[Current treatment of focal hyperhidrosis. Surgical approaches].

A variety of conservative and surgical therapy options are available for the treatment of focal hyperhidrosis. In case of severe therapy-resistant forms, surgery aims to achieve permanent reduction of sweating. For axillary hyperhidrosis, local surgical techniques are the method of choice with minimally-invasive techniques favored in recent years. Sympathectomy is the ultima ratio for palmar hyperhidrosis; however, the procedure can lead to severe side effects. For most surgical techniques for hyperhidrosis, both long-term studies and side-to-side comparisons are not available.

Lower urinary tract fistula: the minimally invasive approach.

PURPOSE OF REVIEW: A variety of fistulas occur involving the lower urinary tract with adjacent organs namely the vagina, uterus, rectum and colon. Most of these arise out of surgical complications and contribute significantly to the morbidity of the procedures. Surgical reconstruction remains the mainstay in the management. This article reviews the use of minimal-access procedures in reconstruction of lower urinary tract fistulas focusing on the bladder. RECENT FINDINGS: Recently, numerous reports of laparoscopic and robot-assisted surgical repair of these fistulas have shown that these techn

[Nursing care related to the "Nuss procedure", a minimally invasive repair of pectus excavatum].

The "Nuss procedure", a new minimally invasive repair of pectus excavatum, is an innovative of surgery and thoracoscopy combined with the proper placement of the bar to achieve correction of pectus excavatum. Patients with significant pectus deformities often experience embarrassment and shame due to perceived abnormalities in their appearance. The Nuss procedure can help them improve their body image by achieving excellent cosmetic results. This article introduces traditional treatment approaches to pectus excavatum repair and the principles underlying the Nuss procedure as well as indication

Subfascial endoscopic perforating vein surgery (SEPS) for the treatment of venous ulcers.

Lower extremity ulcerations that result from venous hypertension are a significant cause of disability in Western nations. Venous ulcers, highly related to lower extremity venous valvular incompetence and post-thrombotic syndrome, demonstrate a protracted course of healing with a high recurrence rate when managed conservatively. Effective treatment includes correcting the elevated lower extremity venous pressure using non-invasive (compression therapy) or invasive modalities (removal or correction of incompetent venous segments, most commonly the greater saphenous vein). Minimally invasive sub

Anatomic basis for a minimally invasive approach to the subtalar joint.

INTRODUCTION: An isolated arthrodesis of the talocalcaneonavicular joint is a common indication in cases of pain and post-traumatic arthroses. MATERIALS AND METHODS: Because of the high infection rate after surgery with the lateral incision, the authors decided to evaluate an alternative, minimally invasive procedure. Joint destruction was carried out via a posterolateral access after the insertion of two guidewires. For an evaluation of the risk for vessel and nerve structures, 102 ankle joint specimens preserved in formalin/alcohol were examined. Additionally, the minimally invasive access

The actual role of classic bilateral cervicotomic approach for primary hyperparathyroidism in the era of minimally invasive surgery.

UNLABELLED: Recent advances in preoperative localisation of parathyroid adenomas and intraoperative prove of complete removal of hyperfunctioning parathyroid tissue have fostered less invasive operative procedures which directly target the diseased gland. Such strategies have partially replaced the previous gold standard procedure of bilateral neck exploration. We herein report on our own series of 1099 consecutive operations for primary hyperparathyroidism performed in a 16 year period and provide information and arguments for primary bilateral exploration in selected cases. 97.1% of patients

Updates in Minimally Invasive Cardiac Surgery for General Surgeons.

Significant improvement and development have occurred in minimally invasive cardiac surgery over the past 20 years. Although most studies have consistently demonstrated equivalent or improved outcomes compared with conventional cardiac surgery, with significantly shorter recovery times, adoption continues to be limited. In addition, cost data have been inconsistent. Further ongoing trials are needed to help determine the exact roles for these innovative procedures.

Minimally invasive fusion: summary statement.

Ideally, minimally invasive surgery (MIS) allows less extensive manipulation of surrounding tissues than a conventional open procedure while accomplishing the same goals and objectives at the target structure. Long-term follow-up combined with appropriate outcome measures are necessary to prove the safety and effectiveness of MIS. For MIS procedures to be widely adopted, they must have an acceptable learning curve. Special skills are needed and are beyond those of traditional open surgery, By definition, as compared with conventional open surgeries, minimally invasive procedures typically invo

Minimally Invasive Surgery and New Technologies in Pediatric Oncology.

Minimally invasive surgery (MIS) has become a cornerstone in many areas of pediatric surgery, with proven benefits including reduced postoperative pain, shorter hospitalization, faster return to normal activity, and improved cosmesis. In pediatric surgical oncology, however, the application of MIS has historically been limited by concerns about compromising oncologic principles, the technical challenges posed by small anatomy, and the lack of robust data compared to adult series. Over the past 2 decades, advances in technology, perioperative care, and collaborative research have led to a gradu

Conventional Total Thyroidectomy Using a Novel Single Retractor: Technical Note for Apollo®.

Despite the increasingly innovative techniques developed in thyroid surgery to offer patients minimally invasive and scarless interventions, conventional open procedures still account for most of the interventions performed in this field. The surgical incision length has been significantly reduced, from 6-9 cm to 3 cm, and therefore patients perceive the scar to be highly acceptable. In this technical note, we present the use of a new single retractor (APOLLO®; AFS MEDICAL GmbH, Teesdorf, Austria) for conventional open thyroidectomies with intraoperative neuromonitoring. This device offers sev

Minimal Incision/Minimally Invasive Medializing Displacement Calcaneal Osteotomy.

UNLABELLED: Minimally invasive techniques are readily applicable to calcaneal osteotomies and have the potential to accomplish hindfoot correction equivalent to open procedures with less morbidity and pain. Use of a guidance jig makes the procedure more predictable. While most anatomic features of the procedure are the same as those with open techniques, special care must be taken to avoid neurovascular injury because there is no open exposure. Anatomic guidelines have been established for appropriately localizing the osteotomy. LEVEL OF EVIDENCE: Level V, expert opinion.

Minimally invasive component separation techniques in complex ventral abdominal hernia repair: a systematic review of the literature.

PURPOSE: Open component separation (CS) has traditionally been a popular method for management of complex abdominal wall hernias. However, it has been associated with significant wound complications. The aim of this systematic review was to examine the evidence in the literature on modern CS techniques (endoscopic and minimally invasive) for the management of such hernias. METHODS: A systematic literature search was performed (2000 to 2013) using major electronic databases (PUBMED, EMBASE). The outcomes of interest were hernia recurrence rate and wound morbidity. RESULTS: A total of 33 publi

[Therapy of pilonidal sinus--is more always better].

Nowadays the etiology of pilonidal sinus is explained as an acquired disease in form of ruptured hair follicles. It occurs most frequently in the proximal rima ani. Pilonidal disease is common in young men. Acute abscess formation and chronic discharge from the fistula represent indications for surgery. Surgical therapy has evolved from radical excision followed by months of open wound healing towards procedures causing less morbidity. In case of an acute abscess it is important to separate time-wise incision of the abscess in local anaesthesia from definitive excision of the fistula. Two surg

[Strategies and innovative practices of European minimally invasive surgery for gastroesophageal reflux disease].

Gastroesophageal reflux disease (GERD) is a common esophageal disorder worldwide and can progress to erosive esophagitis, Barrett's esophagus, and even esophageal adenocarcinoma. Long-term use of proton pump inhibitors cannot fundamentally correct the functional impairment of the anti-reflux barrier, and symptom control is inadequate in approximately 40% of patients. Exploring individualized, standardized, and innovative anti-reflux surgical strategies is conducive to enhancing therapeutic efficacy and improving patient prognosis. Based on existing literature and European clinical practice exp

The New Minimally Invasive Techniques for Pilonidal Sinus Disease Surgery: Choosing the Right Technique for the Right Patient-A Narrative Review.

Background and Objectives: Pilonidal sinus disease (PSD) is a common condition affecting predominantly young adults and it is associated with a substantial impact on quality of life. Despite the wide range of available surgical options, optimal management remains controversial due to heterogeneous recurrence rates, wound complications and prolonged recovery. Over the last decade, a paradigm shift has occurred from midline excision toward off-midline closure techniques and minimally invasive procedures. This narrative review aims to critically appraise contemporary surgical strategies for PSD,

[Challenges and strategies in minimally invasive pancreatic enucleation].

Minimally invasive enucleation of pancreatic tumors has become a focal topic in the field of pancreatic surgery. This technique, which allows for complete tumor removal while preserving maximal pancreatic function, has seen widespread application in clinical practice in recent years. Preoperative evaluation is essential, requiring a thorough assessment of the necessity, feasibility, and appropriateness of surgery, and a careful choice between follow-up observation, parenchyma-sparing resection, or radical resection. If the lesion carries a potential risk of malignancy, radical resection, such

Applications of endoscopic techniques in spinal oncology: A systematic review of the contemporary literature.

INTRODUCTION: At the time of its origination, endoscopic spine surgery (ESS) was introduced to advance minimally invasive spine surgical (MISS) techniques, aiming to reduce tissue traumatization and improve clinical outcomes. Initially, ESS was primarily applied to transforaminal diskectomies for treating low back pain and radiculopathy caused by lateral or posterolateral disc herniations. Early challenges included limited visualization, suboptimal camera systems, and restricted maneuverability of rigid endoscopes. Technological advancements, such as angled endoscopes and interlaminar approach
