A case of congenital tethered cervical spinal cord is presented in a young adult. Metrizamide computerized tomography was the most useful imaging technique for identifying the tethered spinal cord. Intraoperative somatosensory evoked potentials correlated well with clinical improvement following surgery.

Computer-assisted myelography (CAM) is a technique in which computed tomography (CT) is combined with the intrathecal administration of metrizamide to demonstrate the spinal cord and surrounding structures. This retrospective study of 139 CAMs performed at Emory University Hospital included 125 CAMs that were preceded by routine metrizamide myelography (secondary CAM). The remaining 14 CAMs were primary studies without concomitant myelography. A new water-soluble contrast medium, metrizamide (Amipaque) was used to visualize the subarachnoid space within Meckel's cave. By a needle introduced through the foramen ovale, successful filling of the trigeminal cistern was accomplished in 48(80%) of 60 patients. Transoval trigeminal cisternography offers a possibility to control the position of the tip of the needle in various blocking procedures of the trigeminal ganglion, and may also be used for target localization in stereotactic radiosurgery in the treatment of trigeminal neuralgia.

A rare case of von Hippel-Lindau disease showing only spinal cord symptoms is reported. This 18-year-old boy was admitted to our clinic because of urinary disturbance, slowly progressive muscle weakness and sensory disturbance of his body and four extremities. Myelography demonstrated an expanding spinal cord and a figure of complete block of contrast medium at the lower cervical level. Dilated syrinx below the level of Th-3 was shown by delayed scan (19 hr.) of metrizamide CT myelography. Eleven patients suffering from trigeminal neuralgia were treated by percutaneous glycerol trigeminal rhizolysis after visualization of the trigeminal cistern with metrizamide. In 10 of 11 cases treatment was successful and the pain disappeared. The technique of the treatment is described. This form of treatment is recommended as a successful and simple method, especially in the elderly population suffering from trigeminal neuralgia.

A case of intraspinal metastatic carcinoma that presented on combined cervical and lumbar puncture metrizamide myelography as complete blocks at T2 and T12 is described. The extent of disease in the long segment of the spinal canal was inaccessible to conventional myelography. This case illustrates the importance of postmyelogram CT for accurate assessment of disease and for treatment planning.

A 10-year-old boy presented with marked growth retardation. He was found to have an empty sella demonstrated by CT and Metrizamide cisternography. Endocrinological investigation confirmed the diagnosis of panhypopituitarism. This is the first case reported in a twin and the fifth pediatric case with marked endocrine dysfunction reported in the literature.

Experimental studies and some clinical investigations have indicated that Amipaque (metrizamide) is less cardiotoxic than other contrast media in common use for angiocardiography. This study of 75 anatomical specimens and their comparison with contrast metrizamide radiography of the terminal dural sac reveals the exact topography, height, volume and morphology of the dural sheath and for the nerve roots their disposition in the terminal sac, the level of the root emergence and anatomical and radiological views of the foramina above and below the emerging roots.

Seven cases are reported in which syringomyelia was confirmed by neuroradiological studies and surgical procedures. Dissociated sensory loss was noted in four patients in this series. Peripheral paresis and muscular atrophy in arms and hands were found in all seven patients. In only two of the patients the wide spinal canal were found in the lateral view of plain cervical spine films. Assimilation of the atlas to the occipital bone was seen in two patients. A case of large intracranial lipoma involving the suprasellar cistern is presented in a 50-yr-old man with a history of occipital headaches. Evaluations by routine computed tomography (CT) combined with metrizamide CT cisternography were extremely useful in the diagnosis of this rare condition. The non-ionic water-soluble contrast medium Amipaque (metrizamide) shows a lowered neurotoxicity, and therefore it may be used in the entire spinal canal. The better visibility of fine structures compared with oily and gaseous contrast media allows an excellent diagnostic approach to all spinal affections. Our technique of myelography is described and typical examples are given.

A rare cause of superior mediastinal widening from traumatic extravasation of cerebrospinal fluid is reported in two children following automobile accidents. Radiographic findings include widened superior mediastinum evolving during the immediate posttrauma phase, pleural effusion, and CSF leak into the superior mediastinum on metrizamide CT myelogram. Both children had neurologic deficits of the lower extremities.

A rare case of diffuse thickening of the cervical dura mater leading to cord compression in Maroteaux-Lamy syndrome (mucopolysaccharidosis VI) is reported. Spinal computed tomographic scans after intrathecal metrizamide injection and magnetic resonance images are shown. This condition presents as cervical myelopathy. Early decompression can be beneficial for patients with this infrequent metabolic disorder.

The orbital contents of 28 baboons are defined by computed tomography (CT), which provides a simple, reliable, reproducible, and accurate technique for the experimental investigation of the orbit of the nonhuman primate. This method is applied to the diagnosis of exophthalmos. A 55-year-old woman presented with severe recurrent headache accompanied by dizziness. The brain CT scan showed multiple low-density cystic lesions in the suprasellar and left sylvian cisternae with total empty sell syndrome. The communication between the cisternae and the suprasellar cyst was not verified on the metrizamide CT scan. Treatment with praziquantel resulted in headache inilially and a rise in specific IgG.

An 11-year-old girl with repeated pneumococcal meningitis after head injury is reported. High resolution CT scan with metrizamide cisternogram disclosed a fracture in petrous bone with collection of contrast medium in middle ear. Operative repair of the dura defect successfully stopped further intracranial infection. In cases of late posttraumatic infection, aggressive survey of the skull base is indicated.

A patient with syringomyelia and syringobulbia secondary to an asymptomatic ependymoma of the cauda equina involving the conus medullaris is described. Delayed metrizamide computed tomography myelography was decisive for making the diagnosis of a highly extended syrinx associated with the tumor. Syringomyelic symptoms improved remarkably following the removal of the tumor, and the syrinx was not visualized in a postoperative study. The pathogenesis of syringomyelia associated with a caudally located spinal cord tumor is discussed.

A 49-year-old man with an epidermoid tumor had a hemifacial spasm on the left and ipsilateral trigeminal neuralgia--i.e., painful tic convulsif. Computed tomography scanning after metrizamide enhancement clearly demonstrated a cerebellopontine angle tumor. In the year since complete removal of the epidermoid tumor, the patient has been relieved of the facial pain and the hemifacial spasm is improved with decreased frequency of the spasm.

A new technique has been designed to improve myelography examinations of the entire lumbar spinal canal in patients with severe spinal stenosis using a single needle puncture. When a high-grade obstruction to the caudal flow of contrast material is encountered, the patient is placed in a flexed sitting position for 1 minute.
