads to realignment of the forefoot rays from two to five and reduction of the talonavicular coverage angle (TNCA) with good clinical and radiographic results. METHODS: Clinical and radiographic evaluations were performed before and after surgery at 1-year follow-up on forty-six consecutive adult patients with MA, midfoot pa

Outcomes of isolated soft tissue surgery for in-toeing gait in patients with ambulatory cerebral palsy.

BACKGROUND: Soft tissue surgery to address in-toeing gait in young cerebral palsy (CP) patients may be an alternative in some cases to femoral derotation osteotomy (FDO), which is the currently accepted treatment. The relative contribution of muscular contracture, spasticity and bone deformity is still controversial. In this study, we determined the outcomes of soft tissue surgery on hip internal rotation (HIR) when femoral anteversion was less than 45° and the soft tissues were identified as being the cause. METHODS: This prospective study included select adolescent patients who were operate

Clarification of the anatomic definition of the bunion deformity.

Evolution of the terminology applied to the bunion deformity has progressed in parallel with our changing understanding of the deformity itself. Along this path of progression have been multiple terms, sometimes with multiple meanings. Hallux valgus and metatarsus primus varus are 2 of the most common terms for the deformity. Although commonly used, these descriptors can have multiple meanings, and inconsistencies in interpretation can lead to confusion. We propose a more detailed terminology to provide a more accurate description of the entire bunion deformity in 3 planes and for both the hal

A RADIOGRAPHIC ANALYSIS OF MAJOR FOOT DEFORMITIES.

Major foot deformities were analyzed using standardized radiographic drawings of the foot in weight-bearing. Specific criteria and a classification of foot deformities by radiographic analysis are presented, utilizing "lining systems" related to the main bones of the hindfoot. The radiographic appearance of the foot is described and analyzed, as are the principal deformities such as varus and valgus heel and forefoot. The prefixes "talipes" and "pes" have been used to signify congenital and acquired deformities, respectively.Specific foot deformities, including talipes equinovarus (clubfoot), p

A retrospective study of 63 hallux valgus corrections using the osteodesis procedure.

Osteotomy procedures have been the most popular approach to hallux valgus deformity correction. Soft tissue approaches have, in general, been regarded as ineffective for moderate and severe hallux valgus deformities. Osteodesis is a soft tissue technique that has been shown to be effective in the past but is still seldom practiced. In the present report, we describe a retrospective study of 63 hallux valgus feet in 36 patients who had undergone the osteodesis procedure. Their mean age was 46 ± 12 years, and the mean follow-up period was 25.4 ± 9.6 months. The surgical technique consisted of me

Novel device for nonsurgical correction of rigid forefoot adduction in children.

Forefoot adduction deformity (FAD) (commonly called metatarsus adductus) is reported as the most common congenital foot deformity in newborns. Early diagnosis and treatment are important in rigid cases, as better outcomes have been reported if treatment was initiated before 9 months of age. While casting and splinting is the current standard of care for nonsurgical management of rigid FAD (RFAD), several orthoses have demonstrated equal benefit. The Universal Neonatal Foot Orthotic (UNFO) brace is below ankle orthosis that provides continuous pressure, thereby correcting the deformity without

The Windswept Foot: Dealing with Metatarsus Adductus and Toe Valgus.

The windswept foot remains a reconstructive challenge. The hallux valgus associated with the medially displaced lesser metatarsal heads is hard to correct. Either the lesser metatarsal heads need to be displaced laterally or the deformity accepted. With the deformity, all the toes tend to be aligned into valgus with the position of the flexor and extensor tendons. Several treatment alternatives exist and may require a combination of open and percutaneous surgery. The authors think that, in severe metatarsus adductus, proximal correction of the first, second, and third metatarsals is required.

[The etiology of hallux valgus].

Modifications of the forefoot in weightbearing are conductive to hallux valgus, some are congenital and constitutional: supination of the forefoot, adductus of the metatarsal bones, metatarsus varus primus brevis. Others are acquired: transversal flatfoot. Measurement of the different angles observed on a dorsi-plantar X-ray in relation to the external and internal limits of the Lisfranc joint has an etiological signification. To look for changes in the weightbearing configuration of the foot, an element in the formation of hallux valgus, is of use in the choice of a method of correction.

Correlation Between Hallux Valgus Severity and the Prevalence of Metatarsus Adductus in Hallux Valgus.

INTRODUCTION: Hallux valgus (HV) is a common foot deformity, with metatarsus adductus (MA) identified as a potential predisposing factor. MA has been shown to negatively affect surgical outcomes for HV, particularly in severe cases. This study aims to clarify the prevalence of MA in the HV population using different metatarsus adductus angle (MAA) measurement methods and assess whether MAAs are influenced by HV severity. MATERIALS AND METHODS: This retrospective study included 294 feet from 147 participants. Patients were classified into non-HV (normal) and HV subgroups, with HV severity grad

Orthopedic disorders in patients with Down's syndrome.

We have contrasted 161 patients with Down's syndrome admitted to three acute care centers with 107 patients in an institutional setting, outlining their major orthopedic problems and suggesting methods of management based on acute care experience. Severe hallux valgus, metatarsus primus varus, subluxation an dislocation of the patella, severe pes planus, atlantoaxial subluxation, femoral head epiphysiolysis, and dislocation or subluxation of the hips remain the most critical problems against a background of other less common orthopedic abnormalities.

The intoeing child: etiology, prognosis, and current treatment options.

Intoeing, a common entrance complaint in infants, toddlers, and young children, is best defined as internal rotation of the long axis of the foot to the line of progression. Intoeing may be caused by primary deformities within the foot, issues with tibial torsion, and femoral antetorsion (anteversion). Problems within the foot include hallux varus, metatarsus adductus, talipes equinovarus, and pes cavus, each of which has specific treatments available. Treatment must be individualized, and the risks and complications weighed against the predictable morbidity of intoeing.

Normal and abnormal function of the first ray.

The first ray is the most important structure of the forefoot in its contribution to normal locomotion. Because first ray dysfunction is encountered in clinical practice with the development of hallux valgus, metatarsus primus varus, and hallux rigidus, there has been a belief that there is a mechanical basis for these conditions. Since publications in the 1930s, there has been significant research focused on the first ray. This article discusses the subtleties of normal and abnormal mechanics of the first ray to promote a better understanding for foot and ankle practitioners when treating the

'Doctor, my son walks funny': a guide for the perplexed.

The primary care physician plays a critical role in reassuring parents about their child's orthopedic development in the lower extremity. Paramount to this reassurance is the ability to understand the normal and natural history of lower extremity development. Family physicians should be able to diagnose and treat metatarsus varus, internal and external tibial torsion, femoral anteversion and femoral retroversion, bow legs, knock knees, and flat feet. The chief complaint often turns out to be only a variation of normal physiological growth and development.

Surgical treatment of diaphyseal stress fractures of the fifth metatarsal in competitive athletes: long-term follow-up and computerized pedobarographic analysis.

BACKGROUND: Proximal diaphyseal stress fractures of the fifth metatarsal are common in athletes. Conservative treatment has been shown to result in high rates of delayed union, nonunion, and refracture, so internal fixation has become the treatment of choice in competitive athletes. METHODS: Twenty top-level athletes with diaphyseal stress fractures fixed with intramedullary malleolar screws were evaluated. Functional outcome was assessed by American Orthopaedic Foot and Ankle Society midfoot score. Static and dynamic maximum vertical force and peak plantar pressures were evaluated with a com

Relation between hallux valgus deformity and lumbar and lower extremity biomechanics.

Hallux valgus (HV) is a static subluxation of the first metatarsophalangeal joint with lateral deviation of the great toe and medial deviation of the first metatarsal. Our aim was to evaluate the interference of HV deformity with changes in lumbar lordosis and some foot deformities. Twenty HV patients and 20 healthy subjects were selected for the study. Hallux valgus angle, metatarsus primus varus (MPV), metatarsus omnis varus (MOV), first intermetatarsal (1.IMT), second metatarsal (2. MT) and hallux interphalangeal (HIP) angles were estimated from the anteroposterior foot X-rays. Lumbosacral

[Metatarsal osteotomies for hallux valgus].

In the surgical treatment of hallux valgus, proximal, diaphyseal, and distal osteotomies of the first metatarsal bone are commonly used. In an overview article different procedures are described as well as our own stage-adapted concept. Deformities with congruent articulation of the metatarsophalangeal joint are successfully treated with a distal chevron osteotomy. In an incongruent joint a distal soft tissue procedure is required for reduction of the joint. The metatarsus varus displacement is corrected with a proximal osteotomy of the first metatarsal bone. The indications and details of the

[Congenital foot abnormalities].

The foot may be the site of birth defects. These abnormalities are sometimes suspected prenatally. Final diagnosis depends on clinical examination at birth. These deformations can be simple malpositions: metatarsus adductus, talipes calcaneovalgus and pes supinatus. The prognosis is excellent spontaneously or with a simple orthopedic treatment. Surgery remains outstanding. The use of a pediatric orthopedist will be considered if malposition does not relax after several weeks. Malformations (clubfoot, vertical talus and skew foot) require specialized care early. Clubfoot is characterized by an
