Introduction:S. aureus is one of the most prevalent potential pathogenic bacteria found in DHM. Some strains produce heat stable enterotoxins that are able to survive pasteurization. These enterotoxins have been associated with gastritis and potentially necrotizing enterocolitis in preterm infants. The aim of this study is to assess the impact of different storage temperatures on Staphylococcus aureus growth and enterotoxin A and B production in raw and Holder pasteurized donor human milk (DHM). Materials and Methods: Raw and pasteurized DHM were inoculated with enterotoxin A and B producing S

BACKGROUND: Banked donor milk (BDM) has historically been used as an alternative to formula for preterm infants. Recently, BDM has been endorsed by two national organizations for use in healthy infants. We sought to quantify utilization trends and characteristics of mothers and their healthy newborns who received BDM during their postpartum stay between 2013 and 2016 at a single academic medical center.
MATERIALS AND METHODS: In this observational study, we used a clinical log to identify all infants who received BDM in the well-baby nursery between July 2013 and June 2016. From this log, we

BACKGROUND: Food-specific immunoglobulin G4 (FS-IgG4) is associated with eosinophilic esophagitis (EoE); however, it is not clear whether production is limited to the esophagus.
AIMS: To assess FS-IgG4 levels in the upper gastrointestinal tract and plasma and compare these with endoscopic disease severity, tissue eosinophil counts, and patient-reported symptoms.
METHODS: We examined prospectively banked plasma, throat swabs, and upper gastrointestinal biopsies (esophagus, gastric antrum, and duodenum) from control (n = 15), active EoE (n = 24), and inactive EoE (n = 8) subjects undergoing up

Compare with preterm formula, donor human milk (DM) is associated with a lower risk of mortality and morbidity in preterm infants. It is thus deemed superior to preterm formula as the sole diet or supplement to own mother's milk (OMM) for preterm infants, especially for those with very low birthweight (VLBW). This historic cohort study investigated the relationship between DM availability, and enteral feeding, body growth of VLBW infants by comparing two cohorts before and after the establishment of a human milk bank. A sub-analysis was also conducted between small-for-gestational-age (SGA) an

BACKGROUND: Human milk banks (HMBs) provide sterilized donor milk (DM) for the feeding of preterm infants. Most HMBs use the standard method of Holder pasteurization (HoP) performed by heating DM at 62.5 °C for 30 min. High hydrostatic pressure (HHP) processing has been proposed as an alternative to HoP. This study aims to evaluate intestinal barrier integrity and microbiota composition in adult mice subjected to a chronic oral administration of HoP- or HHP-DM.
METHODS: Mice were treated by daily gavages with HoP- or HHP-DM over seven days. Intestinal barrier integrity was assessed through in

Objective: The study aimed to identify how, from the perspective of bereaved parents, hospital-based health professionals can better meet their lactation care needs. Methods: In-depth interviews were conducted with 17 mothers and 7 fathers bereaved by stillbirth, neonatal death, or older infant death. Participants were recruited from three large hospitals in Eastern Australia including two with human milk banks. Qualitative thematic data analysis identified bereaved parents' lactation experiences, needs, and how parents wanted lactation care to be provided. Results: Participants experienced la

Nosocomial infections are a frequent and serious problem in extremely low birth weight (ELBW) infants. Donor human milk (DHM) is the best alternative for feeding these babies when mother's own milk (MOM) is not available. Recently, a patented prototype of a High-Temperature Short-Time (HTST) pasteurizer adapted to a human milk bank setting showed a lesser impact on immunologic components. We designed a multicentre randomized controlled trial that investigates whether, in ELBW infants with an insufficient MOM supply, the administration of HTST pasteurized DHM reduces the incidence of confirmed

INTRODUCTION: Donor human milk (DHM) is an alternative to formula for supplementation of breastfed newborns and has been associated with higher in-hospital exclusive breast milk feeding rates. Its use has increased substantially, most recently among term newborns, yet there is scarce data to describe patient characteristics and volumes administered. We aimed to characterize DHM utilization in our level 1 nursery.
METHODS: We conducted a retrospective cross-sectional study of newborns discharged in 2022 from the level 1 nursery. Variables included sex, gestational age, birth weight, delivery t

The use of donor human milk to provide therapeutic benefit to infants should only proceed where there is positive 'value'. This can be determined through an assessment of the benefit and the known risks. The emergence of new products derived from human milk requires new value assessments. The known hazards in human milk are modified by differences in the donor selection, processing methods and intended use and result in a unique risk assessment where any of these factors vary. The human source of the raw product requires high ethical standards in the design of these services with care taken to

The relationship between breast-feeding and later cardiovascular health has been investigated in randomised trials and observational studies. This review focuses on randomised control trials, regarded as the 'gold standard' in establishing causal relationships between interventions and outcomes. Since it is not ethical to randomise healthy term infants to be breast- or formula-fed, only two randomised control trials have examined effects of breast-feeding on later health. In one randomised control trial, preterm infants randomised to receive banked donor breast milk had significantly lower blo

Tunnel ventilation is an increasingly popular approach to mitigate the effects of heat stress on dairy cattle. Tunnel-ventilation barns use a bank of high-power fans to move air horizontally from one end of the barn to the other at cow level. The overall objective of the present experiments was to determine whether tunnel ventilation is superior to housing with fans and sprinklers with respect to rectal temperature during heat stress and seasonal variation in milk yield. In the first study, rectal temperatures were measured for 1097 lactating Holstein cows in six freestall barns with fans and

BACKGROUND: Donor human milk (DHM) became available in Japan when the first human milk bank was established in 2017. This study investigated the effects of DHM on enteral nutrition (EN) in very low birth weight (VLBW) infants in the single center in Japan.
METHODS: Seventy-six VLBW infants hospitalized between April 2017 and March 2020 at Showa University Hospital were included in the study. We retrospectively evaluated age (hours) at which EN was initiated and age (days) until complete feeding (EN > 100 mL/kg/day) was achieved. We compared the DHM and non-DHM groups, or the early human milk

INTRODUCTION: Breast milk banking (BMB) serves as a vital intervention to support neonatal health, particularly for preterm and low-birth-weight infants. However, cultural, attitudinal, and knowledge-based barriers affect its acceptance. This study assessed mothers' knowledge, attitudes, and willingness regarding breast milk donation (BMD) and donor breast milk (DBM), exploring factors influencing these aspects and the mediating role of attitude.
METHODS: A cross-sectional analytical study was conducted from November 2021 to July 2022, involving 400 mothers selected through convenience sampli

Cronobacter sakazakii and Enterobacter cloacae exhibit the ability to form biofilms, making them resistant to drying, antibiotics, and changes in pH. These biofilms can adhere to different surfaces, including tissues, catheters, enteral feeding tubes, and work surfaces, potentially leading to cross-infection risks in both the food and medical sectors. The objective of this study was to develop a predictive model of biofilm formation over time by C. sakazakii and E. cloacae on medical polyvinyl chloride (PVC) at 37 °C and stainless steel (SS), polypropylene (PP) surfaces at 4 °C and different m

The policy landscape influences both the delivery of health care and patient outcomes. Governmental funding priorities, legislation and regulation impact aspects of neonatal intensive care, which subsequently affect rates of NEC. We review state policies, including Medicaid coverage of pasteurized human donor milk and pending federal legislation such as the reintroduction of the 2019 Access to Donor Milk Act, which aim to reduce disparities in access. We also review federal appropriations, regulations and other measures, which may directly or indirectly, influence NEC rates. Together, these me

BACKGROUND: Mother's own milk (MOM) improves in-hospital outcomes for preterm infants. If unavailable, donor milk (DM) is often substituted. It is unclear if DM vs. formula to supplement MOM is associated with improved in-hospital outcomes in term/late preterm surgical infants with gastroschisis or intestinal atresia.
METHODS: This retrospective study included infants born ≥33 weeks gestational age (GA) with a birth weight of >1500 g who were admitted to a quaternary neonatal intensive care unit (NICU). Using Chi square and Mann-Whitney u testing, we compared hospital outcomes (length of stay

Numerous short and long-term benefits of breastfeeding the full-term infant have been acknowledged. The use of human milk in the neonatal intensive care unit (NICU) is emerging as the beneficial effects are being realized. Challenges facing the practitioners today include providing optimal storage and processing strategies for mother's own milk (MOM), as well as offering an alternative when MOM is unavailable or supply is insufficient. We review the health benefits of human milk for the ELBW infant and the strategies for optimal use of the milk in the NICU.
