General anaesthetics and the acetylcholine-sensitivity of cortical neurons.

1The effects of general anaesthetics on neuronal responses to iontophoretically-applied acetylcholine have been examined in slices of guinea-pig olfactory cortex maintained in vitro. 2 Acetylcholine excited 61% of the prepiriform neurones tested. The excitation was blocked by atropine, but not by dihydro-beta-erythroidine or gallamine. 3 Alphaxalone reversibly depressed the acetylcholine-sensitivity of prepiriform neurones. Pentobarbitone did not consistently depress the acetylcholine sensitivity of these cells. 4 Ether, methoxyflurane, trichloroethylene and halothane caused a dose-related aug

Effects of anesthetics on the kidney.

All general anesthetics employed clinically have significant and reversible effects on renal hemodynamics, water, and electrolyte excretion at concentrations in which they are usually employed for surgical anesthesia. Some of the mechanisms by which these effects are produced are discussed. The minmal depressant effect on renal function of epidural anesthesia is possibly related to local anesthetic effects on the central system and/or the kidney. Methoxyflurane has been demonstrated to produce renal dysfunction when administered in high concentrations for prolonged periods of time to obese pat

Effects of several volatile anesthetics on the Ca(2+)-related functions of skinned skeletal muscle fibers from the guinea pig.

The effects of various volatile anesthetics on intramuscular Ca(2+)-related functions were studied with the skinned fiber technique in guinea pig skeletal muscles. All the volatile anesthetics tested significantly enhanced Ca(2+)-induced Ca2+ release (CICR) from the sarcoplasmic reticulum (SR) at clinical concentrations with negligible effects both on Ca2+ sensitivity of the contractile system and on Ca2+ uptake by the SR. A comparison was made of the enhancing effect of several volatile anesthetics on CICR at clinical concentrations. Halothane was the most potent, followed by methoxyflurane,

Electroretinogram as a possible monitor of anesthetic depth.

The effects of volatile anesthetics, i.e., methoxyflurane, halothane and enflurane, on the electroretinogram (ERG) were studied in 15 albino rabbits. The ERG was analyzed in terms of the a-wave, and the first oscillatory component (01) in the b-wave. The 01 peak latency showed a significant dose-related prolongation when anesthetic end-tidal concentrations were in excess of 0.8 minimum alveolar concentration (MAC). One MAC, a measure of anesthetic potency, is the end-tidal concentration of an anesthetic at 1 atmosphere that induces immobility in 50% of animals against a noxious stimulus. The a

Evaluation of the impact of high altitude on pre-hospital anaesthetic administration: a scoping review.

BACKGROUND: Increasing high-altitude travel and habitation drive demand for care in these environments. High altitude presents unique challenges to anaesthetic administration as hypobaric hypoxia affects physiology, pharmacology, and equipment performance, yet no guidelines exist. This scoping review aimed to identify anaesthetics used at HA, evaluate the impact of high altitude on anaesthetic use, and highlight research gaps. METHODS: This review followed Arksey and O'Malley's methodological framework and is reported according to the Preferred Reporting Items for Systematic Reviews and Meta-

General and local anaesthetics perturb the fusion of phospholipid vesicles.

The effects of general and local anaesthetics on Ca2+-induced fusion of negatively charged lipid vesicles have been investigated. Vesicles composed of phosphatidylcholine and phosphatidic acid (2:1 molar ratio) were induced to fuse using 5 mM free Ca2+. Fusion, assessed by an increase in size using gel filtration techniques and confirmed by electron microscopy, displayed a dependence on Ca2+ and Mg2+ concentration and on temperature. The inhalational anaesthetics halothane, methoxyflurane and diethyl ether enhanced fusion as did the uncharged local anaesthetic benzocaine. In contrast, the char

A study of the mutagenicity of anesthetics and their metabolites.

The commonly used volatile anesthetics, several of their metabolites, and drugs frequently employed by the anesthesiologist were screened for mutagenicity in the Salmonella/rat-liver microsomal assay system developed by Dr. B. Ames and his colleagues. Chloral hydrate, both a sedative and metabolite of trichloroethylene, was found to be weakly mutagenic. Other compounds testing including halothane, isoflurane, methoxyflurane, diazepam and chlordiazepoxide were not mutagenic. Non-volatile compounds were tested for their ability to inhibit growth of bacterial strains with decreased capacity to re

Influence of anesthetics, ethyl alcohol, and Freon on dark adaptation of monkey cone ERG.

Cone dark adaptation curves were measured in a rhesus monkey using the electroretinogram (ERG) response to a 40 Hz flickering stimulus. The influence of anesthetics on the time course of dark adaptation was studied. All volatile anesthetics tested (methoxyflurane, halothane, enflurane, ether, chloroform) retarded dark adaptation but to different degrees; urethane, ethyl alcohol, and Freon 11 also retarded dark adaptation. No effect was found for barbiturates and ketamine. It seems unlikely that metabolites play a role in the observed phenomena. A literature survey reveals that several studies

Morphologic changes in mouse spermatozoa after exposure to inhalational anesthetics during early spermatogenesis.

The authors studied anesthetic mutagenesis following exposure in vivo by use of an adaptation of the mouse spermatozoa morphology assay of Wyrobek and Bruce. The epididymal spermatozoa of (C57B1/C3H)F1 mice were examined for morphologic abnormalities following exposure to near-0.1 MAC and greater concentrations of general anesthetics. Twenty exposure hours (4 hr/day x 5 days) were conducted for nitrous oxide, diethyl ether, chloroform, trichlorethylene, halothane, methoxyflurane, enflurane, and isoflurane, each at two concentrations. Twenty-eight days after exposure, epididymal spermatozoa wer

Mechanisms of chronotropic effects of volatile inhalation anesthetics.

The chronotropic effects of all currently available volatile anesthetics were investigated in isolated rat atrial preparations. Anesthetic ethers, diethyl ether, methoxyflurane, and enflurane elicited a dose-dependent positive chronotropic effect. Fluroxene produced a slight depression at low concentrations. The halogenated hydrocarbon anesthetics, halothane, chloroform, and trichloroethylene, did not show a uniform pattern. Halothane's effect was small and biphasic. Chloroform caused a dose-dependent decrease in heart rate, and trichloroethylene caused a marked positive chronotropic effect. T

Acute pain management in older adults presenting to the emergency department: a clinical review.

Managing acute pain in older adults is a complex challenge in emergency care, influenced by age-related physiological changes, multimorbidity and altered pharmacokinetics. Inadequate treatment contributes to frailty, delirium, functional decline and reduced quality of life, yet oligoanalgesia remains prevalent. Accurate assessment is essential, using tools such as the numeric rating scale, Pain Assessment in Advanced Dementia and Abbey Pain Scale, according to cognitive status. Treatment prioritises acetaminophen as first-line therapy, with cautious use of nonsteroidal anti-inflammatory drugs

Sickling dynamics of red blood cells and other physiologic studies during anesthesia.

Nineteen patients with electrophoretically proven sickle cell diseases were studied prospectively at intervals, using halothane, methoxyflurane, cyclopropane, and fluroxene. Simultaneous venous and arterial samples were taken from the same arm in heparinized syringes at the sampling intervals and analyzed for blood gases, hematocrit, and percent sickle cells. Measurements of blood pressure, pulse, temperature, tidal volume, minute ventilation, and other significant data were recorded. There were no anesthetic deaths, and the morbidity seen was an extension of pre-existing pathology. Sickling w

General anesthesia in pleasure horses.

Anesthetic management of the pleasure horse consists of the appropriate selection and administration of pre-anesthetic medications including anticholinergics, tranquilizers and narcotics followed by appropriate techniques of anesthetic induction. The anesthetic induction must vary somewhat for the pleasure horse practice since many of the procedures are completed on farms and ranches. As a result the inducing of anesthesia will frequently be with the same agent which will be used to maintain anesthesia. Noticeably will be the reduction and duration of anesthesia and surgical time. The inductio

Glutathione depletion following inhalation anesthesia.

Glutathione depletion following inhalation of halogenated anesthetics was investigated as a possible mechanism of toxic reactions associated with anesthesia. Concentrations of reduced glutathione were measured in the blood, liver, lung and kidney of the mouse after anesthesia with enflurane, fluroxene, halothane, isoflurane, methoxyflurane, or trichloroethylene. The anesthetic had no effect on glutathione concentrations in tissues except when fluroxene was used. After two hours of fluroxene anesthesia, glutathione in liver, lung, kidney, and blood was depleted by 93, 85, 85, and 61 per cent, r

A further study of general anaesthesia for Caesarean section.

Methoxyflurane or trichloroethylene in concentrations of 0.2% or 0.1% used in anaesthesia for Caesarean section were compared. There were no differences in the acid-base state or the clinical condition of the infant at delivery, or in the incidence of maternal complications, which could be related to the choice of volatile agent. Either volatile agent in the 0.2% concentration was associated with an overall incidence of maternal awareness and unpleasant dreams of 2.5%, compared with 4.9% when the 0.1% concentration was used. The incidence of maternal nausea and vomiting was less in the 0.2% se
